ANURADHA MASCARENHAS PUNE, JUNE 11 A GENETIC risk score, which tells how a person’s risk compares to others with a different genetic constitution, is effective in diag- nosing type 1 diabetes in Indians, a new research has found. Genetic risk score, developed by the University of Exeter, takes into account detailed genetic in- formation known to increase the chance of developing type 1 dia- betes. The score may be used at the time of diabetes diagnosis to help decide if a person has type 1 diabetes. Until now, the bulk of re- search in this field had been con- ducted on European popula- tions. In a paper published on Thursday in ‘Scientific Reports’, an online multidisciplinary open-access journal from the publishers of ‘Nature’, re- searchers have now analysed whether the European risk score is effective in diagnosing type 1 diabetes in Indians. The study has been carried out by researchers from KEM Hospital in Pune, CSIR-Centre for Cellular and Molecular Biology (CCMB), Hyderabad, and the University of Exeter in the UK. The research, supported by Diabetes UK, the KEM Hospital Research Centre, Pune and the Council of Scientific and Industrial Research (CSIR) in India, found that the test is effec- tive in diagnosing the right type of diabetes in Indians, even in its current form, which is based on European data. Prof Chittaranjan Yajnik, di- rector, diabetes unit, KEM Hospital, said the research has shown a new way of using ge- netics to diagnose diabetes, which could pave the way for better diagnosis and treatment in India. He said: “Misdiagnosis of di- abetes could be an issue in India because of features of diabetes in Indians might vary from the stan- dard western textbooks. Until re- cently, it was widely believed that type 1 diabetes appeared in children and adolescents, and type 2 diabetes in obese and older (typically after 45 years of age). However, recent findings have shown that type 1 diabetes can occur later in life, while type 2 diabetes is on the rise among younger and thinner Indians.” The team studied people with diabetes from Pune, analysing 262 people with type 1 diabetes, 352 people with type 2 diabetes, and 334 people with- out diabetes. Outcomes from the Indian population were com- pared with those of Europeans from the Wellcome Trust Case Control Consortium study. The authors found genetic differences between the popu- lations, which mean the test could be further improved to enhance outcomes for Indian populations. Dr Richard Oram, of the University of Exeter Medical School and one of the study au- thors, said: “Diagnosing the right diabetes type is an in- creasingly difficult challenge for clinicians, as we now know that type 1 diabetes can occur at any age. This task is even harder in India, as more cases of type 2 diabetes occur in peo- ple with low BMI. We now know that our genetic risk score is an effective tool for Indians, and can help get peo- ple treatment they need to avoid life-threatening compli- cations such as diabetic ke- toacidosis and achieve the best health outcomes.” Dr Yajnik said rising diabetes cases among in young Indians makes it imperative that we di- agnose the type of diabetes cor- rectly to avoid mistreatment. The new genetic tool, he said, will help decide when diabetes is mainly caused by autoim- mune destruction of pancreatic beta cells (type 1 diabetes), and when it is mainly caused by re- duced action of insulin due to ex- cess fat and smaller muscle mass in the body (‘thin-fat Indians’), as is the case in type 2 diabetes. Dr Yajnik said, “We look for- ward to using this test in dia- betic patients from different parts of India where the physi- cal characteristics of diabetic patients differ from the stan- dard description.” In a statement, CCMB direc- tor Dr Rakesh K Mishra said: “Since more than 20 per cent of people with type 1 diabetes be- low the age of 15 years of age are in India, developing a genetic test kit to reliably detect type 1 from type 2 diabetes holds a lot of sig- nificance for the country.” The authors found nine ge- netic areas — called the SNPs, or single nucleotide polymor- phism, which is basically vari- ants in the genome of an individ- ual — that correlate with type 1 diabetes, both in Indian and European populations, and can be used to predict the onset of type 1 diabetes in Indians. Dr G R Chandak, chief scien- tist leading the study at CSIR- Centre for Cellular and Molecular Biology (CCMB), told The Indian Express that different SNPs are more abundant among Indian and European patients. “This opens up the possibility that environmental factors might be interacting with these SNPs to cause the disease,” he said. Genetic risk score effective in diagnosing type 1 diabetes: Study New way of using genetics to examine diabetes could pave way for better diagnosis, treatment, say experts EXPRESS NEWS SERVICE NEW DELHI, JUNE 11 IIT-MADRAS, IISc-Bangalore and IIT-Delhi emerged as the country's top three institutions for the sec- ond year in a row in the fifth edi- tion of the National Institutional Ranking Framework (NIRF) an- nounced on Thursday by the HRD Ministry. Jamia Millia Islamia, which was at the centre of the student pushback against the Union gov- ernment this year, broke into the top 10 universities of the country and improved its rank from 19 to 16 under the overall category. Jawaharlal Nehru University (JNU), which witnessed student protests over fee hike, has main- tained its position as the second- best university in the country, af- ter IISc-Bangalore. The NIRF release came a day after many Indian institutions slipped in QS World University Ra- nkings 2021, including at least 10 marked out by the government under the ‘Institution of Eminence’ scheme. Referring to the performance in World University Rankings, HRD Minister Ramesh Pokhriyal said that he did not agree with the use of perception as a perform- ance metric. “Our graduates are now leading global organisations. But our institutions are given low ranks based on perception. I do not agree with this,” he said at the NIRF launch. “We will create a sit- uation when institutes from across the world will want to be listed in the Indian rankings.” Aside from the overall rank- ings, NIRF also lists out best insti- tutions across another nine cate- gories — college, university, medical, engineering, manage- ment, pharmacy, law, architecture and dental. The IITs dominated the engi- neering category by occupying nine out of the top 10 spots. The biggest changes in the top 20 po- sitions in the category were regis- tered by Anna University, which fell five positions from Rank 9 to 14, IIT-Mandi, which dropped out from the top 20 to Rank 31 and NIT Karnataka, which gained eight positions to move up from Rank 21 to 13. Among universities, improve- ments were made by Amrita Vidya Vishwapeetham (Rank 8 to 4), BITS-Pilani (from 23 to 15) and Jadavpur University (from 6 to 5). Aligarh Muslim University (AMU) slipped six spots to Rank 17. AMU also ranked lower at 31, as op- posed to 18 in 2019, in the overall category this year. The university issued a statement Thursday, at- tributing the fall to incorrect data entry regarding its PhD scholars by the ranking agency. AMU said it will file a representation with the ministry and NIRF for rectifi- cation. There was a bit of a reshuf- fle in the college category. While Miranda College retained the top position, Lady Shri Ram College emerged as the second-best from Rank 5 last year and Hindu College dropped from Rank 2 to 3 this year. Presidency College, Chennai, was displaced from the third po- sition to rank 5. Shri Ram College of Commerce slipped from Rank 7 to 12. PSGR Krishnammal College for Women made a significant gain of 12 positions from Rank 22 to enter the top 10. Speaking to The Indian Express , Amroha SP Vipin Tada says Singh, his father Lala and an associate Deepak have been arrested while two others are absconding. Singh is the main accused in an FIR filed under section 302 (murder) and others sections of the SC/ST Act. But the police and the accu- sed's family maintain that the ki- lling was linked to an altercation on May 31 between the two sides over Rs 5,000 that Vikas's brother allegedly owed Singh. According to police, the investigation did not lead to any evidence that suppo- rted the charge of caste animosity. "A few days earlier, we were in- formed about a fight between Harom and Vikas. Vikas's elder brother owed money to Harom which he had promised to return once farm sales are carried out. IIT-MADRAS, IISc-Bangalore andIIT-Delhi emerged as the country'stop three institutions for the sec-ond year in a row in the fifth edi-tion of the National InstitutionalRanking Framework (NIRF) an-nounced on Thursday by the HRDMinistry.

Jamia Millia Islamia, whichwas at the centre of the studentpushback against the Union gov-ernment this year, broke into thetop 10 universities of the countryand improved its rank from 19 to16 under the overall category.Jawaharlal Nehru University(JNU), which witnessed studentprotests over fee hike, has main-tained its position as the second-best university in the country, af-ter IISc-Bangalore.

The NIRF release came a dayafter many Indian institutionsslipped in QS World University Ra-nkings 2021, including at least 10marked out by the governmentunder the ‘Institution ofEminence’ scheme.

Referring to the performancein World University Rankings,HRD Minister Ramesh Pokhriyalsaid that he did not agree with theuse of perception as a perform-ance metric. “Our graduates arenow leading global organisations.But our institutions are given lowranks based on perception. I donot agree with this,” he said at theNIRF launch. “We will create a sit-uation when institutes fromacross the world will want to belisted in the Indian rankings.”

Aside from the overall rank-ings, NIRF also lists out best insti-tutions across another nine cate-gories — college, university,

medical, engineering, manage-ment, pharmacy, law, architectureand dental.

The IITs dominated the engi-neering category by occupyingnine out of the top 10 spots. Thebiggest changes in the top 20 po-sitions in the category were regis-tered by Anna University, whichfell five positions from Rank 9 to14, IIT-Mandi, which dropped outfrom the top 20 to Rank 31 andNIT Karnataka, which gainedeight positions to move up fromRank 21 to 13.

Among universities, improve-ments were made by AmritaVidya Vishwapeetham (Rank 8 to4), BITS-Pilani (from 23 to 15) andJadavpur University (from 6 to 5).Aligarh Muslim University (AMU)slipped six spots to Rank 17. AMUalso ranked lower at 31, as op-posed to 18 in 2019, in the overallcategory this year. The university

issued a statement Thursday, at-tributing the fall to incorrect dataentry regarding its PhD scholarsby the ranking agency. AMU saidit will file a representation withthe ministry and NIRF for rectifi-cation. There was a bit ofa reshuf-fle in the college category. WhileMiranda College retained the topposition, Lady Shri Ram Collegeemerged as the second-best fromRank 5 last year and Hindu Collegedropped from Rank 2 to 3 thisyear. Presidency College, Chennai,was displaced from the third po-sition to rank 5. Shri Ram CollegeofCommerce slipped from Rank 7to 12. PSGR Krishnammal Collegefor Women made a significantgain of12 positions from Rank 22to enter the top 10.

Vikas's family alleges that hewas killed by 18-year-old HoramSingh who had brutally assaultedhim days earlier, on May 31, for en-tering the village temple. In a po-lice complaint, Vikas's fatherOmprakash claims that his sonhad been targeted for his caste ear-

lier as well.Speaking to The Indian Express,
