Welcome to the Annual Meeting of the Florida Society of Anesthesiologists!

Welcome, colleagues! The FSA Board of Directors and I are thrilled that you’re joining us for this exciting and informative Annual Meeting. Our experienced program committee under the expert leadership of Drs. Chris Giordano and Leopoldo Rodriguez have built a weekend for you that is rich with insightful sessions, interesting topics of discussion and entertaining events. And it’s all housed in a location that is truly like no other in the state of Florida; 2017 marks the 15th year that FSA holds our Annual meeting at the Breakers here in West Palm Beach, and this beautiful space only serves to enhance what I know will be a great event. We’re glad you’re here.

Ensuring Patient Safety is our goal and our mission. This weekend we explore ideas and new pathways to achieving that goal, and several sessions present and discuss important technological advances to that end. We’ll look at mobile technologies and perioperative informatics, and be sure to join us Saturday morning for a 3-D Virtual Cadaver Demonstration, sponsored by Pacira Pharmaceuticals. Several sessions this year look at innovative ideas in pain management, and we again offer a hands-on Ultrasound Workshop.

Prominent leaders from our colleague societies will also be on hand to share their insights – we welcome Dr. John Dombrowksi, ASA Secretary and Dr. Brenda Fahy, ABA Vice President. And we are delighted to welcome back to Florida US Congressman Dr. Andy Harris, who will update us on the current state of healthcare in the US. Important people with important messages – please make it a point to attend their presentations.

With all this great material to absorb, my advice to you is this – immerse yourself. Everywhere you look there will be something interesting to do, to hear, to learn. Attend the sessions, review the great posters that surround the meeting space, visit the exhibit hall and stroll through a truly impressive room full of great ideas and people. Find your friends, make new friends, and don’t forget to stop every once in a while and look up – we’re in a grand and beautiful space on an inspiring stretch of shoreline. Don’t miss that.

The benefits of membership to the FSA are many, and include:

• Representation – through the FSA Political Action Committee – along with relevant, up-to-the-minute information about current state and federal legislative issues affecting anesthesiology;

• Promoting interactions and facilitating mentorships between students, trainees and senior anesthesiologists;

As a state component of the American Society of Anesthesiologists, FSA members must also belong to the ASA. The FSA uses the ASA membership application for state membership. There are several available categories for membership, and you can find your appropriate category on the FSA website at www.fsahq.org/join-fsa.

FSAPAC is the political component of the Florida Society of Anesthesiologists. Federal law as well as Florida state law prohibits associations such as FSA from involving themselves in the political process. Only PACs can participate, and FSAPAC strives to play a key role in shaping the policies important to Florida anesthesiologists. Compensation, regulatory issues, the role of non-physician providers in the practice of anesthesiology – these and other vital issues are the focus of FSAPAC, and the committee provides direct contributions to incumbents, challengers and open-seat candidates for office who have proven that they are willing to listen to FSA’s message and share the vision of providing the highest quality anesthesiology care.

FSA 2017 offers several opportunities to help fulfill your Maintenance of Certification in Anesthesiology (MOCA®) requirements. Sessions marked with the MOC-PS icon will count toward Part II Patient Safety requirements. Look for the MOC-PS icon to identify designated sessions. BIOSChristopher Giordano, MD PROGRAM CHAIR

Dr. Chris Giordano was born and raised in Fort Lauderdale, Florida. After graduating from Saint Thomas Aquinas H.S., he attended the University of Florida for undergraduate and University of South Florida for Medical School. He attended the University of Alabama – Birmingham for his Anesthesiology Residency, and returned to the University of Florida for a Liver Transplant fellowship where he continued on and is now Division Chief of Liver Transplant. He is very active in the medical student education as the Anesthesiology and Critical Care clerkship director and course supervisor. In his free time, he enjoys playing golf, tennis, watching sporting events, and spending time with his wife, Tracy, and children, Gigi and Dominic.

Leopoldo Rodriguez, MD, FAAP PROGRAM CO-CHAIR

Dr. Leopoldo Rodriguez graduated from the Universidad Central de Venezuela Medical School and completed his Anesthesiology Residency in Vanderbilt University as Chief Resident taking the advanced Sub-Specialty CA3 Track in Pediatric anesthesia. From 2000 to 2005, he was in Private Practice in Pensacola Florida, dedicating his career to High Risk Pediatric Anesthesia and Adult Minimally Invasive Cardiac Anesthesia. From 2005 to 2008, he served as Cardiac Anesthesiologist, and later, Chief of Anesthesiology at Aventura Hospital and Medical Center. Since 2008 to present, he has been the Medical Director and Chief of Anesthesiology at the Surgery Center of Aventura. He Chairs the Envision

Healthcare Physician Services Ambulatory Anesthesia Quality Committee. He is the FSA Treasurer, serves in the Board of Directors of SAMBA and is enrolled in the University of Massachusetts MBA program. In his free time, he enjoys playing tennis and practicing traditional Japanese martial arts of the Bujinkan Dojo.

Rebecca Welch, MD DISTINGUISHED SERVICE AWARD RECIPIENT

Rebecca Welch, MD is a Pediatric Anesthesiologist at Arnold Palmer Hospital in Orlando, Florida. Dr. Welch has been an active member of the FSA for over 25 years. She served on the Executive Committee for several years and, from 2004-2005, she is very proud to have served as the President of the FSA. During her presidency, the FSA was instrumental in helping to pass the legislation that allowed Anesthesiology Assistants to be licensed in the state of Florida. FSA also strongly supported the increased regulation of Office based surgery by the Florida Board of Medicine. Supporting these efforts, along with combating the negative effects of the infamous “Ortiz” decision, which negatively

impacted the role of physician supervision in Florida, made her presidential year very active. Professionally, serving as the FSA President was an extremely rewarding and fulfilling experience. Over the years, she has served in many other offices including FSA Central Florida District Director, FSA Treasurer, Secretary and President-Elect, ASA Delegate, ASA Committee on Communication member, the Chair of the FSA Bylaws Committee, Editor of the FSA Today newsletter, and FMA Delegate. Currently, she Chairs the Communications Committee, and has done so for most of the past 20 years. During that time, the FSA has grown, as have the methods of communication at our disposal, making this an ever changing and challenging committee. Dr. Welch also currently serves as the alternate delegate to the Florida Medicare Community Advisory Committee.

Originally from Fort Wayne Indiana, Dr. Welch attended college at the University of Chicago and the University of Chicago, Pritzker School of Medicine. She then completed a Pediatric Residency, Anesthesia Residency and Pediatric Intensive Care Fellowship at the University of Florida, Shands Hospital. There she met and married Jim Welch, MD, who completed his Urology Residency, and Urologic Oncology Fellowship at the University of Florida. He is currently the senior partner of Orlando Urology Associates. They have been married for 31 years and together they have 2 wonderful children – Danny, a third year medical student, and Emily, a first year medical student, both at the University of Florida. GO GATORS! Family has always been the most important part of her life, and she would like to truly and whole-heartedly thank Jim for the love and support, without which this award would not be possible.

For Dr. Welch, the most important mission of the FSA is the constant drive to forward patient safety. Working within the FSA toward that goal has had the benefit of learning how to work with committees, on boards, understand budgets, and has provided creative and educational opportunities. Dr. Welch feels that involvement in the FSA has helped her to understand the legislative, legal and regulatory processes in Florida and nationally. Gaining that understanding through the FSA allows participants to feel not like a helpless bystander as the waves of political intrigue flow around us, with minimal understanding of our profession, our patients, or our training, but rather empowered to help make a difference.

This talk will discuss the importance of providing quality regional anesthesia for patients that lasts beyond discharge from the PACU and into the perioperative recovery period. There are many methods for providing this extensive pain control ranging from placing continuous peripheral nerve catheters to choosing new long lasting formulations of local anesthetics. This talk will discuss the use of local anesthetic adjuvants to extend the duration of action of peripheral nerve blocks after surgery.

In the older surgical patient, Post-operative cognitive dysfunction (POCD) is ever more being accepted as one of the most frequent neuropsychological consequences of anesthesia. It is characterized as mild cognitive disorder separate to delirium and dementia, and categorized by subtle impairments in memory, concentration and information processing that are often interpreted as ‘senior moments. Its diagnosis requires neuropsychological testing and presently, it isn’t labelled as a psychiatric diagnosis. Its occurrence in the immediate postoperative period has been reported as being high. Although it is usually transient some patients may experience long-term dysfunction lasting weeks or months and it may even become permanent. In this talk I will aim to describe POCD, possible underlying mechanisms, and how it differs from other postoperative cognitive syndromes. Potential approaches to both prevention and treatment will be described as well as long-term consequences of its occurrence.

Lecture 2: Principals Underlying the Safe Administration of Anesthesia for Surgery in the Older Patient.

The elderly population (>65 years) is increasing at a rapid pace, and a substantial proportion of these patients go through surgical procedures. This group has a distinctive array of requirements due to the aging process, several which causes them to be especially susceptible to postoperative problems and a protracted recovery. The intent of this lecture is to examine the literature, confirm existing guidelines, and offer a series of suggestions to aid anesthesiologists, safely manage older patients during the perioperative period.
