1. Who are we? The Dr. J.D. McLean Continuing Education Centre of the Faculty of Dentistry was designed and built in

cooperation with the Faculty, alumni and the dental industry in 1998-1999. It comprises approximately 5500 square feet and includes a patient reception area, ten dental cubicles,

radiology heads , four station KaVo lab, SteriCentre, pan room, 3M Conference Room for 30-50 people. The leaseholds were paid for by our alumni and the Faculty and user fees are used to maintain the quality and look of the Centre.

The dental equipment and systems are supplied by the dental industry as partners. Our partners include: Sustaining Partners (more than $30,000) Adec KaVo Canada 3M Canada DentalEZ Group Patterson Whipmix Dentsply Canada Pelton and Crane Henry Schein Ash Arcona Sirona Supporting Partners ($5,000 to $30,000) ABEL Dentsply/Tulsa Dental SciCan Apple Canada Exan Takara Belmont Conex Contributing Partners (under $5,000) Brasseler Canada Chairs Limited DesRo Dental The support of the partners is on the understanding that the Centre’s facilities will be used by senior dental students on specialty rotations, practicing dentists and support staff for continuing education courses and study clubs and will be available to the partners and their sales agents for display and sales. The equipment continues to belong to the partners. It is for sale and when sold will be replaced with new/current technology. Sales, removal and replacement of equipment will be coordinated by the partners, their agents and Faculty support staff so that all facilities remain set-up and usable for dental care activities. The Faculty is always interested in hearing from dental industry representatives who are interested in becoming supporting partners in the Dr. J.D. McLean Continuing Education Centre. We are located in the south end of Halifax at the intersection of University Ave. at Robie Street (north east

corner). The main entrance in on University Ave. After hours and on weekends access to the building is on the east side near the College Street loading dock, opposite the Tupper Building.

4. Parking: There is little to no parking on the University Campus and surrounding streets. We suggest visitors utilize the

Professional Centre (corner of Robie and Spring Garden Road) or at the IWK/Grace or Victoria General Hospitals, especially for half or whole day events.

5. Regular Hours of Access Normally visits or events will not start before 8:30 a.m. and not go later than 6:00 p.m. without special

arrangements made 24 hours in advance. Please note that regular business hours are Monday through Fridays from 8:30 a.m. to 4:30 p.m. 6. Building Security - After Hours and Weekend Access Dalhousie University Security requires visitors to sign in and out of the building after 5:00 p.m. You must

request and receive faxed or email approval for after hours or weekend access at least 48 hours in advance of the event.

A name list must be submitted to the Clinical Affairs office at least 24 hours in advance of the event showing

all persons who are to attend. This list will be submitted to Dalhousie Security for building sign in purposes. Photo ID may be requested.

8. To book an event into the Centre Events scheduled during regular business hours To set up an appointment for a sales or demonstration visit to the Centre during regular business hours, please

contact Roberta Emms at (902) 494-1681 at least 24 hours in advance of the time required. If you call and would like access the same day we will assist if possible.

Events scheduled after hours or on weekends Requests for early or after hours access on weekdays or weekends require written approval from Roberta

Emms (902-494-1681) by e-mail or fax. Usually you must request and receive faxed or email approval for booking at least 24 hours in advance of the

event or sales tour. Same day requests will be honored providing there are no schedule conflicts. 9. User Fees

User fees generally apply, except to ‘supporting partners’ who are listed on the sign at the entrance to the Centre. User fee rates will apply to 'supporting partners' if they are charging an event registration fee to each participant.

13. Food, Liquor, Sign In, Clean-up a. Absolutely no food or refreshments are allowed in the Treatment Centre. b. If you are using the 3M Conference Room and require refreshments you can do one of the following: - go to 'Fillings' catering kiosk in the Dentistry Building foyer (open during normal daytime hours - Closed

May - September) - go to 'Tim Hortons' in the Tupper Building (open during normal daytime hours) - bring it with you - cater through a caterer of your choice. Catering arrangements must include pick-up of any serving

equipment (preferably the same day as the event). Some suggestions of caterers are: • Scanway Catering at (902) 425-6806 • Deli Green Catering at (902) 421-1313 or 425-5858 • Aramark (caterers on University) at (902) 494-2078 • Sodexho (caterers on University) at (902) 494-2126 c. Liquor, whether a cash or host bar, served in the CE Centre must be ordered through Dalhousie Bar Services

14. Telephones A phone is available in the Centre for outgoing calls by participants at events. To place an outgoing call you

must dial ‘9’ for an outside line.

The phone is located in the hallway outside the 3M Conference Room. Cell phones are not reliable in this facility.

Smoke-Free, Scent Free, Latex reduced Environment This is a smoke-free and scent reduced building. Please inform visitors/guests in advance. The Clinical Care

area is a latex free zone. Please do not bring latex products for demonstration in the treatment rooms of the Centre without prior consultation with the Dental Assistant in the Centre. Absolutely no latex products with powder coating are permitted.

17. Sales Information / Business Cards The Reception staff maintains a business card folder for sales and owner/manager staff of supporting Partners

only. You can see the receptionist to provide a supply of business cards. Please ensure that you keep your supply current.

18. Equipment and Services Literature The dental assistant staff maintains a file cabinet with literature for on-site equipment models from supporting

Partners only. Please ensure you provide the coordinator with suitable current literature and advise her of who you would like the literature distributed to.

b. Clinical Care Area Arrangements for clinical supplies and equipment are to be made with the unit coordinator prior to the event. 20. Study Club Protocol with Patient Care Activities

Groups wishing to establish a dental continuing education study club at the Dr. J.D. McLean Continuing Education Centre of the Faculty must be preapproved by the Assistant Dean, Clinics and Clinic Manager of the Office of Clinical Affairs. Contact Roberta Emms at (902) 494-1681 or [email protected] for application information. The following protocols apply: 1. Waiver: Participants will be required to sign a waiver (Appendix 1) at the start of each year of Study

Club meetings, indicating they have read and understand their waiver responsibilities. 2. Infection Control: Dentists and their auxiliary staff participating in Study Clubs are required to read

and follow the infection control requirements of the Faculty as explained in the Faculty of Dentistry’s Infection Control manual (available on-line at the Dentistry website, Policies and Manuals) and Infection Control for Study Clubs and Hands-on Courses (Appendix 2 attached).

Scotia Dental Association AND must have current professional liability insurance applicable in the Province of Nova Scotia. Proof of both will be required by the Assistant Dean of Clinic before a Study Club can have access to facilities. (See Appendix 5)

6. Study Club Members: must be licensed to practice dentistry in Nova Scotia through the Nova Scotia

7. Patients: must be a patient of record with a dentist registered in the Study Club. They cannot be an

active patient of the Faculty of Dentistry. They must review and agree to and sign a patient care waiver (Appendix 6) before they can be treated in Dalhousie Clinic facilities.

8. Dental Assistants: Whenever patient care is provided during Study Club activities each dentist is

expected to provide their own chairside assistant as required. Cubicle set-up and clean up is the responsibility of the participant according to the Dr. JD McLean Centre criteria as in #2 above.

21. Hands-on laboratory/preclinical or patient care courses sponsored by the Faculty, Continuing Dental

Education office (CDE):

The Clinical Affairs office and the Dr. J.D. McLean centre do not organize such courses. The CDE Office is responsible to organize such courses and schedule the McLean Centre or other Faculty facilities through the Clinical Affairs office. Companies or groups who wish to sponsor such courses at the Faculty of Dentistry need to contact the CDE office at (902) 494-1674, email is [email protected]

Appendix 1

WAIVER OF FACULTY OF DENTISTRY FOR RESPONSIBILITY FOR INFECTION CONTROL OF INSTRUMENTATION

USE IN PATIENT CARE IN DALHOUSIE DENTAL CLINIC The Assistant Dean of Clinics, Faculty of Dentistry, requires that you read and sign this waiver of responsibility before being permitted to bring personal dental equipment or handpieces into Faculty clinics for patient care activities. Please direct any questions to the Assistant Dean of Clinics, Dr. Blaine Cleghorn, (902) 494-1681. I have read the Infection Control protocol in the Visitors Guide for the Dr. J.D. McLean Continuing Education Centre. I have read and understand how to apply the instructions in the Dalhousie University Faculty of Dentistry Infection Control Manual 2006 by Cathy MacLean, RN. I will apply all the required infection control guidelines as outlined in the above documents for all equipment and instruments I bring into the Dalhousie Dental Clinic and/or use at the Dalhousie clinics. I understand that I am responsible for any infection caused to the patient I treat, any auxiliary staff and myself as a result of poor infection control/cross contamination protocol on my part. APPENDIX 2 October 1, 2007

INFECTION CONTROL FOR STUDY CLUBS & HANDS-ON COURSES

Prior to the start of each Hands-on Course and at the beginning of each Study Club year, time will be allotted to familiarize participants with the infection control protocol of the Dalhousie University, Faculty of Dentistry (Infection Control Manual, July 2006). The dental assistant, responsible for Continuing Education Courses, will advise dentists and their auxillary of the following requirements. 1) Protective attire

Lab coats, gloves, masks, and eyewear will be supplied to participants. These items are to be worn while providing patient treatment.

2) Instruments and Equipment

The Dalhousie University, Faculty of Dentistry will normally supply instruments and equipment used for patient care. Instruments and equipment from other sources must comply with the requirements of CSA 2314.22-04 Management of Loaned, Shared and Leased Medical Devices.

3) Privately Owned Instruments and Equipment - Processed at Dalhousie University, Faculty of Dentistry

Instruments from dentists not currently licensed in Nova Scotia, New Brunswick, Prince Edward Island, or Newfoundland will be washed, packaged, and sterilized prior to use, by the Central Sterilization Unit (CSU) of the Faculty of Dentistry. Sterilization monitoring will be conducted on each load. Instruments, in clean condition and good working order, are to be delivered to CSU, 3 days prior to use.

4) Privately Owned Instruments and Equipment - Processed in Private Practice

Dentists currently licensed in Nova Scotia, New Brunswick, Prince Edward Island, or Newfoundland may use their own instruments, packaged and sterilized in accordance with recommendations of CDA Infection Prevention and Control in the Dental Office 2006 (IPC-04-01 to IPC-04-04). All new items must be cleaned and sterilized prior to use (e.g. burs, files, brownies, greenies). Sterile items are to be transported in a puncture-resistant, sealed container. a) Waiver Dentists bringing processed equipment, handpieces, and hand instruments into the facility will be

required to sign a waiver (Appendix 1) indicating that any instruments they bring in to the Faculty have been cleaned, sterilized and transported according to these guidelines..

b) Biological Monitoring Dentists will comply with the CDA 2006 (IPC-04-04) requirement for weekly biological monitoring of

sterilizers used in private practice. Upon request, dentists will supply documentation of biological monitoring results.

c) Used instruments Following patient treatment, used instruments will normally be washed and packaged prior to removal

from the facility, by the dentist or their auxillary. d) Release of Contaminated instruments Following patient treatment, unwashed items may be removed from the facility, if placed in a

puncture-proof, sealed container with a Bio-hazard label affixed to the outside. Separate containers are required for transporting clean/sterile instruments and soiled instruments

5) Privately Owned Equipment – Processing and Transporting All sterilizable components of equipment are to be prepared according to the instructions in #3 and #4 above.

Non-sterilizable parts are to be covered with a disposable barrier or cleaned and disinfected after each use. Equipment is to be transported in a puncture-proof, sealed container, separate from clean/sterile items.

6) Surface Disinfection and Equipment Decontamination

Instructions and supplies will be provided, so that the dentist or their auxillary may carry out surface disinfection and equipment decontamination. Dental unit water lines are to be flushed prior to and after patient care. As well, the evacuation lines are to be cleaned after patient treatment.

7) Decontamination of Prosthetic Materials and Devices

Supplies will be available for participants to clean and disinfect impressions, bite registrations, and appliances prior to sending to the lab.

8) Sharps Disposal

All sharps are to be discarded by the dentist or their auxillary, in a puncture-resistant container after patient treatment. The one-handed scoop technique should be used for recapping a dental syringe.

9) Hand Hygiene

All participants are expected to carry out good handwashing practices while providing patient care. 10) Management of an Occupational Exposure

All participants are expected to maintain immunity against vaccine preventable diseases. Any laceration, puncture wound, or splash to the participant’s mucous membranes, eyes, or non-intact skin with a patient’s blood or bloody saliva is cause for concern. The area should be immediately cleaned and an assessment of risk conducted. Further treatment is available at the Emergency Department of the Queen Elizabeth II Hospital.

APPENDIX 4 FACULTY OF DENTISTRY January, 2009CE Centre Rate Card

1. DIRECT SALARY AND NON-SALARY COSTS:Trained, on-site dental assistant and reception staff operate the Centre.Dental assistant and Receptionist employees salaries are charged at a rate of $22.00 per hour, prorated by the 1/2 hour when theyare staffing an activity for which charges apply (see Facility Fee below). Time and a hald after 4:30 pm and double time on weekends.

2. FACILITY FEE:

The following facility fees apply to Supporting Partners; and, study clubs and societies whenever there is a registration fee beingcharged to attendees:

The facility fee is intended to cover equipment maintenance and upgrades, liability insurance, new equipment and infrastructure (building). Clinical Care Facility 1/2 day $600 (includes evening until 10pm if scheduled in advance)(includes 10 complete cubicles and a 4 station KaVo lab) per day $1,200 (includes evening until 10pm if scheduled in advance)

Insurance and licensure requirements for Practitioners not licensed in Nova Scotia for hands-on CDE courses or Study Clubs

sponsored in Faculty facilities Licensure and insurance requirements for visiting dental or dental hygiene presenters/practitioners from outside of Nova Scotia who will be providing any patient care activities including any intra-oral assistance with a procedure. Such visiting clinicians shall provide written documentation for all of the following to the Manager of Continuing Dental Education and the Assistant Dean Clinics office at least 48 hours before the course they are involved in begins: 1. Evidence that they are licensed in their home province or state and that they are in 'good standing' with the licensing

Board there. 2. A Provincial Dental Board of Nova Scotia ‘temporary license’ for the days of the course. Contact number # 902-420-

0083. 3 Evidence of malpractice insurance in an amount of not less than $3,000,000.00 or other amount set out in the

Compulsory Malpractice Insurance Regulations made under subsection 45(1) of the Dental Act (http://www.gov.ns.ca/just/regulations/regs/dentinsu.htm), including correspondence from the Insurer that the insurance is effective in NS.

Visiting clinicians who do not have transferable malpractice and liability insurance to Nova Scotia can try contacting CDSPI at the information below to discuss/arrange insurance coverage for the course in Nova Scotia, as outlined above.

The Clinic Director, Faculty of Dentistry, Dalhousie University requires that you read and sign this waiver of responsibility before being treated as part of a dental continuing education program or study club in the Dr. J.D. McLean Continuing Education Centre. Please direct any questions to the Assistant Dean, Clinic, Dr. Blaine Cleghorn, 494-1681. I understand that I am to receive dental treatment rendered by a licensed dentist and that the treatment is part of a continuing education program or study club in which the dentist is participating. I understand that while the treatment will be rendered within the Faculty of Dentistry (“Faculty”) building, the continuing education program or study club, including my treatment, is not part of the undergraduate or graduate student curriculum of the Faculty. I agree that while there will be a record of my treatment at the Faculty, the Faculty will not be responsible for the quality and quantity of treatment I receive. I will not hold the Faculty responsible for problems associated with the treatment I receive while a participant in this program nor for unassociated dental problems which may arise.
