A flexible, person centred service in a thriving community

At Consensus, we are committed to ensuring that time, attention and

care is given in the selection process and the sensitive transition

for individuals moving to the service. This ensures that the selected

service environment is conducive to help meet their individual needs,

aspirations and wishes. Transitions into the service are gradual

for some individuals; a visit for dinner, a day visit or perhaps an

overnight stay to help them feel at ease during the transition process.

The staff at Consensus are committed to providing quality services

and continuous improvement in all levels of support. King Edwards

Grove support results in positive outcomes for individuals and

promotes their active participation. An appropriate ratio of staff to

individuals is maintained ensuring the required level of support

is available whilst encouraging independence. Staff are invited

to continually develop their skills, specialism and support for

individuals based on the support they require.

Support staff work extensively with people using the service to

establish their personal likes, dislikes, strengths, needs, goals and

aspirations. Through identifying individual goals, a person centred

support plan can be implemented to support them to achieve their

most aspirational outcome. The team are committed to maintaining

close contact with the individual’s family ensuring all are involved in

developing the person centred support plan. Staffing rotas are drawn

up to provide flexible support during the evenings and weekends

to facilitate their choice of activities; an essential part of the support

team function.

The promotion of choice, person centred planning and empowerment

are an integral part of Consensus’ ethos. Ensuring people using

the service are included in all stages of service delivery provides

opportunities for individuals to take control and enhance their overall

quality of life.

Teddington, Richmond Upon Thames

Located in Teddington in the London Borough of Richmond Upon Thames, King Edwards Grove offers accommodation for eight individuals with learning disabilities, autism and associated mental health and complex needs.

This Consensus service provides eight bright and

contemporary styled en suite bedrooms. There

are a number of communal areas including a large

kitchen, dining room, two lounges and a computer

room. Individuals using the service are provided with

opportunities to embark in new interests should they

wish to do so. Activities such as swimming, going to the

cinema, bowling, farm visits, museum excursions and

paid or voluntary employment are actively participated by

those using the service. Each individual is also supported

to plan and enjoy a minimum of one holiday per year of

their choice.

The main aim for staff at King Edwards Grove is to

offer support and guidance for individuals with learning

disabilities. People using the service are encouraged by

staff to enjoy life and live it to the full in a safe, happy

and fulfilling environment. Appropriate person centred

support plans are developed in consultation with each

individual to reflect their needs and aspirations. Our aim

is to increase confidence and nurture independence for

all individuals we support by keeping them informed of

their choices, responsibilities and rights within both the

local and wider community.

A structured weekly programme is generated by

agreement with each individual; a weekly activity plan is

developed with an individual’s key worker or team leader.

Our aim is to provide individual care, promote personal

development and foster a valued lifestyle within a

comfortable homely environment. We believe by creating

a supportive environment, all individuals with a learning

disability have the ability to meet their full potential, live in

the community and experience valued lives.

Information for care professionalsKing Edwards Grove provides a highly personalised, safe and inclusive environment. The outcome-focused support delivered is based on developing key social, communication and daily living skills with a view to gaining greater levels of independence.

Those we support are encouraged to positively contribute to the day-to-day running of the service and open communication with those we support and their loved ones ensures each service continually evolves and improves. Our services use ‘Total Communication’ to ensure that we select the best combination for each person’s needs. This could be through speech, signing, touch, body language, objects, photographs, writing or any combination of these methods.

Each service is specifically adapted as required to meet any additional needs such as autism spectrum condition, sensory needs, limited mobility, physical

disabilities and complex health conditions. Each service works closely with the local community to identify potential social, educational and employment opportunities to offer choice to those who use the service. Through Person Centred Planning, each individual is then supported in accessing such opportunities.

All costs quoted are fully inclusive of access to our in-house Positive Behaviour Intervention Team, ensuring specialist support and reduced opportunities for placement breakdown.

At Consensus, our commitment is to provide every single person we support with the framework to identify the opportunities that really are available to them, make considered choices about what they want to achieve and work in partnership to successfully achieve those outcomes.

Positive Behaviour Intervention TeamAt a time when many providers are reducing their specialist support teams, Consensus are investing heavily in developing and equipping our team members with the skills they need to thrive in their roles, along with reducing the opportunities for placement breakdowns.

Our Positive Behaviour Intervention Team (PBIT) comprises of care professionals with decades of relevant experience in supporting people who have behaviours that may present challenges. The team is managed by a Lead Behaviour Practitioner and includes four further behaviour practitioners who support services throughout the UK.

The methodology used is ‘Applied Behaviour Analysis’ (ABA) providing Functional Assessment and intervention strategies to reduce challenging behaviours. The team also provides staff advice, support, mentoring and training in Positive Behaviour Support; Autism Spectrum Conditions; Self Injurious Behaviours; Intensive Interaction and TEACCH. In addition, the team supports initial assessments

of complex new referrals to Consensus services and transitions to new developments.

The Positive Behaviour Intervention Team’s (PBIT) values are based on a person centred approach and works very closely with the Inclusion Lead who promotes PCP training and supports the PCP champions network. Furthermore, we have a robust approach to clinical governance, led by our Clinical Director, which enables us to go above and beyond what the regulators require. We constantly seek to build on excellence in everything we do.

We have adopted a partnership practice for the development of our new services. Through this approach we are able to deliver innovative support services that meet

a real need in the local area.

Information for family & friendsWe take our responsibility towards both you and your loved one very seriously. We know that you probably know your loved one better than anyone and we value your involvement in the support we provide. It is our intention that through open communication you will continue to play a key role in their life.

We strive to ensure that you and your loved one are included in all decisions relating to the service, including who provides the support and how it is delivered. Our first concern is that people we support have choice and control over their life. We encourage and advocate those who want to be involved in planning and organising their own support, but we also understand that some people would like others to do it for them.

We appreciate the importance of knowing each and every person we support. Only by doing this, can we begin to explore what they respond positively to and then recognise their individual successes. As such, we thoroughly assess each person we support and work with them to develop a Person Centred Plan covering every aspect of the life they live and, more importantly, the life they want to live.

This Person Centred Plan focuses on who and what is most important to your loved one. It helps us to get to know them, design a service individual to their needs and understand how best to communicate with them. Quality of communication can affect self-esteem, confidence and decision-making. We use ‘Total Communication’ to ensure that we select the best combination for each person’s needs. This could be through speech, signing, touch, body language, objects, photographs, writing or any combination of these methods.

The Person Centred Plan sets out what your loved one wants to achieve, whether it be in the next two weeks or the next two years. It then helps to map out the steps needed to achieve their goals and what each person involved in their life can do to support them along the way.

Safety and security is paramount within our service. We have invested heavily in designing a safe environment where those we support can develop their social, communication and life skills with a view to gaining greater levels of independence, including education and employment.

The support staff at King Edwards Grove are highly skilled in identifying and communicating risks to those they support, whilst empowered to be imaginative with the support they offer and the opportunities they provide. They are specifically trained to work with learning disabilities and associated complex needs including autism spectrum condition, mental health conditions, epilepsy and challenging behaviour.

A high emphasis is placed on healthy living within our services, with everyone encouraged and supported to complete and review their own Health Action Plan. This enables people to take ownership over their own health, and to think about how different aspects of their life contribute to their health. By actively contributing to this plan, each person can build knowledge that will enable him or her to make informed decisions in a more independent environment.

Ultimately, you want to know that your loved one will be happy, safe and fulfilled. At Consensus we work with individuals to put every opportunity and outcome in place to not only make that a possibility, but to ensure it becomes a reality.
