1. Can you explain the purpose and history of the organisation?

2. How is the organisation managed? (Please explain the management structure you have in place. You must also provide a copy of your constitution and a copy of your last full year accounts)

3. Where does the organisation hold its bank account? (Please provide the sort code and bank account number - but not detailed records)

4. Are you applying for a grant of £2,500 or £5,000?

5. How would the grant be used by your organisation? (Provide a table showing breakdown of costs if possible, to clearly show use)

6. If you were successful, how would your activity benefit the community? In particular, how would it promote educational attainment or foster cross-community cohesion?

7. What is the age range of those who would benefit from the grant?

8. Are you able to estimate the number of young people that would benefit from the impact of this grant to support your activity?

9. What timescales, key dates or milestones do you propose for your activity and how do you propose to engage the Foundation with these?

10. How will your project reflect the diversity of the community you operate in?

11. What do you propose to do to gain some media coverage and stakeholder involvement from the grant and your activity?(Please provide proposed dates and ideas)
