14-CSA-100

A. GENERAL

1. Description of Service

DCOSS Custom Service Arrangement provides service under the terms and conditions set forth in the General Exchange Tariff, Part II, Section 41, Sheet 1. Communication within the DCOSS is provided over station lines, which also provide access to system features and special services which are inherent in the common equipment and, therefore, considered as basic. Access to and from the Local Exchange and Toll Networks is provided over Exchange Access Lines in accordance with Local Exchange Tariffs, Part X, the number of which will be determined by Telephone Company traffic studies. In addition to Exchange Access Lines, the Customer shall subscribe to Direct Inward Dialing service in accordance with Local Exchange Tariffs, Part X and Telephone Numbers in accordance with General Exchange Tariffs Part II, Section 34. The customer is provided with primary and additional listings in the Telephone Company’s Directory. Such listings are subject to regulations and rates set forth in Tariffs Part II, Section 4, Directory Listings.

B. RATES AND CHARGES 1. General

The schedule of rates as set forth following includes equipment to provide DCOSS to this custom service arrangement.

3. Cancellation Charges Before Installation

In accordance with Tariffs Part I, General Regulations 6.A, if a contract for DCOSS is canceled in whole or in part prior to complete installation or start of service, the customer is responsible for payment of the non-recoverable expenses incurred by the Telephone Company in connection with the order, not to exceed the One Time Charge as set forth in Section B.2. In addition, non-recurring charges as set forth in the Tariffs Part II, Section 22 shall apply for all work completed.
