Regionally at the Champlain LHIN he facilitated the coordination of stakeholders in Renfrew County and led the new 30 million dollar “Aging at Home” initiative. In Barry’s Bay as COO of St. Francis Memorial Hospital, Jérémy led the collaborative effort necessary to create the rural health hub model. And now in Mattawa as the CEO of the Hospital and the LTC Home he is integrating both organizations and rebuilding the LTC to create its Health Hub. His leadership has also generated several integration projects such as the first merger in Ontario of a community health centre with a rural hospital; furthermore, he implemented an effective deficit elimination strategy. As is the case with effective leaders everywhere, Jérémy achieves results through team work where he demonstrates vision and the skills necessary to motivate others. Not only does Jérémy exercise his leadership professionally, he is very involved in his community at large where he volunteers as a Church Leader, coaches and runs sports leagues, and stays involved with the University of Ottawa MHA program and the Ontario Hospital Association (OHA) Leadership committees. Indeed in 2012, the OHA awarded Jérémy the Small, Rural and Northern Award of Excellence. His experience and learning to date have prepared Jérémy very well for continued growth in the field of health care management. His accomplishments deserve the Robert Zed Young Health Leader Award.

Section B: Report
1. Introduction
As the youngest Hospital CEO in the Province, Jeremy Stevenson is a healthcare leader who understands his capabilities, has established ambitious goals and has and has achieved results through a keen appreciation of engagement and collaborative effort.
2. Leadership Lead Self
o Responsible for the overall leadership and operations of the St. Francis Memorial Hospital (SFMH) in Barry’s Bay, as COO and of the Mattawa Hospital as CEO. Refer to Appendices 1 and 2 for an overview of the organisations;
o As integrated entities with the noted hospitals, responsible for the management of Long Term Care Home and Community Health Center;.
o Community leader working with youth.
Engage Others o Within two years, at SFMH facilitated and implemented a community vision of a
rural healthcare hub to consolidate health service providers. The project engaged 14 local and regional Health care providers to help create the Madawaska Communities Circle of Health, which resulted in several integration initiatives:
- Co-location of Home Support Services; - Preliminary work of co-management between the Valley Manor LTC Home
and the SFMH; - Service provider for the CCAC Nursing Clinic; Full integration of the
Community Health Center with the SFMH, - Development of Hospice beds within the hospital, - Staff and services sharing agreements with several partners.
Achieve Results
o Developed and implemented a strategic recovery plan to address a deficit during his first year at the SFMH. The Hospital ended the year with a surplus, and was in a surplus position for the last five years of Mr. Stevenson’s stay.
o Between 2007 and2010, as a Senior Planner, for the Champlain Local Health Integration Network, Mr. Stevenson led the Province’s 3 year “Aging at Home” initiative, the most significant new investment in the Champlain Region ($30 Million) in addressing the needs for Seniors. Developed the Directional Plan and the Strategy. Led and coordinated team of multidisciplinary professionals in the implementation and evaluation of over 30 regional and local integrated programs.

o This resulted in successfully receiving and collaborating on an annual $3.8 M multi-year regional initiative.
Systems Transformation o Led the project and chaired the South Renfrew Health Link to present the first
Business Plan in Champlain with all the health partners along HWY 60. As a result funding was received to implement the Health Link in South Renfrew County.
o As part of a team Jeremy has oriented SFMH to take on all transformation and integration opportunities, one example being one of the region’s first Health Links. In Mattawa, he continues to champion change.
o Quoted by his Hospital Board Chair: “Mr. Stevenson brings a well-needed collaborative leadership approach to healthcare, oriented towards the patient and Transformation. He has demonstrated success in creating a local health hub and integrating health service providers on one site.”
3. Responsibilities
Jeremy Stevenson currently is President and CEO of a Hospital and a Long Term Care Home. In addition to the complexities of managing two Boards in Mattawa, Mr. Stevenson also interacts with the overarching Board of the Catholic Health Corporation of Ontario (CHSO)/Catholic Health Corporation of Ontario (CHCO). Please refer to attached file to review the organisational chart (attached).
Mr. Stevenson was appointed to the OHA Small, Rural and Northern Hospitals Provincial Leadership Council – Impact: Advise and make recommendations to support the development of healthcare policy on issues pertaining to small and rural hospitals. As a young Master in Health Administration graduate, Mr. Stevenson was assistant to the Montfort Hospital’s CEO and was involved in the redevelopment to double the hospital square footage, increasing the beds 166 to 300. As Senior Planner at the Champlain Local Health Integration Network he led the Ontario’s 3 year “Aging at Home” initiative, ($30 Million) for the region. He was also responsible for the monitoring, system planning and communication in the Long Term Care Home sector in general, and specifically for 5 local Hospitals, 10 Long Term Care Homes, 13 Community Support Services, 3 Community Health Centers, 6 Mental health and Addictions Services and the Community Care Access Centre (CCAC). Jérémy is actively involved as a committee member organizing the University of Ottawa’s MHA 50th Anniversary Event and participates in the Annual MHA Preceptor Conference.
4. Creative approaches to Obstacles & Barriers
Integration Significant challenges in rural health care relate to access to care, sustainability and
collaboration made difficult by distance, size and isolation.

Jeremy Stevenson has implemented initiatives that usually take years, and received support and buy-in from local, regional and provincial partners to address these obstacles:
o Jeremy was instrumental in the creation of the first full community integration working group in Champlain
Impact: with partners identify, prepare and locally implement integration opportunities in collaboration with the LHIN.
o He secured funding from the LHIN to renovate the ambulance building permitting the co-location with Community Support Services, and Mental Health and Public Health.
Impact: efficiencies for all and a new revenue stream for SFMH; o The hospital staffed and operated the CCAC Nursing Clinic.
Impacts: decreased emergency room visits, increased at home services, improved quality of care and increased efficiencies.
o Feasibility Study to address the potential rebuilding and co-locating of LTC Home with SFMH.
o Implementation of a palliative care rural model and creation of a 2 bed Hospice within the hospital.
Impact: Residents are able to die in dignity with support either at home or in hospice according to their needs.
o Successfully integrated services and shared staffing: IT staff with existing partners: LTC Home, Community Health Centre, Community Support Services and Family Health Team.
Impact: full-time employment, better quality along continuum of care by same health worker, standardization, no duplication.
o Partnered with local pharmacy to provide medications to Emergency patient’s after hours
o Led the first Health Link approved in Champlain.
Mr. Stevenson was successful in the development and submission of new services and funding, totalling approximately $1 Million from the LHIN to drive these integration efforts and further build the local health hub. The Ministry has now adopted pilots to promote and replicate this type of rural healthcare model called the Rural Health Hubs.
Potential Loss of Services South/West Renfrew County was unable to provide sustainable primary care services.
The only Community Health Center in the area was not able to recruit a physician and was risking closure. As a solution, Mr. Stevenson led and facilitated the integration and merger of the CHC with the hospital (the first in the province). Within the first year, he was successful in recruiting three physicians to this rural CHC, and in completing needed capital improvements.
Financial During his first year, Jeremy developed and implemented a strategic recovery plan to
address SFMH’s deficit. The Hospital ended that with a surplus which was sustained

during his last five years at SFMH. Mr. Stevenson has implemented a Hospital Improvement Plan resulting in a 3% savings without cutting services or staff.
5. Accomplishments
Improvement in patient and quality outcomes Integration of the Algonquin Nursing Home with the Mattawa Hospital
o Impact: Sustainable service for the community. The Nursing Home has better access to physicians and the community benefits from an improved continuum of care due to shared staff. There are also savings and efficiencies for both organizations.
Partnership between the St. Francis Memorial Hospital & the Renfrew Victoria Hospital. o There is access in Barry’s Bay to many services that would otherwise require an
hour travel time. The first full merger of a Community Health Center with a hospital
o Outcome and impacts on patients: More sustainable primary care, more space for more clients, and the recruitment of three (3) physicians and one (1) Nurse Practitioner. An enormous success for its community.
Creation of a health hub: as explained in this report’s section 4: “Integration”. Leading one of the first Health Links approved in the Champlain LHIN.
o Impact: Providing coordinated care to patient’s with Complex Chronic Conditions along HWY 60.
Recipient of the 2012 Ontario Hospital Association’s Small Rural and Northern Health Care Award of Excellence in recognition of Mr. Stevenson and his team’s innovative leadership in collaborating and establishing partnerships to expand and improve health services in the Madawaska Valley.
Appointment to the Ontario Hospital Association’s (OHA) Small, Rural and Northern Hospitals Provincial Leadership Council.
Private Physiotherapy Clinic – Due to the lack and need of physiotherapy services locally, the SFMH has created a partnership with a Physiotherapy company and renovated space in its Health Center for a new clinical in Barry’s Bay. (Fall 2012)
SFMH was awarded full accreditation from Accreditation Canada and was considered for Exemplary Standing.

Eastern Ontario Regional Laboratory Association – was part of the 16 hospital partnership that creates the Champlain wide integrated and standardized laboratory services (Recipient of Award in recognition of contributions)
Involved in the creation of the Champlain Small Hospital CEO group – This resulted in successfully receiving and collaborating on an annual $3.8 M multi-year regional initiative.
6. College Involvement Mr. Stevenson has been a Member of the Canadian College of Health Leaders since 2011 (ID# 24559). Recognizing the value of the CCHL as Health Leaders’ professional body, Mr. Stevenson offers College membership in all executive contracts. In 2013 he was among the candidates considered for the CCHL Sweden Study Tour Emerging Health Leader Scholarship. Mr. Stevenson also became a member of the American College of Health Executives, helping to make links between the CCHL and ACHE on shared opportunities and learnings, such as at the 2014 ACHE Annual National Congress on Healthcare Leadership. Jérémy’s goal is to become a CCHL Certified Health Executive (CHE) 2016-17 Short-Term Goal:
Certified with the CHE designation (Spring of 2016 – Complete the CHE Program Application Form)
Publish an authored paper in the Communiqué College Newsletter on Integration Success and Rural Health Hub Models (Fall/Winter 2016)
5 year Long-Term Goal: Become a surveyor for Accreditation Canada to learn the best practices of other
organizations and their Leaders, while promoting and making the CCHL links to support. Mr. Stevenson has been involved as Preceptor for MHA students, and has encouraged them to be involved with CCHL for a stronger and united profession. For example, he has promoted and attended all of the CCHL events at the OHA HealthAchieve, and with MHA partnership events.

7. Conclusion Jeremy is the father of four. He is very involved and visible in his community where he volunteers as Church Leader, coaches, runs sport leagues and sits on the University’s MHA and on OHA’s Leadership committees. He is fluent in English and French.
At this early stage in his career, Jérémy Stevenson is already an accomplished health services leader. His capabilities, learning and experience to date have prepared Jérémy very well for continued growth in the field of health care management.

Hôpital Mattawa Hospital has a proud tradition of care. For more than 130 years, HMH has cared
for the people of Mattawa, surrounding communities, and beyond, being a significant player

I am writing you to nominate Jérémy V. Stevenson, President and Chief Executive Officer of the Mattawa Hospital and Administrator of the Algonquin Nursing Home, to the MHA Alumni Young Achiever's Award. I feel that having received the Ontario Hospital Association’s Excellence Award, and with his experience as Chief Operating Officer of the St. Francis Memorial Hospital (SFMH) and his recent appointment at the Mattawa Hospital & Algonquin Nursing Home (MH/ANH) he meets all criteria to be a successful candidate to receive this award and thus promote the success of the University of Ottawa MHA program.
I know Jérémy personally, as I have met him when he was a student in the MHA program and later in his career at the Montfort Hospital, at the Champlain LHIN and more recently at the SFMH. Furthermore, as I was actively involved in the recruitment of the President & CEO & Administrator position of the MH/ANH, I sought his application for this role as he offered the right combination of professional experience and education. Please find enclosed his résumé from that recruitment process.
In support of his application to this award, please note THE FOLLOWING additional highlights of Mr. Stevenson’s accomplishments:
Work experience:
Developed and implemented a hospital strategic recovery plan to address a significant forecasted deficit during his first year at the SFMH. The Hospital ended with a surplus that same year, and has been in a surplus position since then;

On behalf of the Ontario Hospital Association (OHA), I am pleased to congratulate you on your appointment to the Small, Rural and Northern Hospitals Provincial Leadership Council (Council). Your first term will commence at the next scheduled meeting to be confirmed for early March.
As you know, Council members provide advice to the OHA on health care policy issues pertaining to small, rural and northern (SRN) hospitals.
Over the past year, the Council has led a number of important activities on behalf of SRN hospitals, such as the development of the Local Health Hubs for Rural and Northern Communities report and the Small Hospital Working Session in May to discuss its implementation. The Council also published Enhancing Access through Integration in fall 2013, to highlight successful integration projects across the province. More recently, the Council provided advice to government with regard to the Health Infrastructure Renewal Fund (HIRF).

• Integrated the Algonquin Nursing Home with the Mattawa Hospital. Approved to redevelop a $16 Million Nursing Home collocated
and integrated with the Mattawa Hospital.

• On-site Administrator responsible for the overall leadership and operations of the SFMH hospital, which offers full acute care services,
including emergency, medical/intensive care, Complex Continuing Care, Dialysis, Radiology, Laboratory Services/ECG, Echos,
Mammography, Ontario Breast Screening Program, Ultrasound, Bone Density, Telemedicine, Diabetic Clinic, Surgical & ENT
Clinics, Internal Medicine, Physiotherapy, and Foot care. SFMH provides services to a catchment area of over a one-hour radius,
which includes 11 Townships.
• Within two years led, facilitated and implemented the vision of a rural healthcare hub to consolidate health service providers on one-
site: The creation of the Madawaska Communities Circle of Health, The co-location of Home Support and the Valley Manor LTC
Home with the SFMH, The hospital providing services for the CCAC Nursing Clinic, The full integration of the Community Health
Center with the SFMH, The creation of Hospice beds within the SFMH, and the sharing of staff and services with partners.
• Developed and implemented a hospital strategic recovery plan to address deficit during my first initial year at the SFMH. The Hospital
ended with a surplus that same year, and has been in a surplus position for the last five years.
• Serve as ex-officio member of the Board of Directors, Board committees and act as Secretary for the Board.
Member of the Senior Management Team, Renfrew Victoria Hospital 2010 – 2015
• Combined RVH and SFMH are facilities of 80-beds, 40,000 ER visits, with 600 employees, and annual operating budgets of $50.5M.
• Successful in the development and submission of new services and funding, totaling approximately $1 Million during the last year.
Chief Operating Officer, Rainbow Valley Community Health Center (CHC) 2011 – 2015
• Led and facilitated the first full integration and merger of a community health center with a hospital in the Province. Within the first
year was successful in recruiting three physicians to this rural CHC, and completing capital improvements.

• Transformation and redevelopment of the Hospital, which saw it double in square feet, going from 166 beds to 300 beds, and partner
with the Canadian Forces. Researched and analyzed the Hospital’s current operations and Functional Program, to ensure that
operationally the hospital was ready to occupy the new space.
• Developed future operating model to reflect most current and best practices;
• Led and coordinated a team of multidisciplinary professionals in the accomplishment of the redesigning of plans and proposal of the
Ottawa Withdrawal Management Centre within budget.
