on the mobile home used as their principal residence.

Applications for General Assistance and Emergency Assistance can be obtained at the township

office during regular business hours for those residents living within the Plainfield Township

boundaries. Emergency financial aid may be provided to qualified recipients to alleviate a life-

threatening circumstance or meet basic needs such as food, rent, utilities, etc. once in a 24-

month period to Township residents who meet the eligibility requirements, with a lifetime

maximum of five (5) times.

This program is available to residents of the Village of Plainfield caring for any person diagnosed

with Autism, Alzheimer’s, or other mentally debilitating disease that may subject them to be

disorientated and/or wander from their caregiver. The Care Trak Program provides participants

with a water-resistant transmitter bracelet. This transmitter gives searchers a tool to help find

the participant more quickly.

Pharmaceutical Indigent Patient Programs

Illinois Department on Aging Senior Helpline – (800) 252-8966

This program allows a physician to obtain certain prescription medications from participating

manufacturing companies for eligible patients at a reduced cost or no cost to the patient. Each

pharmaceutical manufacturer that chooses to participate in the Indigent Patient Program

establishes patient eligibility requirements, as well as the process the physician is to follow

when requesting assistance for a patient. Requests are accepted from licensed physicians only.

Many doctors are unaware of the program and not all pharmaceutical companies participate.
