S u m m a r y : The study conducted the analysis of the medical examination
of adolescents to enroll in military schools, a comprehensive assessment of their
health status. In reforming the military education system interagency integration is
necessary in carrying out the basic stages of a medical examination of young people entering
the military school in child health clinics of the Ministry of Health.

S u m m a r y : The research represents the analysis of infant mortality and
factors determining its rate in Tver Region for the period of 1996-2007. It reveals a
positive dynamics in the indexes of infant mortality for the last 11 years which is to
large extent can be attributed to the implementation of the «Complex Program on
Women and Child Care and Child Mortality Reduction» in Tver Region, carried out
in 1989-2010. It also proves the effectiveness of developing and launching strategic

programs on fighting both the rate and the definite causes of death among the
population.

Infant mortality belongs to the leading index of not only the effectiveness of
the State Health Service but the national well-being on the whole. For several
decades Tver Region has been among the regions with the highest infant mortality
rates both in The Central Federal District and in Russian Federation on the whole.
T h e a i m o f r e s e a r c h was to substantiate the necessity of
developing and implementing strategic programs to lower the rate of mortality among
the population especially occurring due to definite causes.
M a t e r i a l a n d M e t h o d s o f R e s e a r c h : The research
represents the analysis of statistic data provided by the Local Committee of Federal
Statistic Data Service of Tver Region on infant mortality, perinatal mortality
including early neonatal one and mortinatality.
Multi-factor analysis applied determined quantitatively the importance and
share of different factors influencing the rate of infant mortality.
R e s u l t s o f t h e r e s e a r c h a n d t h e i r d i s c u s s i o n .
The rates of infant mortality in Tver Region in 1997-2007 fluctuated between 19.3‰
in 1998 and 9.8‰ in 2007. Since 1999 the positive dynamics of this index might be
observed – there has been a gradual decrease in infant mortality. On the whole the
index fell down by 46.2% (from 18.2‰ in 1996 to 9.8‰ in 2007). But at the same
time it continued exceeding the average rates of child mortality in Russia. By 2007
this difference has amounted 4.3% (9.4‰ throughout Russia and 25.6% throughout
the Central Federal District (7.8‰).
Similar trend could be observed in urban areas of the Region since 1999 but in
the rural area this process started a bit later – in 2002. The comparison of mortality
rates among urban and rural infants aged less than 1 year old displayed the exceeding
of deaths the latter above the former (except in the years of 1998, 2000 and 2006

when the mortality among rural children was lower than among the urban ones.) At
the same time infant mortality in urban settlements fluctuated between 8.3‰ in 2007
and 19.9‰ in 1998, as for the rural ones the figures were 10.2‰ in 2006 and 24.1‰
in 1996. Thus positive dynamics in the rates of infant mortality could be equally
observed both in urban and in rural areas. The indexes of 1996 dropped (by 47.8%
and 42.7% correspondingly) and the most rapid changes could be observed in rural
settlements since 1996 to 2006.
In the urban districts of Tver Region the rates of infant mortality in 2007
remained within the boundaries of 5.0‰ and 16.2‰. In comparison to 1996 the
indexes decreased by 8.5% that is they became 3.6 times lower. Among the
municipal districts the range of infant mortality indexes varied considerably between
2.8‰ and 43‰. At the same time the changes in indexes for the studied period
proved diverse in different districts of the region: the increase in infant mortality was
reported in 12 districts with the decrease in 14 ones; it remained practically the same
in 4 districts and in 6 ones the cases of infant mortality were not registered at all.
The biggest drop in the mortality rate of infants aged less than 1 year old was
registered among girls (for them it became 2.1 times lower (1.5 lower for the boys))
in comparison to 1996. Thus the share of male infants in the structure of those who
died in early child age has increased. So if in 1996 this share slightly exceeded the
half of the total number of children who died under 1 year old (54.6%) in 2007 it
amounted 62.1%.
For the studied period the index of perinatal mortality in Tver Region exceeded
the average rates throughout the country (with the exception of 2000) and in 2007 the
difference made up 15.4% (10.4‰ against 8.8‰). In 2007 Tver Regin was placed the
forth among the regions of the Central Federal District in perinatal mortality
following Ryazanskaya (12.8‰), Kostromskaya (11.2‰) and Kaluzskaya (10.6‰)
Region, exceeding by 1.3‰ the average rate of the Central Federal District.
The coefficient of perinatal mortality in Tver Region was within the limits of
10.4‰ and 19.3‰ (in 2007 and 1998 correspondingly). For 11 years it has fallen by
42.8%. The dynamics of early neonatal mortality and mortinatality, determining the

rate of perinatal mortality has remained unstable for the studied period but there has
been some progress in it. By 2007 the rate of mortinatality became 3 times smaller
and the index of early neonatal mortality dropped by 15.3%.
The causes of death among children under 1 year old proved to be different in
urban and rural areas. In the former ones perinatal death has been found out to be
responsible for 53% of deaths among newborns, in the latter ones this is the cause of
33% of deaths. The second major cause of urban newborn deaths (24% of deaths) is
congenital anomalies (22% for the rural newborns). At present the diseases of
respiratory system are considered to be a leading cause of newborn deaths in the rural
area (25%, while for the urban area it is 13%).
For the last 11 years the coefficient of infant mortality that could happen due to
definite causes has decreased; namely, the number of deaths due to the congenital
anomalies became 2.4 times smaller, mortality due to some causes of perinatal death
decreased by 31%, due to accidents, poisoning and traumas – by 40%, due to the
diseases of respiratory tract – by 38%.
The research has revealed the social character of factors contributing to the
presence of a considerable level of infant mortality in cities as well as in the rural
area. It was found out that these factors are eliminable and can be eliminated by the
collaboration of medical workers and social organizations involved. It is known that a
number of these factors fully depends on the level of health service provided for
children aged less than 1 year old, pregnant women and women in labour. The
problem of medical services available for women in labour is especially critical for
the rural settlements of the region.
The integrated evaluation of medical and social factors determining the
frequency of deaths among infants under 1 year old has revealed the following factors
being the most considerable: bad habits of a father, financial well-being of a family,
mother’s social status, mother’s marital status, parents’ heredity, living conditions,
mother’s bad habits and frequency of prenatal nursing.
In the rural areas 8 leading factors of medico-social character determining the
rate of infant mortality are as follows: father’s bad habits, mother’s social status, full-

term of a newborn by the time of labour, financial well-being of a family, father’s
educational background, complications in labor, frequency of prenatal nursing and
mother’s marital status.
Most of these factors are of social character and refer to the category of
eliminable ones.
There has also been studied the degree of influence of social, biological and
medical factors on the infant mortality rate in the region on the whole, in the district
centers and in the rural settlements of the region.
As an instrument for measuring the influence of factors on the result a multy-
factor analysis has been applied. It provided quantitative estimates of the influence of
different factors on the result that is on the rate of infant mortality.
On the whole 27 of the manageable factors determining the mortality rate of
infants under 1 year old have been studied. They have been grouped into 3 main
types: manageable, partially manageable and unmanageable ones. Moreover within
each group factors of medical and not medical character have been distinguished.
These groups of factors are rather conventional and still they help to determine those
ones which are possible to manage in order to bring down the rate of infant mortality
and those of them which depend mostly on the quality of medical service. Obviously,
manageable factors determined are much of a practical value.
The estimates have revealed that irrespective of the type of settlement these are
manageable factors that play the major role in building the rate of infant mortality,
with factors of medical character (that is depending on the level of health service)
predominating. Among manageable factors of medical character the biggest ratio
belongs to the complications in labour (7.1% in the rural area and 12.4% in the
regional centre) as well as full-term of a newborn at birth (more than 5%).
Manageable factors of not medical character also play an important role in
building the rate of infant mortality. With their ratio of 22.2% in towns and up to
32.4% in the rural settlements of the region they are being more critical for the rate of
infant mortality in the rural area. It must be pointed out that infant mortality depends
on the way of life of parents-to-be especially that of a mother (it is responsible for

14.7% of deaths among infants in Tver and for 29.0% of deaths in the rural
settlements).
Thus cultivating a healthy way of life among parents (fighting with smoking,
for instance) is at present primary and the most reasonable way to lower the rate of
infant mortality in towns and especially in the rural settlements. Certainly that is the
task that must be accomplished not only by medical workers but by every member of
the society.
Another priority in fighting infant mortality is the prevention of prematurity
and complications in labor, as well as carrying out definite tasks on normal physical
development of infants aged under 1 year old. The coordinating role in fulfilling
these tasks undoubtedly belongs to medical workers.
It must be pointed out that such factors as parents’ social status, their
educational background, family income and living conditions determine the rate of
infant mortality in towns as well as in the rural settlements but to very little extent.
The multifactor dispersion analysis applied allowed to determine quantitatively
the share of different factors in building the rate of infant mortality. Irrespective of
the type of settlement manageable factors proved to have the biggest ration (more
than 55%) with the factors of medical character predominating, especially in towns.
As for the rural settlements the rate of infant mortality there has been found out to be
to large extent determined by the manageable factors of not medical character,
connected with parents’ way of life and living conditions.
All the above mentioned things considered further steps towards lowering the
rate of infant mortality in Tver Region are found to depend on the implementation of
such medical and social tasks as:
the improvement of medico genetic aid provided for the population and
further development of prenatal diagnostics.
preventive measures aimed at cultivating healthy way of life among
parents, delivery and bringing up healthy children.
stricter monitoring of labour protection in respect of women.

improving the quality of legal aid provided for families, mothers and
children.
introduction of the latest forms of data supply, record keeping and
monitoring of the effectiveness of Mother and Child Care measures.
At the same time positive dynamics of the infant mortality indexes that has
been reported for the last 11 years in the region is to large extent due to the
implementation of the “Complex Program on Women and Child Care and Child
Mortality Reduction” in Tver Region carried out in 1989-2010.
Among the key points reflected in 16 chapters of The Program the most
important are:
the improvement of prophylactic work among the youth aimed at
preserving the health of spouses-to-be.
the performance of definite tasks on improving the health of females at
different stages of their maturity and preparing them for becoming mothers.
the improvement of forms and methods of prophylactic medical
examination of pregnant women and those of fertilization age.
the improvement of quality of medical aid provided for women and
newborns in labor (optimal obstetrics)
the improvement of prophylactic and curative measures among children
(The Program “A Healthy Child”).
strengthening the financial and technical base of obstetric services and
childhood protection
improving the quality of legal and social aid provided for families,
mothers and children etc.
The main part of The Program defines the importance of improving personal
health at such stages as: improving the health of parents’-to-be (I); conception (II);
pregnancy period (III); labour (IV); postnatal period (V). Considering close
connection of the I and V stages one should speak about the closed constantly
evolving cycle passing though generations.

C o n c l u s i o n . Thus the main features of the above mentioned program
are its being: long-term, complex, permanent and scientific.
Being long term enables it to determine to larger extent the character and
indexes of obstetrics service.
Being complex means its phase structure and multi sided approach to bringing
up healthy children (starting with health of parents’-to-be and finishing with child
health in postnatal period following the chain: a female child – a young woman – a
woman – a pregnant woman – labour – a newborn).
Being permanent means constant development of The Program, its
improvement with respect to the changing conditions, infrastructure, financial base,
staff efficiency and financing, that will enable timely determination of priorities and
measures needed, that is this Program represents a complex integrated system, all the
levels of which are in the dynamic balance.
The implementation of this Program proves the necessity of development and
launching strategic regional programs aimed at lowering mortality rate among the
population, especially from definite causes.

S u m m a r y : In the article are published results of occupational morbidity
contrastive analysis from the angle of the workers of water-supply. The received
results testify, that one third from all HNT illnesses are fit on ear diseases. Prevalence
of ear diseases with hearing loss is higher with influence of production loss.

S u m m a r y : For improving regional information databases of social and
hygienic monitoring it is necessary to search extra information providers which
indicates about influence of environmental aspect on population health, including
objective appraisal of labour conditions to working man. Creation mechanisms of
using valid results of systems production control in social and hygienic monitoring. It
is important problem solving about compatibility and adequacy of data ware. One of
the methods which did not find proper introduction in system of social and hygienic
monitoring up to date ,is production control carrying out by plant facilities and forces
of accredited laboratories.

S u m m a r y . Data on the results of the restorative treatment of patients
surviving surgical operations in St.-Petersburg are analyzed. The role of new
organizational technologies, modern medical equipment and effective corrective
procedures in improving the quality of restorative treatment is shown. The main areas
of the practical activities are presented, allowing improving the quality of restorative
treatment of patients who have tolerated hi-tech surgical operations, shortening the
period of their disability and frequency of their incapacity.

S u m m a r y : Influence of factors of residing characterizing condition
lifestyle, pressure of educational sports-training activity and value judgment of the
health on the general disease and the academic progress of students among students
of sports high school were studied. The most essential negative factors are: the shirk
through illness more than 7 days per year, a self-estimation of the health are not
above satisfactory, sensation of weariness after employment in high school,
participation in competitions of three and more times per year lasting more than 30
days, smoking, the alcohol use, not observance of a mode of a dream and a food.

S u m m a r y : the state of upper extremities microcirculation was studied in
168 computer users, 98 jewelry polishers and 64 metal hull assemblers. The
dependence of upper extremities microcirculation disturbances on manifestation
degree of manual physical load was revealed.

S u m m a r у : Potential areas on livestockfarmi have been divided into 3
zones depending on conditions of their manufacture for future comparative analysis
of quality, food and biological value of the products from meat and milk, are
producible in shallowness environment. Study of forage crops, waters for animals, as
well as indicators for food and biological value of the products from meat and milk
had generated a need for experts to draw attention to criteria of safety and on food
and biological value of production are producible in shallowness environment.

S u m m a r y : the peculiarities of the emotional tension formation were
revealed and the changes of the central haemodynamics as a response to
psychoemotional stress were assessed. 186 healthy volunteers (at the age of 19-25,
male and female) were examined when they were calm and when they experienced
the emotional stress. The dynamics of the emotional state (by assessing the indicators
of anxiety, aggression, depression) and the changes of the haemodynamics (by using
the integral rheography of the body) were researched. The results of our experiment
showed that the value and the focus of the haemodynamic processes depend not only
on expression of the forming tension, but also on individual characteristics and
peculiarities of the organism.

Significant and constantly growing loading on psychoemotional apparatus of a
modern person makes extreme demands to the systems of adaptation and can lead to
the overtension of the regulative mechanisms [8]. Character of the body responses
depends on physiological activity of nervous, endocrine and cardiovascular systems

[7]. Mechanisms of the development of general adaptive syndrome (GAS) -
nonspecific response of an organism - are identical for various individuals. Despite of
the generality of typical adaptive reactions to adverse influences experimental
supervisions show, that there are groups of people possessing certain stability to
emotional stress. Individuality of reactions depends on many factors of the external
and internal environment and, first of all, from genetically caused properties of
systems of adaptation, and also from psychological peculiarities of a person [1,5].
Recently the concept of so-called «negative affectation» is introduced in
modern literature. “Negative affectation” means tendency to experience negative
emotions and to create the negative "I-concept", including the most significant
negative feelings - anxiety, depression and aggression, - emotions, very often
developing under the influence of stress-factors [3]. People with high negative
affectation experience distress, discomfort and dissatisfaction in the situations even
when vital circumstances as a whole are favorable for a person [6]. The question
about the connection between haemodynamic peculiarities of regulation and changes
of an emotional state is still not solved. Emotional dynamics is the change of
manifestations of personal properties in conditions of stress in comparison with an
initial condition. It can be positive (when expressiveness of individual properties
grows during stress) or negative (when the initial level of personal peculiarities
display is higher, than during stress). It is not known which dynamics is adaptive,
useful to an organism. The question about the role of emotional dynamics as a trigger
mechanism of individual adaptation is not investigated in general, especially there are
no information about functional changes in effector (cardiovascular system) and
regulative systems of an organism. There are no full data on distribution of types of
blood circulation among people with different variants of the emotional answer
(bearing in mind such individual properties as anxiety, aggression, depression).

of central haemodynamics during emotional stress among people with different types
of emotional state.

M a t e r i a l s a n d m e t h o d s o f t h e r e s e a r c h : the
emotional state was examined in our research: anxiety (personal (Ap), reactive (Ar)),
aggression were assessed by the scale of Spielberg - Khanin; the level of depression
was defined by the questionnaire of V.Zung; the vegetative index of Kerdo (the most
simple parameter of adaptation) was calculated for the definition of sympathetic and
parasympathetic influences of the vegetative nervous system [4]; the index of
functional changes was calculated either [2]. In R.M.Baevskii´s and A.P.Berseneva's
opinion (2008), assessment of a level of functioning (the especial characteristic of a
homeostasis) by using the index of functional changes, at all simplicity, provides the
system approach to the decision of the problem of quantitative measurement of the
health degree. The types of blood circulation were defined with the help of integral
rheography of a body (the computerized monitor complex of cardio-respiratory
system and hydration of tissues, KM - АP-01 Diamant).
Researches were carried out twice: in an initial condition and in a situation of
psychoemotional tension. Exam served as a situation of emotional stress.
186 healthy volunteers (at the age of from 19 till 25 years; 68,8% - female,
31,2% - male) have taken part in the research.
Statistical computing was carried out with the help of parametrical (criterion of
Student) and nonparametric methods (the methods of Wilkokson-White, Shapiro-
Wilkes).
R e s u l t s o f t h e r e s e a r c h a n d d i s c u s s i o n : as a result
of the carried out experiment all examinees have been divided into 3 groups
depending on dynamics of the emotional state. The first group included persons with
equilibrium emotional dynamics (Ap = Ar) - 9 persons, the second group (51
persons) contained examinees with great individual anxiety (personal anxiety was
higher than reactive anxiety - Ap › Ar). The third group submitted by persons with
positive emotional dynamics (Ap ‹ Ar) had the greatest number (126 persons).
In our research statistically significant distinctions between indices in initial
condition and during emotional stress-formation have been revealed at the
comparative analysis of anxiety (personal and situational), depression and aggression.

So, in all groups the increase of level of depression (that was the greatest in the third
group - with 34,3 in a background up to 40,9) was marked during examination (р ‹
0,01). The comparative analysis of aggression as an individual peculiarity and
aggression as a reaction to stress situation has shown, that the authentic increase in
parameter of aggression was observed only in the third group (р ‹ 0,05).
The investigation of the correlation of emotional state and peculiarities of the
vegetative balance has revealed close correlation between them.
Assessing the activity of vegetative nervous system it has been established that
activity of parasympathetic nervous system during stress predominated among 55
persons (from the general number examinees), parasympathetic nervous system
prevailed among 120 persons, and 11 individuals showed the balance between the
activity of sympathetic and parasympathetic departments of the vegetative nervous

In our research persons with domination of parasympathetic regulation
constituted practically half of examinees in the second group (41,2%), and only a
quarter (25,4%) in the third group. According to data presented by L.E.Panin and
V.P. Sokolov (1981), prevalence of tone of parasympathetic nervous system is typical
for “low-anxious” people. Among individuals with high anxiety the index of
sympathetic nervous system dominates. It is known, that the most successful
adaptation is provided with prevalence of sympathetic influences above
parasympathetic.

It has been revealed that in the first group tension of the regulative mechanisms
took place among 4 persons (44,4%) during emotional stress, and 1 person (11,1%)
showed unsatisfactory adaptation.
In the second group 36 persons had satisfactory adaptation, 1 person (1,9%)
had unsatisfactory adaptation, 14 persons (27,5%) experienced tension of the
adaptive mechanisms.
In the third group 81 examinees (64,3%) had satisfactory adaptation, 10
persons (7,9%) had unsatisfactory adaptation, tension of the adaptive mechanisms
was observed among 33 persons (26,2%). It is necessary to note, that in this group the
failure of adaptation was revealed among 2 individuals.
Results of the research have shown, that psychoemotional tension revealed
itself by different focus and intensity of changes of the base parameters of the system
of blood circulation.
The peculiarities of haemodynamic changes have been investigated in the
second and in the third groups (the first group was not taken into account because of
small sample for the analysis). In the third group, described by increase of level of
anxiety, the increase of heart rate (on 25%), blood pressure (on 12,5%) were
observed. Thus 54% of examinees were characterized by hyperdynamic type of blood
circulation: cardiac output increased in 25,6%, minute blood volume increased in
60,3%. 46% of examinees showed hypodynamic type o blood circulation: minute
blood volume decreased up to 3,83 l/minute (picture 2А).

The decrease of anxiety (Ap › Ar, however reactive anxiety is regarded as high
in the specific limits), tension of the regulative mechanisms (71,4%), changes in the
functioning of cardiovascular system were observed among individuals with
“negative” emotional balance (the second group) (fig. 2B). During exam all persons
developed tachycardia (in 1,4 times from initial). 57% of all examinees showed the
decrease of arterial pressure or blood pressure without changes. The type of blood
circulation was hypodynamic (with sharp decrease of cardiac output and minute
blood volume) in 43%, normodynamic in 28%, hyperdynamic in 29%.
It is necessary to mention, that quality of the "useful" result (mark for the
exam) was approximately identical in all groups: about 20% of excellent, 60% of
good and 20% of satisfactory marks.
T h e c o n c l u s i o n : apparently, all types of emotional dynamics have
adaptive value. However obtaining the useful result is caused by various types of
psychoemotional reactions with different degree of tension of adaptive processes
which, probably, is determined by different intensity of stress - limiting systems.
Thus it is defined, that the value and the focus of the haemodynamic answer depend
not only on expression of the forming tension, but also on individual - typological
characteristics and peculiarities of an organism.

S u m m a r y : At the present time according to successes of surgery there is
the possibility of eradicatory low invasive surgical treatment of localized renal
cancer. The problem of a great importance is the selection of patients for saving-
organ operations inclusive of absolute and secondary indications. Complex
ultrasound examination at renal cancer allows to diagnose the character of the renal
blood flow, what is important for exact diagnostics and monitoring after surgery.

S u m m a r y : In a series of 10 multifocal epilepsy patients it was shown that
the data obtained using neurophysiologic and neuroimaging studies made it possible
to localize epileptic foci in functionally important areas to apply the method of
subpial transsection and achieve a stable therapeutic effect.

S u m m a r y . The high level of oxidized low density lipoprotein (OxLDL)
and autoantibodies to OxLDL were revealed in sera of patients with ОА at 60% and
29% of cases accordingly. Thus OxLDL and autoantibodies to OxLDL may have
important significance in pathogenesis of ОА.

S u m m a r y : This article is devoted to modern methods of therapy in
treatment of staghorn calculi. It is generally known that staghorn calculi are the most
difficult form of kidney stone disease. According to data provided by the Ministry of
Healthcare for the last four years illness rate has grown from 405,2 till 535,8 per
100 000 thousand people. The main objective of our research was to explore
effectiveness of examination and treatment of patient wit staghorn calculi with
maximal removing of calculi and minimal painfulness while applying modern
visualization and treatment methods. This aim was achieved owing to advanced pre-
surgical, examination, relevant antimicrobial therapy with due account for individual
peculiarities of patient. Grounding on obtained information it may be concluded, that

at the present time DLT is one of the most effective and safe methods of staghorn
calculi treatment.

coronary artery bypass grafting surgery (CABG) and valve replacement in the selling
of assisted circulation. Therefore ihe cerebral proteclion becomes llic crucial pan of
preoperative care. VJe performed prospective randomized conirolled trial to estimate
the efficacy of cytoflavin in 103 patients with ischemic heart disease (ICD) and
acquired valve disease who undergone CABG and valve replacement surgery. The
patients were divided into 4 groups. Group 1 (main) consisted of 32 patients with
ICD (mean age 55.717.9 years) who underwent CABG and received 20 ml of
cytoflavin i.v. in 3 consecutive days before and 3 days after surgery. Group 2
(control) included 40 patients with ICD (mean age 48.2± 12.6 years) and CABG
without cytoflavin. Thirteen patients with valve disease (mean age 53.1±8.4 years)
were randomized in Group 3 (main); they underwent valve replacemeni and were
treated with cytoflavin using the same regimen (20 ml i.v. in 3 days before and 3 days
after surgery). Group 4 (control) was comprised of 18 patients with valve disease
(mean age 48.2± 12.0 years) and surgery without cerebral protection with cytoflavin.
One month after the intervention sig nificant improvement (p<0.05) was observed in
Group I vs. Group 2 in the dynamic modality of cognitive performance, recent
memory and long-term memory, and clear trend to improvement in the same

variables in Group 3 vs. Group 4. Moreover, in Group 2 (CABG without treatment)
significant deterioration was observed in long-term memory (p=0.028), along with
the distinct trend towards worsening in Group 4 in recent and long-term memory. As
an effective antioxidanl. cytoflavin renders reliable ncuroproiectivc effect in patients
who underwent CABG in the selling of assisted circulation and improves cognitive
functioning and emolional well-being, Cytoflavin ameliorates absolute characteristics
of long-term and recent memory in patients after valve replacement surgery.
Cytoflavin improves absolute parameters of cognitive evoked potentials (P300)
decreasing the latency and increasing the amplitude in patients after CABG and valve
replacemeni. Cylollavin is recommended for use in patients with ICD and acquired
valve disease who underweni CABG and valve replacemeni in he setting of assisted
circulation.

S u m m a r y : One of key factors of endogenous intoxication development at
a widespread purulent peritonitis is a misbalanced free-radical oxidation.
Hyperactivation of peroxide lipid’s oxidation and decrease of antioxidant activity
underlie infringement cell’s mitochondria function. The carried out researches have
shown that metabolic remedy «Cytoflavin» possesses expressed antioxidant
properties, provides effective and fast restoration of peroxide lipid’s oxidation and
antioxidant activity balance, increases speed of free radicals elimination.

S u m m a r y . The article presents authors’ own material and literary data on
cycloferon pharmacotherapeutic efficiency. Cycloferon belongs to a group of
interferon inducers, immunomodulators. The effect of the preparation at various
children socially-significant diseases is described. These include ARVI, bronchial
asthma, allergic conditions accompanied by the disorder of antiinfectious protection,
mycoplasmic infection, broncho-pulmonary complications in ARVI with low
intensity of free-radical oxidation. Suppression of autoimmune processes, which are
the cause of postvaccinal complications in often ill children, is shown with the use of
cycloferon on the background of planned vaccination. Results of cycloferon use in
case of gastrointestinal tract diseases, intestinal infections both of viral and bacterial
genesis are described. Cycloferon use for intestinal dysbiosis correction is suggested,
microflora restoration to normal level is noted in 95 % of children. Preparation use
for surgical pathology, particularly in appendicular peritonitis, with the aim of
decreasing postoperative complications, and also for the correction of immune
disorders in chronic viral hepatitis C and B in children treated with the combined
therapy is described. Cycloferon safety and efficiency is proved by the postmarketing
randomized studies of its efficiency.

S u m m a r y : Correction of hemodynamic disorders in acute right ventricle
insufficiency (ARVI) due to myocardium infarction (MI) of the right ventricle
usually consists of treatment by inotropic maintaining drugs and preload increase on
the right ventricle (RV) for increased output into pulmonary artery. However at
present, there are no precise recommendations on fluid introduction in ARVI
treatment.
To determine optimum criteria of infusion therapy 42 patients with MI of the
lower localization and MIRV were included into the study. MIRV was complicated
with ARVI. All patients were prescribed dobutamin and reopolyglukin (300 ml/hour)
was introduced intravenously under pulmonary artery pressure (PAP) control,
jamming pulmonary artery pressure (JPAP), central venous pressure (CVP).
Perfusion cardiac index (CI) was determined by thermodelution method.
Echocardiographic examination was performed before treatment and at optimum
JPAP values.
It was found that in ARVI patients at initially low APP and JPAP (16.2±0.21
and 7.0±0.18 mm hg) CVP is increased (11.2±0.25 mm hg). On the background of

reopolyglukin introduction CI increased from 1.82±0.03 to 2.51±0.04 l/min/m2 (p
<0.001) at JPAP of 18 mm hg. At the same time no marked CVP increase was
observed. Further fluid introduction with JPAP up to 20 mm hg did not result in
further CI increase and was followed by substantial CVP increase (from 13.7±0.23 to
16.2±0.24 mm hg; p <0.001). On the background of treatment LV size increased
(from 52.3±0.57 to 54.4±0.60 mm increased; p <0.05) and fraction output also
increased (from 37.5±0.87 to 41.9±0.99 %; p <0.05) in the absence of significant
dynamics on the right cardiac chambers.
The obtained data show expediency of preload increase on the right ventricle
for ARVI treatment. Fluid should be introduced together with inotropic maintaining
drug therapy. Target JPAP in IRVI treatment is 18 mm hg at which FV volume LV
increases thus showing the improvement of its filling. Fluid infusion is not expedient
at CVP increase higher than that of JPAP. In this case final increase of diastolic
pressure in LV is leveled by higher rates of RV filling pressure and no increase LV
filling is observed.

S u m m a r y : In the present work there was conducted the research of
dynamic of thresholds of tactile sensation during the psychical relaxation with
psychogenic pain as exemplified by patients with anxiety phobic diseases versus
patients, affected somatogenic pain, and healthy people. Assessment of patients’s
psychophysiological state of both groups was being carried out in the beginning and
at the end of psychological correction course. There was used the empatho-technique
as method of psychical relaxation.

The pain is one of the most popular and relevant complaint in clinic practice of the
therapeutist. The problem of her control is important practical task. In order to choose
adequate methods of therapy it is necessary to comprehend particularities of
pathogenesis of pains disorders, of which general groups are somatogenic,
psychogenic and neurogenic pain [5, 6, 1, 9]. Psychogenic pain is of the most interest,
of which progression mechanism supposes the leading significance of negative
emotional state, which initiate the pain in the absence of serious somatic diseases [4,
7, 1]. Therefore, determination of the leading pathogenetic mechanism, that is at the

root of occurrence of pain syndrome, contributes the improvement of effectiveness of
rendering assistance. In view of this the present work describes studying of changes
of tactile sensitivity in patients with psychogenic pain as exemplified by anxiety
phobic diseases during the psychological correction versus the patients with
somatogenic pain. We have assumed that threshold of tactile sensation depend on
sustained by the time of research feeling of internal and external irritants, that can
change under the influence of stress factors. It was also expected that instability of
excitative process, that is linked with psychoemotional stress, must be in the picture
of alteration of wavering of threshold of tactile sensation during the psychological
correction with psychic relaxation’s application.
T h e p u r p o s e o f s t u d y. Study of the interrelationship between the intensity
of painful, the intensity of negative emotional state and tactile sensitivity of patients
with anxiety phobic diseases.
Materials and methods of research.
Research was carried out for 42 patients with anxiety phobic diseases (11 men and 31
woman ) with cephalagia, abdominal, vertebra pains, which had psychogenic nature
(Group 1). Patients with anxiety phobic diseases had complaints on high anxiety,
obsession fears, anxiety, sleep disturbance.
As a control group there was researched:
- 30 persons (22 women and 8 men) with somatogenic pain (group 2). They had
complaints about cephalgia (craniocerebral trauma in parity), abdominal (there was
educed cholelithiasis) and vertebragenous pain (there was found thoracic and cervical
spine osteochondrosis). Patients was examined in the clinic № 45 of SPb, where were
made relevant diagnoses.
- 54 healthy subjects (40 women and 14 men), who didn’t have neurosal and somatic
complaints and presented the group «conditionally healthy persons».
An average age of researched persons was 34, 07±1,41.
Psychological testing was conducted at the beginning and end of the course of
psychological correction. There were used the anxiety scale of Spielberg-Chanin and

scales of enquirer of psychopathologic symptomatic SCL-90-R [10] for assessment of
emotional state. Pain intensity was identified by the scale «pain» from an enquirer of
life quality MOS SF-36 [8]. Therewith the factor was the better, the higher. All
enquirers are validated.
There was used the plant consisted of the generator and focalizing ultrasonic
source (frequency of focused ultrasound 1,7MHz) for tactile sensitivity research [4].
Thresholds of tactile sensitivity ware determined on the skin of terminal phalange of
right arm forefinger. For that end the finger was placed on the room-temperature
water surface in such way that the medium of phalange of forefinger was in the center
of focalizing source. Impulse intensity was taken as the threshold of tactile sensation,
that conveyed a tactile sensation not less than 2 times during 3 presentation. The
tactile sensitivity was researched with persons of all groups: 1. healthy people –
before and after one psychical relaxation session; 2. patients with ТФР and persons
with somatogenic pain – before and after every psychical relaxation session during
the whole course of psychological correction. There were taken measures for analysis
that was got before and after the first empatho-techniques session and also before
and after lat session.
There was used psychical relaxation method (empatho-techniques) for
management of psychogenic pain and leveling of negative emotional state, that
presented in detail in the previous work [2, 11].
Statistical processing of the data obtained was carried out using the STATISTICA
v 5.5 computer program.
R e s u l t s a n d d i s c u s s i o n
At the beginning of the course of psychological correction patients with APD had
the pronounced pain (56,86±4,12), whose factor was credibly (p < 0.01) lower than in
the group of healthy subjects (77.57±2,56) and credibly (p < 0.05) higher than in the
group of subjects with somatogenic pain (41,3±3,59). Pain senses of the patients with
APD were accompanied by excessive values of the questionnaire SCL-90-R on all
scales and high level of ЛТ (Table 1). In most cases all found factors were credibly (р
< 0,01) higher that for the patients with somatogenic pain (Table 1).

Empatho-techniques use permitted to improve the emotional state of the patients of
both groups, that conduced to decrease of the intensity of the pain. Thus at the end of
psychological correction course there was the credible (p<0,001) decrease of pain
intensity (78,21±2,93) in the group of patients with the APD, whose factor didn’t
have credible difference from healthy subjects factor versus patients of group 2
(50,27±2,62). Patients with somatogenic pain had continued to feel pain against the
end of course, that can be linked with the particularities of mechanisms of
psychogenic and somatogenic pain formation.

Against the end of the psychological correction course there was noted credible
(р < 0,01) decrease of all researched factors in the group of patients with

psychogenic and somatogenic pain. However their average values at the group
of patients with APD and subjects with somatogenic pain didn’t have credible
differences but were credibly different from healthy subjects factors (Table 1).
The research of thresholds of tactile sensitivity indicates that their average values
didn’t have credible differences in all researched groups before and after the
empatho-technic session in the beginning and at the end of the psychological
correction course (Diagram 1).

However by comparison with group of healthy people there were revealed
credibly (p < 0,05) higher values in the subjects with psychogenic and somatogenic
pain in the beginning of the psychological correction course, and in subjects only
with somatogenic pain at the end of the course, that can be determined by differences
in mechanisms of psychogenic and somatogenic pain formation (Diagram 1). For
research of the intensity of the thresholds of tactile sensation changes we were
defining the difference between factors of absolute thresholds of tactile sensation got

before the empatho-technic session and after that for every studied subject, and
specified that difference as delta (│∆│, dB).
∆1 complied with 2,56±0,33 dB in patients with APD before psychological
correction that was credibly higher than in subjects with somatogenic pain (1,37±0,18
dB) and healthy people (1,23±0,12 dB). ∆2 complied with 1,57±0,21 dB in patients
with APD at the end of the course that was credibly higher than in subjects with
somatogenic pain (0,90±0,15 dB) but came around to delta value of healthy people
(Diagram 2).

Conclusion. Therefore, according to received results, there was found that pain of
high intensity in the patients of both groups was accompanied by negative emotional
states and expressed increase of tactile sensation delta before psychological
correction. Negative emotional states leveling contributed to psychogenic pain
management and credible decrease of tactile sensation delta at the end of the course
that can argue about recovery of regulatory processes and correlation of the central
and peripheral tactile sensation links and reflect the optimization of the CNS
functional status indirectly. Thus, according to earlier researches there was noted the
reduction of the neurodynamic processes fluctuation in terms of brain bioelectrical

activity (BEA) and the reduction of excitative processes expression according to the
data from driving rhythm of the photostimulation in the patients with APD [2].
Correlation relationships analysis showed that the pain of subjects with APD was
interrelated with the scale «somatization» factor as corporal equivalent of anxiety,
and tactile sensation delta – with the aggression factor. The pain factor of subjects
with somatogenic pain was correlated with interindividual sensation factor, the pain
factor of healthy subjects – with depression.
C o n c l u s i o n s
1. The psychogenic pain of the patients with APD is accompanied by higher
values of aggression, anxiety, depression factors and expressed tactile
sensation delta.
2. The psychorelaxation methodology contributes to credible reduction of
negative emotional states intensity, credible reduction of tactile sensation delta
and psychogenic pain management.
3. The somatogenic pain is credibly more expressed in the beginning and at the
end of psychological correction course that psychogenic, and tactile sensation
delta is credibly higher in subjects with APD rather than in subjects with
somatogenic pain.

S u m m a r y : The clinically based Atlanta Classification was formulated in
1992, but in recent years it has been increasingly criticized. No formal evaluation of
the use of the Atlanta definitions of predicted severe pancreatitis in the literature has
ever been performed. We have performed such analysis and concluded: complex
evaluation of different laboratory and instrumental data is more reliable instrument
than conventional APACHE II ≥ 8.

S u m m a r y : The study observes contraction function of striated mouse
muscles at protein sensibility – m. diaphragm, m. extensor digitorum longus, m.
soleus. It is shown that protein sensibility changes contractile force of all researched
muscles. Thece changes differ for muscles with different fenotipe.

Well-known, that airway muscles functional state and, primarily, diaphragm, is essentially
changed in asthma, the disease of allergic origin. We have previously studied the influence of
ovalbumin sensabilization on contraction function of guinea-pig isolated diaphragm strip. To be
more concrete, its ability to change its contractile functions by increase of force and shortening
velocity on carbacholine (CCh) in terms of ovalbumin sensabilization was shown. To explain this
fact, changes of electrogenic features as well as histochemical profile of muscle fibers were
proposed. Furthermore, as the contraction of isolated muscle was initiated by agonist we can
suggest the participation of surface membrane cholinergic receptors activation mechanisms in the
change of muscle fibers functional features at ovalbumin sensabilization. Diaphragm is a mixed
muscle; it consists of “fast” and “slow” muscle fibers (MF). Recently we showed that ovalbumin
sensabilization changes contraction functions of isolated – “fast” and “slow” – shank mouse skeletal
muscles. Dynamics of contraction function of studied skeletal muscles shows remarkable
differences. For clarity, cholinemediated processes of excitement of postsynaptic membrane play an
important role in the mechanisms of contraction force change. The ability of mouse diaphragm to
change its functions in terms of allergic restructure and its possible mechanisms remain to be
determined.
T h e p u r p o s e of the present study is to reveal cholinemediated mechanisms in
pathogenesis influence of ovalbumin sensabilization on contraction functions of mouse isolated
muscles: “mixed” – diaphragm strip (m. Diaphragma); “fast” - m. extensor digitorum longus (m.
EDL) and "slow" – m. Soleus.

The complex researches were performed to investigate this problem. Two experimental
models which are characteristic for cholinemediated processes of excitement of isolated mouse
striated muscles were used to study the effects of ovalbumin sensabilization on: 1) the rates of
contraction muscles response, caused by agonist CCh and 2) the level of non-quantal secretion of
acetylcholine (ACh) in a zone of a trailer plate.
M a t e r i a l s a n d m e t h o d s . Experiments were conducted on both sexes mice
weighing 17-22 g. Animals were twice ovalbumin sensabilitized (OS) with gel hydrate aluminum (2
mkg of dry gel substance + 150 mkg ОА in 0,5 ml of a physiological solution) parenteral. The
second injection was made in 14 days after the first one. Animals were got into experiment on a
pique of sensabilization – 7-10 days after the second sensabilization injections. Mechanomiographic
researches were conducted on a preparation of an isolated muscle in terms of an isometry. Skeletal
muscle was stretched during 20 minutes with force of 500 mg at a constant perfusion by a solution
Krebs type to maintain isometry and temperature at 20-21 C. Contraction was recorded by the
photoelectric converter. Agonist CCh was investigated at submaximal concentration: for m. EDL –
7х10-4М, a m. Diaphragm strip - 2х10-4М, m. Soleus – 5х10-4М. Contraction function was analyzed
according muscles contraction parameters on CCh. Muscle contraction force (Poc) and speed (Vос)
were estimated. Contraction force developed by isolated muscle was related to its mass (m) (Poc* -
numerically equal volume of a muscular preparation) to get objective information at the force
characteristics analysis.
To study a condition of muscle fiber postsynaptic membrane in the zone of a trailer plate non
quantum secretion of Ach was studied. It was measured by glass microelectrodes (with the
resistance of 8-12 MΩ, filled with 2,5 M KCl). To determine its size armin action
acetylcholinesteraze, then on a muscle was eliminated during 8-12 minutes application m-
cholinergic receptors blockader d-tubocurarine (d-TC) (10-5М). The rates difference of membrane
potential before and after application d-TC corresponds to the rate of not quantum secretion of ACh
(Н-effect).
The results were analyzed by comparing data from experiments on intact muscles (control)
and sensibility animals (experiment). Statistical analyzes was performed by Student's t test.
R e s u l t s . Biometric parameters of the studied skeletal muscles are represented in Table
1. Contraction parameters of isolated m. EDL, a m. Diaphragm strips and mouse m. Soleus on CCh
at submaximal concentration in the control and at ovalbumin sensabilization are represented in
Tables 2, 3 and 4.

For "fast" muscle it is shown that CCh at submaximal concentration caused contraction of
with force of 76,59±6,51 mg and speed of 14,3±1,6 mg/s. m. EDL contraction force decreased,
speed practically did not change at ovalbumin sensabilization (Table 2).

Study of non quantum secretion of ACh in "fast" mouse muscle fiber has shown the following
data. Initial rate of membrane potential in the terms of rest was 72,3±0,6 мВ (n=150). But at
presence of d-TC it increased up to 77,4±1,6 мВ (n=150). Thus, the Н-effect in the terms of control
makes 5,1±0,4 мВ (n=150). In the terms of ovalbumin sensabilization initial rate of membrane
potential of the rest was 73,9±0,5 мВ (n=150). At presence of d-TC it increased up to 79,7±1,7 мВ
(n=150). It means that the Н-effect value has increased, making in the described terms of
experiment 5,8±0,5 мВ (n=150, р <0,05).
In the "mixed" on intact muscle mouse CCh at submaximal concentration caused contraction
with force of 342,8+-18,54 mg and speed 31,0+-1,7 mg/s. Ovalbumin sensabilization resulted in
force increase and speed increase - 32,08+-0,89 mg/s of slow muscle contraction (Table 3).

Study of non quantum secretion of ACh of "mixed" muscle has shown: initial rate of
membrane potential of the rest was 70,7±1,9 мВ (n=150). At presence of d-TC it increased up to
75,9±0,7 мВ (n=150). Thus, Н-effect in the control makes 5,2±0,4 мВ (n=150). Initial rate of
membrane potential of the rest was 70,0±1,5 мВ (n=150). At ovalbumin sensabilization at the
presence of d-TC it increased up to 74,4±0,6 мВ (n=150). Tit means that the Н-effect value
decreased, making in the described terms of experiment 4,4±0,5 мВ (n=150, р <0,05).
At "slow" intact mouse muscle CCh at submaximal concentration caused contraction with the
force of 237,77±20,61 mg and speed of 13,1±1,0 mg/s. Ovalbumin sensabilization resulted in
force and speed increase (Table 4).

Study of non quantum secretion of ACh has shown: initial rate of membrane potential of rest
was 70,9±1,7 мВ (n=160). But at presence of d-TC it increased up to 75,9±1,3 мВ (n=160). Thus,
Н-effect in the control makes 5,0±0,7 мВ (n=160). Initial rate of membrane potential of rest making
69,4±0,9 мВ (n=150) at ovalbumin sensabilization at presence of d-TC increased up to 72,5±1,0 мВ
(n=150). It means that the Н-effect value decreased making in the described conditions of
experiment 3,1±0,6 мВ (n=150, р <0,05).
D i s c u s s i o n . Results of studies indicate that ovalbumin sensabilization
changes contraction function of diaphragm strip as well as isolated shank mouse
skeletal muscles (Table 2-4). To clarify, nature of these changes is essentially
different for “fast” and “slow” muscles. Basic differences of morph-functional status
of studied objects and its change mechanisms in the process of allergic restructure of
an organism can explain this fact.

Presumably, the fiber structure defines the differences in contractile force of
observed muscles of nonsensibility animals on CCh. Soleus mouse muscle contains
50-60% of “slow” filaments, m.EDL – 97-100% of “fast” filaments. Mouse
diaphragm, that takes media position, contains 88.6% of fast myosin. Presumably,
force differences result from different level of muscle fibers sensibility to CCh that
suggests direct dependence on the area of synapse. It is known that the size of a
trailer plate of “slow” muscle fiber of soleus mouse muscle is 3 lengths than that of
“fast” muscle fiber (m.EDL). Considering similarity of biometric parameters (length
and mass, Table 1) of observed muscles, more sensibility to cholinomediate, caused
by greater number of cholinergic receptors in the area of synapse, must result in more
contractile force of m. soleus and diaphragm on CCh.
Changes of diaphragmatic muscle functional features in terms of ovalbumin
sensabilization suggest that changes in muscle fiber during allergic restructure of an
organism are complex. Changes occurring in muscle fiber during sensabilization may
affect surface membrane, electromechanical connected mechanisms or contractile
protein system. The absence of corresponding changes of shortening velocity on sub
maximal concentration of agonist of all the muscles doesn’t suggest
electromechanical connected system changes. However, various of force change
vector of “fast” muscle from the one side and “mixed” and “slow” muscles from the
other side indicate principle difference of change mechanisms of muscles` functional
features at ovalbumin sensabilization.
While contractile force (Poc*) of “fast” muscle decreases (Table 2), contractile
force of “slow” and “mixed” muscles increases (Table 3, 4). This dynamics confirms
the fact that differences of “fast” muscle from the one side and “mixed” and “slow”
muscles form the other side at ovalbumin sensabilization affect, primarily,
cholinemediate processes of excitement of muscle fibers manifold effect.
Evidence that ovalbumin sensabilization is able to affect the mechanisms of
excitement of postsynaptic membrane of different muscles in different way comes
from comparing muscle contractile force dynamics on CCh with level change of non-
quantal secretion in the zone of a trailer plate. Force change vector of observed

muscles at ovalbumin sensabilization correlates with level change of non-quantal
secretion of Ach in the zone of a trailer plate (H-effect) (Figure 1A, 1B)

We may conclude that the decrease of contractile force of “fast” muscle on
CCh results from the decrease of postsynapse sensibility to cholinomediate. The
evidence of this fact comes from the increasing H-effect (Fig. 1A, Б). Increase of
non-quantal secretion of Ach in the zone of synapse contributes intensification of
desensabilization mechanisms of cholinergic receptors of postsynaptic membrane.
Correspondingly, we observe reverse picture about “mixed” and “slow” muscles.
Increase of contractile force on CCh results from increase of postsynapse sensibility
to CCh. Decreasing H-effect reflects this process (Fig. 2A, Б, 3А, Б).

C o n c l u s i o n . Thus, allergic restructure of an organism causes changes
of contraction function of isolated mouse skeletal muscles. Contractile force (Poc*)
on CCh of “fast” muscle decreases and, correspondingly, of “mixed” and “slow”
muscles increases. This rate change occurs from the following. Decrease of
contractile force of “fast” muscle (m.EDL) results from decrease of postsynaptic
membrane sensibility to CCh that is caused by increasing of non-quantal secretion of
Ach in the zone of a trailer plate. “Mixed” and “slow” muscles show reverse
dynamics of contractile force as well as of non-quantal secretion of ACh. Increase of
contractile force of these muscles on CCh at ovalbumin sensabilization results from
increasing postsynaptic membrane sensibility to cholinomediate that is caused by
decrease of non-quantal secretion of Ach in the zone of a trailer plate. Different
changes of contraction function of skeletal muscles at ovalbumin sensabilization are
caused, primarily, by dynamics of cholinomediated processes of membrane
excitement of muscle fibers.

chronic liver diseases (CLD). However, during the PPh it is nesessary to predict the
risk of complications risk. The aim of our study was to offer the method of prognosis
of plasmapheresis complications at the chronic liver diseases patients. The basis of
this method – polihepatography (PHG). PHG is method of estimation liver

S u m m a r y : On the basis of worked out biophysical body’s model, by the
method of bioelectrical impedance analysis, it was determined at 129 patients with
decompensated liver’s cirrhosis the quantity of aesthetic liquid in the abdominal
cavity. Taking that into account, the body’s mass and the index of body’s mass were
calculated. It was determined that 55,8% examined patient had got hypotrophy. The
degree of hypotrophy was associated with the length of alcohol’s abuse, with

accumulation of radiopharmpreparation in the spleen, with dimensions of the liver
and the spleen, with circumference and coliperometric facts.

S u m m a r y : The contemporary complex characteric of the skin basal cell
carcinoma (SBCC), one of the most frequent neoplasm, is present. The clinical course
of the SBCC strongly depends on the topographic form, proliferative activity and
stromal reactions. The prognostic value of these morphological markers is discussed.

S u m m a r y : The article deals with biopharmaceutical studies concerning
comparative evaluation of antiviral drugs protective activity (cycloferon, remantadin,
Tamiflu). Cycloferon use in influenza experimental infection results in post-influenza
pneumonia focus limitation. This effect is noted in infection with all of the studied
influenza A viruses, irrespective of their sensitivity or resistance to the drugs of
comparison. Cycloferon protective activity ranges from 11% to 50 % depending on
the used virus dose and modeling strain, cycloferon protection index was 35-41 %.
Cycloferon was shown to be a drug influencing both natural immunodefence system
(endogenous interferon induction), and processes of proper virus morphogenesis,
reducing infection properties of the virus progeny.

S u m m a r y : 73 blood and lymphocyte samples from blood donors have
been tested by ELISA. One HBsAg positive blood serum sample has been revealed
and HIV markers were not revealed. 9 HBsAg one HIV markers positive

lymphocytes samples have been revealed. These results with justification of
lymphotpropic features of HBV and HIV indicate the great practical value of these
features of viruses, improving the quality and reduction of false-negative results
during ELISA of blood donor’s samples for HBsAg and HIV.

S u m m a r y : With the purpose to study the mechanisms of formation and
peculiarities of periodontitis course in patients against the background of focal
tuberculosis, the examination of 45 patients divided into three basic groups (patients
with periodontitis and HIV-infection; patients with periodontitis; and HIV-infected
without periodontitis) has been carried out.
It was determined, that the patients with periodontitis and focal tuberculosis
had the development of local inflammatory reaction with infectious assignment,
activation of anti-inflammatory cytokines in parodontal pockets’ fluid. It was also
proved, that the cause factor of frequent and long relapses in parodontal pathology in
focal tuberculosis was the development of pathologic process against the background
of endotoxicosis syndrome, disbalance of lipid peroxidation and antioxidant system.

S u m m a r y : In 79 children acute intestinal infections (AII) the correlation
of dynamic of antigen-binding lymphocytes (ABL) level sensibilized for tissue
antigen (TA) of kidney and severity of acute renal failure have been studied. ABL for
the kidney tissue may indicate the acute renal failure risk and degree of ARF in
children with AII.

S u m m a r y : 67 patients with severe and moderate typhoid fever have
been tested for dynamic of cortisol, ACTG and ABL (antigen-binding
lymphocytes) against tissue antigen of adrenal cortex. The patients with signs of
glucocroticoidal failure there was reduction of cortisol on the background of ACTG
and ABL rise (up to 8.0% and over); ABL rise up to 4.0-6.0% on the background
of ACTG and cortisol rise is the early sign of glucocorticoidal failure.

S u m m a r y : 337 persons observed before and after vaccination with
typhoid fever vaccine (TFV) spirituous dry Russian Federation manufactured
(TVFSD RF). 157 additionally administered Timoptin™. Before vaccination
moderate weight proteins (MWP), O-antigen S. typhi titer before and 1 month after
vaccination were studied. The vaccination efficacy based on protective titer level
failed in 27.20% with positive effect in 73.80%. Seronegative persons MWP level
before vaccination exceeded the seropositive ones 1.5-2.02 fold. It reflects the
correlation of seronegativity forming with MWP level during vaccination. After
vaccination with stimulation by Timoptin the seronegative effect observed in 3.80%
against 27.20% in vaccinated persons without immune stimulation. Thus, Timoptin
application improved vaccination efficacy up to 96.20% against 73.80% with
vaccination without immune stimulation.

S u m m a r y : Standards of medicinal therapy in ischemic heart disease
include currently five and more medical products which action is directed on various
pathological processes: inflammation, apoptosis, hypercoagulability, atherosclerosis
etc. By means of various laboratory methods of diagnostics it is possible not only to
reveal pathological process, to specify the diagnosis, but also to estimate efficiency of
spent medicinal therapy, to pick up doses and to warn development of undesirable
collateral reactions. In given article the review of laboratory methods which can be
used at monitoring of medicinal therapy sick of an ischemic heart disease (IHT), an
acute coronary syndrome, including a myocardial infarction and their importance in
the estimation of efficiency and safety of medicinal therapy is resulted. The value of
such testing for an estimation of pharmacotherapy is not defined. Modern methods of
laboratory diagnostics which are accessible but not used in every day clinical practice
are also considered in this review.

S u m m a r y : The review is devoted to the modern approach to the therapy
of various clinical forms of herpetic infection (genital, herpes-virus skin infection,
herpes-virus infection type 4 – Epstein-Barr viral infection, infectious
mononucleosis). Factors leading to virus activation and its transition from a latent
form to an active state are described. Genital herpes is characterized. Cycloferon use
in experiment in Vero cell culture is proved, Cycloferon use in herpetic infection is
substantiated.
Cycloferon and cycloferon with acetilovir pharmacotherapeutic and economic
efficiency is shown. It was found that disease outcome resulting in clinical remission
is higher in patients in whom cycloferon is used.
Comparative data on cycloferon liniment and virolex cream application are
given in herpetic skin infections. Advantage of cycloferon liniment use as a means of
local therapy is given. Positive dynamics of cycloferon application for local treatment
of patients with herpes zoster was registered. The authors consider that the
association of herpes-viruses with active participation of opportunistic
microorganisms is involved in the formation of a clinical picture of infectious
mononucleosis. Cycloferon promotes quicker termination of virus activation, allows

to reduce antibiotics use, to minimize by-effects characteristic for chemopreparations.
In 91.7 % of cases of papilloma viral infection, cycloferon promoted prevention of
the infection manifestation, two times decreased the frequency of relapses with more
than 3-fold increase of inter-recurrent period. Restoration of epithelium
morphofunctional state in the areas of herpetic infection was found in 83.3 % of
patients receiving cycloferon as a part of a combined therapy.

S u m m a r y : The article deals with the problems concerning introduction
of Bologna declaration recommendations into educational process of the future
specialists. From the stand point of constructive pedagogics, intensification of
training is suggested, using elements of educational process optimization on the basis
of the modern pedagogical technologies promoting improvement of specialist
training. The evaluation of students’ knowledge, different from the traditional one is

offered depending on the level of training. Attention is given to the problem training
combining inductive and deductive methods, providing activization of informative
activity of students.
