The Jonathan House is so excited that you have chosen us to be a part of your journey to freedom

and wholeness! We are looking forward to working with you!

Enclosed is a list of questions (some of which are a bit personal in nature) that will help us

identify potential entry points that could negatively influence your life and business. Answer

each question truthfully and attempt to give as much detail in your answers as possible so that

you will gain the maximum benefit from our time together. If additional space is needed to fully

explain any item, please attach additional sheets that are either typed or legibly handwritten.

Before filling out your questionnaire we would encourage you to spend some time in prayer. Ask

the Holy Spirit to bring back to your memory any hurtful or destructive words that have been

spoken by you or others, any places or events filled with envy, strife, confusion, or chaos, and

any resentment, bitterness, unforgiveness or other ill feelings you may have from your past.

As part of your Spiritual Business Inventory, The Jonathan House Ministry team will come to

your place of business and deal with any spiritual roots, break any curse that may have been

placed over the business, building, or land, pray over all buildings, offices and equipment, as

well as bless the business.

Please know ALL information shared on your questionnaire and during your ministry time is

kept confidential. Your questionnaire is kept in a locked file and viewed only by our ministry

team. All of our ministry teams hold to a strict policy of confidentiality and understand your

need for privacy concerning sensitive information.

Financial Considerations:

Everyone goes into business with different goals but one goal everyone has in common is to

make money. Desiring to be successful in business is not a bad thing, in fact Psalms 35:27 says,

And let them say continually, "The LORD be magnified, Who delights in the prosperity of His

servant." God wants us to be successful in all that we do. Romans 8:31 reminds us that God is

for us not against us. Sadly, many well intentioned people go into business and quickly disregard

spiritual principles instead replacing them with the “wisdom” of the world. Our prayer is that you

would see the wisdom in laying a spiritually sound foundation on which your business is built.

The Jonathan House has as our main goal – “to set the captives free,” whether that’s an

individual struggling with issues or a business that looks like it is headed towards failure. We

have a long-standing policy stating we will not withhold ministry from anyone due to their lack

of finances. However, we want you to be aware that we are solely supported by the donations

from people receiving ministry and you should make every possible attempt to make a financial

donation toward your ministry experience.

We strongly believe in the spiritual law of sowing and reaping - that if you want to be blessed

you need to sow seeds of blessing and then in return you will be blessed. You can bless The

Jonathan House Ministries with monthly support, with a tithe if you aren’t already doing so, or a

one-time donation at the time of your ministry. If your business is not in our local area, we do

require you to cover our travel expenses at the current travel rate established by the GSA

(General Services Administration of the U.S. government). We do ask that you cover our

travel expenses at the time of ministry.

Please carefully read over the donation options on the following page and check the one that

works best for you and your business. Since all ministry sessions are by appointment, we will

contact you to schedule your Business Cleansing once we have received your completed

questionnaire. We do ask that you send in a $50 deposit when you return your completed

questionnaire.

Please remember that The Jonathan House is only able to offer this much needed ministry to the

body of Christ because of generous gifts from people like you. We are a non-profit, faith-based

organization, and have a 501(c)(3) standing with the IRS. All of your financial donations to the

ministry are tax deductible. We accept ministry donations in the form of money orders, cash,

checks, Pay Pal, or credit/debit cards.
