Fellow AGD Members,

I am happy to report the 2017 House of Delegates held in Chicago, in the beginning of November was very successful. Our Educational meeting held this last July in Las Vegas had a strong number of lifelong learners in attendance. This new format of separating our governance and educational meetings has given our members who are in leadership positions an opportunity to more easily participate in our fine CE programs.

Our House of Delegates’ meeting was run very smoothly by our wonderful hard working and talented headquarter’s staff. Our most spirited discussions were regarding the level of student dues. Some states such as Wisconsin support the student dues. Some states with multiple dental schools find supporting dues a hardship at higher dollar levels. The positive and negative positions and implications were passionately debated and voted upon.

Here in Wisconsin we are supporting our student member dues as well as initiating educational and support programs. A number of our WI and IL board members have volunteered to help with this programing. Please consider helping our students with sharing your experiences in diverse types of practice. Short personal stories of practice in groups, military, solo or blends are valuable to students. YOU CAN HELP WITHOUT HAVING TO SPEAK TO A LARGE GROUP.

Changing of the Guard

Hello to all of my fellow AGD members in Michigan and Wisconsin. Let me take this opportunity to offer you my heartfelt wishes for a Happy, Healthy and Prosperous New Year!

After our 2017 Annual Business meeting in November, we say “Thank You” to Dr. Sam Shamoon, our Region IX Trustee who is stepping down. Sam represented our Region with distinction for the last six years. Now, we welcome Dr. John Olsen as he assumes that prestigious position. I am privileged to accept the Regional Director post that John just completed, where he served as chair of the Regional Directors for this past year.

The Regional Directors serve as coordinators between the National AGD and the local, grass roots organizations. The RD’s focus is on the needs of our members. We help maintain and grow the membership. We offer support for our members and offer suggestions to help implement the policies of the AGD.

I invite you to offer your assistance to both State constituents (Michigan and Wisconsin) in any way that you can. Both states need your help as participants who utilize our individual programs and on a leadership level in planning and implementing those events. You, our members, determine our ultimate success.

Our region has three dental schools. Our interactions with the dental students have the opportunity to nurture future members. We have opportunities to demonstrate our Academy values- Excellence in Continuing Education and Advocacy for the General Dentist in organized dentistry and the governmental arenas. If you are involved with one of the dental schools you may be able to help us reach out to our dental students. Please feel free to contact me or one of the state board members if you can assist the AGD.

Wow, I had been trying to think of what I wanted to write about for this newsletter when here comes a news flash from the AGD. For those of you who missed it, the Canadian Association of Orthodontists ran a series of TV ads comparing a general dentist doing ortho to a plumber or hairdresser providing the same service. The AGD steps in and threatened legal action and the ads have ceased.

Sometimes I do feel like there is a lot of talk and little action from our professional organizations, especially where politics are involved. For me though, this is a perfect example of the strength in numbers and the power of AGD. This is how we as general dentists benefit from being members of the AGD. I don’t think Arni or Bruce or even Clint (I know I am aging myself ) could have done it any better. Go AGD.

Congratulation to Dr. John Olsen, our new Region 9 Trustee. Dr Olsen has been active on the WiAGD Board, serving on various committees and holding various offices for many years. He had recently been our Region 9 Director and will be serving as our Trustee. Thanks to John for all his hard work and dedication to the WiAGD/ AGD in the past and in the future.

Congratulation to Dr. Richard Winter, he has received his Diplomate in the American Board of Laser Surgery and will be a speaker at their upcoming symposium in Orlando, Florida on April 6th-8th, 2018. The ALSC is dedicated to promote Science-based Laser Dentistry, Surgery and Safety Education and to help Dentists, Physicians and Veterinarians to excel at efficient and safe applications of laser energy.

If you or someone you know has received an award, promotion, or has accomplished something special we would like to share that. It’s always nice to get a pat on the back. We know we have some outstanding members who have given back to dentistry, or their community, or achieved some special goals. Trustee Report December 2017

This is my first report as your new Trustee for Region #9 (Wisconsin, Michigan). I have completed my 6-year term as your Regional Director and I am pretty sure by now most of you know me or have heard of me. If not, I can e-mail any one in this region my CV. I want all of you to know that when I took over for Dr. Shamoon I had awful big shoes to fill. Dr. Shamoon accomplished much for the Region and the AGD. His leadership skills and mentorship of me was remarkably helpful. My promise to all of you in the AGD is that I will represent the Region as Trustee with the same diligence Sam did. I will be the watch dog for spending our finances in a positive direction for the betterment of the Academy.

The AGD had their first HOD meeting as a separate function apart from the Scientific session. The meeting being held in Chicago, the home of our headquarters, proceeded in a positive direction with the help of all the delegates of the AGD. There were not too many issues brought up, but great strides have been made in developing better directions in membership, continuing education, and advocacy. There was a stance the AGD passed on flossing being important in promoting proper oral care. A white paper was put out and passed on our stance on Opiate use in health care. The importance to all health care providers to solve our costly health care costs upon the US and the addiction problems from their use. We swore in our new President Dr. Manuel Cordero and elected Dr. Connie White as president our vice president. Dr. Michael Lew was elected our new secretary to the AGD.

After the HOD my first board meeting was held with the role call involving five new Trustees from around the AGD. There was not much discussed at this meeting except the approval of several committee members to all our councils.

Again, thank you for trusting me as an elected board member in the AGD. I promise you my due diligence to the best of my knowledge and experience as a leader.

A short refresher on endodontic anatomy will help the clinician choose the most predictable cases. After the establishment of a glide path, the clinician is faced with many options to create the final shape and cleanliness within the root canal system. An evidence-based review of special emphasis on instrument design and mechanical movements, will be covered. Additionally, enhanced irrigation protocols will be explored, prior to presenting state of the art obturation and restorative techniques.

Afternoon Session: Dr. Michael Schuster

Building a One-to-One Whole Person Practice

Michael Schuster, D.D.S. is the Chief Executive Officer of The Schuster Center for Professional Development in Scottsdale, which he founded in 1978 to teach concepts of managerial excellence, economic freedom and personal growth. The Schuster Center is a state of the art teaching facility dedicated to increasing the level of health and patient care in the United States by empowering dental organizations to freedom and profitability through overhead control, resource management, effective and efficient organizational systems and a team that works and communicates successfully with each other and their patients in an insurance free environment. Dr. Schuster is a 1966 graduate of Marquette University and as C.E.O. of The Schuster Center for Professional Development, has been involved at the leading edge of dentistry for over 30 years. He has practiced the concepts he teaches

successfully in a variety of environments including a small, rural area of 3500 people, a Midwestern town of 65,000 and a large metropolitan city of 1,000,000+. He has taught at Temple University in Philadelphia, The University of Iowa. Today, he continues on in teaching as adjunct faculty member for both The Dawson Center and OBI. When Dr. Schuster created this business model, he did it based upon certain principles that enabled him to create his ideal practice – profitable, high quality, able to serve one patient at a time to the very best of his abilities. These principles were proven to be effective in any type of community, in any specialty as long as it fits the dentist’s personal value system for optimal care. At the time he assembled these principles into a cohesive practice development model, the only other model in dentistry was a high volume, production driven model. Additionally, the production driven approach creates a deficit in time available for each patient, handicapping the doctor’s ability to spend adequate time with patients to build relationship, rapport and trust for recommended treatment. This model did not fit Dr. Schuster’s values and it doesn’t fit for many dentists today – especially those with advanced technical training. He has spent a lifetime traveling, teaching and helping dentists obtain the knowledge and apply the skills necessary to bridge that gap.

“Recipe for Success” with direct composites: You Missed It!!! Why?

The course started by reviewing today’s requirements for a composite. Nine “must haves” that are essential to keep us out of the hands of “what’s latest and greatest” vs “tried and true.” Dr. Lambert did a brilliant job of walking the class through todays Posterior Composite Challenges and made sure that we weren’t left hanging. From Stress Vectors to Shrinkage Stress and another list of 7 to make sure that you don’t wind up with that white line when you are finished. This course was an excellent teeter totter of Problem and Solution.

The course moved through Consequences of Improper Light Curing and an excellent website recommendation to get your lights evaluated for free!

Three different techniques were reviewed for placement and why each should be treated with a different set of rules.

Proximal contacts were discussed at length as well as simple powerful finishing.

For Anteriors he let the class in on the best kept secret in Esthetic Dentistry, as many of you know he is part of the educational team for the Post-Graduate Course in Esthetic Dentistry at the University of Minnesota School of Dentistry.

Q: What is Ultaire™ AKP?

A: Solvay: Ultaire™ AKP is an innovative high-performance polymer for RPDs. It’s a metal-free, comfortable, lightweight, and aesthetically pleasing alternative to metal frames. Ultaire™ AKP RPDs are biocompatible, non-corrosive, non-toxic and non-irritating with no metallic taste.

Q: Can you talk about some of the bene�ts for dentists?

A: Solvay: Sure! Let’s talk design. Because Ultaire™ AKP RPDs are designed within the CAD/CAM digital work�ow, they are custom �t to each patient. For dentists, this means they’ll see a secure �t with minimal �tting sessions and adjustments needed. In addition, dentists may see improved patient comfort and compliance, decreased risk of bone loss and higher patient satisfaction.

Q: What are the design speci�cations for Ultaire™ AKP?

A: D&S: We can speak to our experience at D&S. Since Ultaire™ AKP is slightly thicker than metal, we’ve found that the marginal ridge and rest preps need to be at least 1.5mm deep. Teeth will be plated in order to add teeth in the future. Solvay has developed a clasp design speci�cally for this material, which allows shorter and thicker clasps to be used. We do not use a vertical projection or embrasure-type clasps that cross the occlusal surface.

Q: Is this like Dura�ex or other �exible products on the market?

A: Solvay: No, Ultaire™ AKP isn’t a �exible. It’s a paradigm shifting polymer that is rigid enough to be a metal replacement.

Q: How strong is the material?

A: Solvay: Ultaire™ AKP has strength characteristics that surpass those of acrylics and acetals on the market today. Bone like in its properties; the material is strong enough to be tooth supported but �exible enough to engage undercuts more deeply. Ultaire™ AKP is speci�cally designed to be supportive and have a long lifetime, with bench strength well over ten years with normal insertion and removal.

AGD’s Annual Meeting of the House of Delegates (HOD) was held Nov. 3-5, 2017, in Chicago. The three-day event was the first time the HOD met separately from the annual scientific session, which now focuses exclusively on education, exhibits and networking.

During the meeting, delegates considered policies, procedures, resolutions and reports related to the business operations of AGD and organized dentistry.

The House adopted several public health policies, including a statement advocating flossing as an integral part of good oral hygiene; a statement on the consumption of sugar and its health consequences, including diabetes; a statement on the value of HPV vaccinations; and a statement on the role of dentistry in addressing the opioid crisis, finding that, while the profession does not contribute as heavily to the opioid epidemic as sources relying solely upon the number of prescriptions might suggest, it can play a key role as part of the solution.

Other policies regarding dental practice were adopted that:

• Support dentists prescribing or administering silver diamine fluoride to arrest caries and other legally marketed medical and dental products for off-label use, if in the best interest of their patient(s).

• Consistent with international expansion of AGD membership, no longer forewarn against treatment by dentists outside the U.S. or Canada, while still advocating for patient education regarding potential risks.

• Support federal and state legislative efforts to compel third party payers to provide notice on participation in leased networks and the identity of parties to which the networks are leased.

The 2018 budget was approved with an increase in dental student dues from $17 to $20.

House policy was also amended to include term limits on the Speaker of the House position to three terms of two years each and the Editor position to three terms of three years each.

AGD also installed its new leadership during the meeting, including Manuel A. Cordero, DDS, CPH, MAGD, as president; Neil J. Gajjar, DDS, MAGD, as president-elect; Connie L. White, DDS, FAGD, as vice president; and Michael W. Lew, DMD, MAGD, as secretary. Speaker of the House Bryan C. Edgar, DDS, MAGD, and Editor Roger D. Winland, DDS, MS, MAGD, were reelected to their positions.

The Fundamentals of Occlusion, TMJ and Myofacial Pain Dr. Hal Stewart- A biologic and systems-focused dentist, he believes that the focus of treatment should be to achieve and maintain a healthy and functioning chewing system. His focus is on the treatment neck and shoulder pain, headaches, clenching, and grinding, often caused by dental problems. He is one of fewer than 200 dentists worldwide to have completed the entire curriculum Orthognathic Bioesthetics International (OBI) Foundation. He lectures extensively, teaching dentists across the country and even around the world about advanced restorative and cosmetic dentistry. He has published numerous articles in professional journals and is a frequent lecturer on the topics of System Based and Cosmetic Dentistry, Dental Practice Management, and Building Patient-Doctor Relationships. He is a member of the Orthognathic Bioestheitics International Foundation, the American Academy of Cosmetic Dentistry, the American Dental Association, and the Academy of General Dentistry.

Dr. Craig Herre- In his Leawood, Kansas practice, Dr. Herre has provided comprehensive care with a focus on life enhancing, Biorejuvenation dentistry for over ten years. He has completed hundreds of hours of continuing education and has trained at the prestigious Pankey Institute, The Schuster Center for Professional Development, and Orthognathic Bioesthetics International. Dr. Herre has served as President of the Kansas Dental Association, as a delegate to the American Dental Association House and as Chair of the ADA Peer Review Committee. He is a Fellow of the American College of Dentists and holds memberships in the American Dental Association, American Academy of Cosmetic Dentistry, American Academy of Oral Medicine, and the American Academy of Sports Dentistry. Mr. Dan O’Rourke, CDT Over the past three decades Mr. O’Rourke has studied with many of the world leaders in dentistry. He has attended such institutions as The Dawson Center, The Schuster Center for Professional Development and the Foundation for Bioesthetic Dentistry. He co-founded and co directed Integritas, an interdisciplinary dental continuing education program and has published numerous articles in leading dental journals.
