If you are interested in auditioning for the JKO School, it is important that you consider the following details: The JKO School is a full year pre-professional program for dancers ages 12-18. Moore-Hill Dormitory is located on 21st St between San Jacinto Blvd and Speedway on The University of Texas at Austin campus The dormitoryrsquos physical address is 204 E 21st Street Austin TX 78705 To access Moore-Hill Dormitory from IH-35 travel west on MLK Jr Blvd turn right on San Jacinto Blvd and left on 21st St Moore-Hill Dormitory will be on your right

There are limited parking spaces available on 21st St Please read the posted signs carefully regarding possible parking restrictions Limited metered parking is available on the east side of Speedway Paid parking is also available in the Brazos Parking Garage (BRG) located on Brazos St near the intersection of MLK Jr Blvd and Brazos BRG is open 24 hours and parking rates are available online at wwwutexaseduparkingparkingvisitor

Check In

uml When you arrive enter Moore-Hill Dormitoryrsquos main doors off of 21st Street

uml Upon check in you will be furnished with a room key and a meal card IMPORTANT DO NOT LOSE YOUR CARD OR KEY The meal card will be used to purchase your meals at all Housing and Food Services located in Jester Center while you are attending the summer program Lost or unreturned room keys cost $7500 to replace These fees are not refundable even if the items are later found or returned after the Intensive There are no exceptions to this rule There will be no refunds for balances left on the meal card

uml After collecting your meal card and room key check in at the ABT registration table You will receive a name badge and any updates regarding procedures andor schedules

H F SOUSING AND OOD ERVICE

ORIENTATION MEETING

uml THE ORIENTATION MEETING IS ON SUNDAY JULY 2 AT 415 PM

The orientation meeting is mandatory for all students Information about location will be provided at check in Parents are welcome to attend Staff will be introduced at this meeting The orientation meeting will be followed by the studentsrsquo first dinner

Students live with a roommate and share a community bathroom in Moore-Hill Dormitory Although requests cannot be guaranteed students may indicate a preferred roommate on the attached registration form If no preference is indicated an effort will be made to match students of the same age Roommate assignments are subject to change and are at the discretion of the UT office staff

All dorm rooms have microwaves and small refrigerators The dormitory also provides internet accessibility to those bringing personal computers There are no public-use computers available to students

Students are advised to keep their room locked and to leave non-essential and valuable personal items at home Never leave personal items unattended The University of Texas cannot be responsible for studentsrsquo money or other personal items

Mentors will be assigned to oversee groups of students for the ABT Summer Intensive The mentors will live in the dorm escort students to the cafeteria classes rehearsals and recreational activities The mentors and supervisors will be available in case of emergencies

All students will be housed in Moore-Hill Dormitory Boys are not allowed on the girlsrsquo floors at any time nor will the girls be allowed on boysrsquo floors All resident mentors and supervisors will strictly enforce this policy

Mail Center Participants can send and receive letters and packages from July 2-July 28 In order to receive a package the studentrsquos name and the program name (ABT Summer Intensive) MUST be included Deliveries sent via FedEx UPS etc are delivered to students Monday through Friday only Moore-Hill Dormitory cannot accept and deliver any mail or packages on weekends

Dining Center Students who reside in Moore-Hill Dormitory purchase three meals a day in Jester Centerrsquos 2nd Floor Dining Area (J2) using their meal card The recently renovated cafeteria offers a contemporary take on traditional buffet dining Jester Center (201 E 21 St) is located directly across from Moore-Hill Dormitory

Day students are encouraged to purchase a meal card at utabtorg These meal cards will allow day students to purchase lunches during the Intensive

Jester City Market (Convenience Store) The convenience store sells everything from postcards greeting cards mailing supplies and school supplies to UT t-shirts hats and umbrellas personal toiletries laundry supplies pain relievers paper products snack foods soft drinks health drinks etc The meal card may not be used in Jester City Market Parents will be required to pay for costs incurred on their studentrsquos meal card for inappropriate purchases

Moore-Hill Dormitory does not provide telephones in the individual dorm rooms Parents may wish to send a cell phone with their child for the duration of the Intensive Students are encouraged to pre-arrange a time after 900 pm to call their parents During the day students will be busy with classes and activities

The Moore-Hill Dormitory desk is staffed 24 hours a day and serves as headquarters for housing information The University of Texas staff also handles mail and messages Non-emergency messages are posted on the message board in the lobby The phone number for the Moore-Hill Dormitory Front Desk is (512) 471-1462 To contact your child by mail from July 2-July 28 please use the following address

If a student must leave the program early due to a medical or family emergency the parent or legal guardian must contact Program Coordinator Jessica Champion She or a residential supervisor will sign the student out and obtain their room key and meal card

Students will not be permitted to leave campus for any reason other than an emergency a pre-approved doctor visit or pre-scheduled program activity unless we receive a written request (see Permission to Leave Campus form in DocuSign) Parents and guardians should include their names on the Permission to Leave Campus form Please note that your submission of Permission to Leave Campus form does not guarantee that the request will be approved

Those listed on the Permission to Leave Campus form are welcome to check out the student during scheduled non-class times and return them to the dormitory prior to lights out We ask that this is arranged in advance and that the student notify their mentor in advance so we may coordinate the checkout accordingly Parents may submit a written request to check out their child during non-class times by emailing utabtaustinutexasedu

C I LASS NFORMATION

Class Placement Placement classes will be Monday July 3 2017 Audition schedules will be provided to students on the evening of July 2 Please remember that placement reflects ABTrsquos training philosophy focusing on both traditional and classical methods Determinations are based on the sole judgment of the American Ballet Theatre artistic staff

Dress Code Policy PLEASE NOTE Any student not adhering to the dress code will be dismissed from class and may only return when properly attired Dancers may bring leg warmers t-shirts shorts and sweat pants for warm-up and between classes but may not wear those items in class

Female Dancers Hair must be up and neatly secured at all times during class

Ballet Technique Classes (technique pointe pas de deux variations) uml Monday-Thursday ndash Black leotard pink tights

Friday-Saturday (if applicable) ndash Any solid color leotard pink tights uml All technique class leotards must have two sleeves or straps no halter cut or off the shoulder and pink tights must be

footed (convertible tights are advised there will be limited time to change between classes) uml Coordinating solid colored skirts may not be worn during technique and are at the teacherrsquos discretion for pointe and

variations uml Pink ballet technique shoes with sewn elastics and pink pointe shoes with sewn elastics and ribbons uml Cut off tights and dangling jewelry are not permitted

Modern Jazz Pilates Yoga uml Any color leotard with pink or black tights (no shorts or knee length tights) uml Fitted ankle length jazz pants are also permitted (including flare or boot cut) uml Black jazz shoes

Character Class uml Ballet technique class leotard relevant to the day of the week uml Character skirt (17rdquo or below the knee is standard but you may bring any character skirt that you use in your home

studio) uml Black character shoes with strap (2rdquo heel is standard but bring what you have)

For Final Performance (You may be asked to wear any of the following for the final performance Please bring all that you already own from the list below)

uml At least five solid colored classic cut leotards (any shade blue green red pink purple etc) uml Coordinating solid or flower colored wrap skirts uml If you do not bring a colored leotard for the final performance please be prepared to purchase one during the program

Male Dancers

Ballet Technique Classes (technique pas de deux menrsquos variations menrsquos class) uml Monday ndash Thursday white fitted shirt with sleeves (such as Under Armour brand) Friday - Saturday any solid colored

fitted shirt (no large logos or text) and either black ankle-length tights white socks white ballet slippers with sewn elastics AND black footed tights black ballet slippers with sewn elastics

uml Dance belts uml Cut off tights and dangling jewelry are not permitted

Modern Jazz Pilates Yoga uml Black or white fitted shirt black or gray tights (no shorts or knee length tights) uml Fitted ankle length jazz pants are also permitted (including flare or boot cut) uml Black jazz shoes

Absolutely no chewing gum food or drinks other than water allowed inside the dance studios All cell phones are to be turned off and stored in bags during class

Recommended Reading We strongly advise the following reading for all ABT students and parents As standards change within the art form students are facing injury at a much earlier age The Healthy Dancer ABT Guidelines for Dancer Health is a valuable resource for dancers of all ages to maintain a healthy facility for dance The Healthy Dancer is compiled by Medical Professionals from the fields of sports and dance medicine nutrition physical therapy and orthopedics These health guidelines are an integral part of the ABT National Training Curriculum for the development of young dancers Guidelines are available at Amazoncom or httpwwwabtgiftshoporg for $1995

PHOTOGRAPHY

Saturday classes (10-1130 am and 1-230 pm) will include dance history yoga Pilates pointe shoe maintenance and expressive movement

ldquoNo Visitorsrdquo Policy for Classes and Rehearsals All classes and rehearsals are closed Parents and teachers are invited to attend an observation class on July 27 and 28 and the final performances on July 28

The American Ballet Theatre Summer Intensive will offer a variety of weekend activities including swimming bowling arts and crafts movie nights karaoke nights and a talent show

Photographers are often invited to ABTs Summer Training Programs for publicity purposes Your childs name and image may be used in promotional materials including brochures newspaper articles books andor television and for commercial purposes

Scheduling is tentative Scheduling and faculty changes are at the discretion of the American Ballet Theatre Summer Intensive

CLOSING SPECIAL EVENTS

Parent Observation Day We are scheduling two observation classes that provide parents the opportunity to observe a warm-up class taught by ABT faculty The following observation times are scheduled

uml Thursday July 27 2017 - 915 am to 1045 am uml Friday July 28 2017 - 915 am to 1015 am

Observation seats will need to be reserved beforehand (at no cost) Beginning the week of July 17 students will be given the opportunity to register their parentguardianteacher to attend an observation class

Final Performances

uml THE TWO FINAL PERFORMANCES ARE FRIDAY JULY 28 AT 1200 PM AND 230 PM

The performances will be held at the McCullough Theatre which is located in the Performing Arts Center (PAC) at 2375 Robert Dedman Dr Austin TX 78705 on the UT campus httpsmapsutexaseduutm All students are expected to be present for both performances

Performance Tickets uml ADMISSION IS $1500 PER PERFORMANCE FOR ADULTS AND $10 FOR CHILDREN AGES 5-18 THE THEATRE IS OPEN SEATING

Tickets go on sale Monday July 17 2017 Tickets may be pre-ordered online at utabtorg If you choose this method of purchase your tickets will be held at the McCullough Theatre Will Call and you can collect them on the day of the performance You will also be able to purchase tickets on the day of the performance by cash or check only The box office opens 90 minutes before the 1200 pm performance and 60 minutes before the 230 pm performance There are no refunds for performance tickets purchased in advance Those arriving on campus for the performance are advised to park in the Manor Garage The Manor Garage is located at 2017 Robert Dedman Dr Austin TX 78712 Information about parking for the event can be found at wwwutexaseduparkingmapsvisitorindexhtml Performance DVDs Videotaping and flash photography of the final performance are strictly prohibited However for your convenience Take One Productions will professionally record the performances DVDs may be pre-purchased or purchased on the day of the performance The ABT Summer Intensive will be sending information about purchasing DVDs closer to the Intensive

CHECK OUT AND DEPARTURE You may check out of the dorm following the final performances beginning at 400 pm on Friday July 28 or on Saturday July 29 by 1000 am Students may not be checked out prior to 400 pm on Friday July 28

Moore-Hill Dormitory will close at 1000 am on Saturday July 29 and all students must be checked out by this time Students are to leave the room as it was originally found on July 2 If any student leaves their room in disarray they will be required to pay a $5000 cleaning fee A supervisor will inspect each room upon checkout and the student must turn in their key to that supervisor If the key is not properly returned the student will be charged a lost key fee of $7500

The University of Texas will not be responsible for returning any items left by students

If the studentrsquos flight leaves after check out time on Saturday July 29 it is the parentsrsquo responsibility to arrange accommodations andor transportation off-campus Parents are responsible for arranging their childs transportation in advance (Please see the Travel form in DocuSign)

The last shuttle from the University of Texas to Austin-Bergstrom International Airport will be at 1000 am on July 29 2017 The University of Texas is not responsible for any accommodations andor airport transportation after this time

FOR CLASS uml Dance clothes ndash Please see Dress Code Policy on page 6 for details on expected dance attire

uml Dance shoes- It is the studentrsquos responsibility to have adequate shoes for the duration of the program We suggest that each dancer bring approximately one pair of pointe shoes per week of the program In addition dancers may wish to bring an additional pair of shoes for final rehearsals and performances Dancers are encouraged to take care of their shoes and use hardeners when necessary to lengthen the life of their pointe shoes

Students are expected to acquaint themselves with the rules procedures and standards of conduct established by the American Ballet Theatre Summer Intensive at The University of Texas at Austin A student who does not fulfill the responsibilities set out by such rules procedures and standards of conduct may be subject to disciplinary action including dismissal from the program

Students not fulfilling the rules procedures and standards of conduct as outlined below may be subject to the following disciplinary actions as per the discretion of the American Ballet Theatre Summer Intensive at The University of Texas at Austin Department of Theatre and Dance program coordinator and residential staff

uml Written Disciplinary Action form and a phone call to parentlegal guardian andor disciplinary meeting with program coordinator

uml Dismissal from program

Rules and Standards of Conduct

uml Students are required to participate in all scheduled classes activities and the final performance(s) The program coordinator must approve any exceptions

uml Students should immediately report any accident or illness to their mentor andor teacher

uml If a student must leave the program early due to a medical or family emergency the parent or legal guardian must contact the program coordinator to sign the student out

uml Students are required to wear their nametags in a clearly visible manner at all times

uml Students should eat all meals at Jester Center in their designated dining area

uml Students must be escorted by a mentor or residential staff at all times when walking between the dorm and campus buildings

uml Students must follow all applicable policies outlined in the forms in DocuSign including the Quiet Time and Day Student forms

uml Students must obey the strict ldquoLights Outrdquo rule at 1000 pm nightly and stay in the dorm room assigned to them

uml Students should carefully read the daily bulletin board It contains up-to-the minute information on rehearsals classes and special activities

uml Students may not drive or have cars on campus during Intensive Due to university parking restrictions UT does not offer parking permits

Examples of unacceptable conduct by students which are subject to disciplinary penalty including dismissal encompass but are not limited to

uml Possession or use of drugs alcohol fireworks firearms and tobacco

uml Visitation to rooms or halls of students of the opposite sex

uml Fraternization with students or adults on campus who are not associated with the ABT Summer Intensive

uml Unexcused tardiness or absences from attendance checks classes rehearsals performances or other scheduled activities

uml Inappropriate use of the meal card such as the purchase of merchandise or other Jester City Market products

uml Gambling participating in lotteries or any other games of chance on the premises at any time

uml Disruptive conduct

uml Destroying or defacing institutional property

uml Theft

uml Creating a condition hazardous to another person on the premises

uml Disorderly conduct horseplay harassment of other dancers or use of abusive language on the premises

uml Fighting encouraging a riot or threatening or attempting to cause injury to another person on the premises

uml Elevator misbehavior and

uml Violations of safety rules or accepted safety practices

Students who are dismissed will be sent home at the expense of the parent or legal guardian If a student is dismissed for reasons of unacceptable conduct there is no refund on Tuition or Room and Board Any student dismissed under these conditions will not be allowed to return to ABTUT in subsequent summers It is the responsibility of the program coor dinator to make decisions in this area

HEALTH ARE AND ERVICES C S All ABT Summer Intensive participants receive medical care at University Health Services (UHS) UHS is located on the UT campus in the Student Services Building at 100 W Dean Keeton Street Austin Texas 78712 The summer operating hours for the University Health Services are from 800 am - 530 pm Monday through Friday

Participants under the age of 18 in need of medical attention outside of UHSrsquos hours andor those with potentially serious or life-threatening conditions will be taken to Dell Childrenrsquos Medical Center of Central Texas The Dell Childrenrsquos Medical Center is located at 4900 Mueller Boulevard Austin Texas 78723

Participants who are age 18 at the time of the Intensive and in need of medical attention outside of UHSrsquos hours andor those with potentially serious or life-threatening conditions will be taken to St Davidrsquos Medical Center The St Davidrsquos Medical Center is located at 919 E 32nd Street Austin Texas 78705

UHS is a full-service ambulatory healthcare clinic with on-site laboratory radiology and pharmacy facilities All ABT Summer Intensive participants are seen in the UHS Urgent Care Clinic A physician and supporting clinical staff provide treatment in this area If any student is seriously ill or injured it is the policy of UHS that the doctor on the case will contact the parents Individuals with potentially serious or life-threatening conditions will be taken directly to a hospital emergency room

Students who require medical attention will be accompanied to the University Health Services by the residential coordinator supervisor or mentor All medical costs incurred for treatment will be the responsibility of the parents or guardian of the individual treated

Some health maintenance services are available for program participants with special needs such as frequent allergy injections as long as they can be administered at University Health Services during the regular hours of operation Participants or parents of minors should contact the residential coordinator by phoning 512-232-7099 to make arrangements with University Health Services for these services

Consent for treatment of a minor is required by law in order for University Health Services to render any medical treatment except in life-threatening situations thus no student will be registered for the ABT Summer Intensive without a Consent for Treatment form (Please see form in DocuSign) The consent form requires four signatures by the parentlegal guardian ndash one giving permission to release health history information one releasing liability in regards to administering medication one giving permission for the minor to receive treatment and the other acknowledging that they have received a copy of the UHS ldquoNotice of Privacy Practicesrdquo

University Health Services Charges

All summer program participants who go to UHS for medical care are assessed a $6000 office visit charge There are additional charges for many of the procedures clinicians may need to perform in order to diagnose and treat illnesses or injuries Samples of services with additional charges include

uml Lab tests X-rays uml Casts splints crutches and dressings uml Injections and IVs uml Nebulizer or other asthma treatments uml Medications and pharmacy supplies

The participant will receive a bill from UHS immediately upon being treated The participant or parentlegal guardian must pay this invoice by the end of the next business day following the participantrsquos treatment UHS accepts payment in form of cash and MasterCard at this time

UHS does not accept outside insurance for medical services The UHS staff can provide information that may be needed for patients to file claims with their own insurance companies if their insurance plans allow them to do so

Please be sure to provide a copy of your childrsquos medicalprescription card This will be required at check in on July 2 In the event your child needs medical treatment from St Davidrsquos Medical Center Dell Childrenrsquos Medical Center or a specialist we will present this insurance card on your behalf

Physical Therapy Services

Students will have access to a physical therapist after being evaluated by a medical provider at University Health Services The physical therapist will be on site Tuesdays and Thursdays from 530 pm to 730 pm for the assessment and treatment of musculoskeletal dysfunction and or pain and for the assessment and treatment of functional loss Students will have to reserve a 15-minute timeslot by signing up in the morning of on Tuesday or Thursday (log is located at the ABT daily commuters desk at Moore-Hill Dormitory available until 830 am)

When University Health Services recommends therapeutic treatment for an injury the program coordinator will contact the parentlegal guardian of the student to inform himher and to ask for a signed ldquoConsent for Therapeutic Proceduresrdquo form This way we ensure a real-time information flow between the program the student and the parentlegal guardian The program coordinator will provide this form on an as needed basis These services are provided to the students at no cost

No student will be seen by the physical therapist if the Consent for Therapeutic Procedures form is not received by the program coordinator before 500 pm on the day of treatment

uml Upon check in you will be furnished with a room key and a meal card IMPORTANT DO NOT LOSE YOUR CARD OR KEY The meal card will be used to purchase your meals at all Housing and Food Services located in Jester Center while you are attending the summer program Lost or unreturned room keys cost $7500 to replace These fees are not refundable even if the items are later found or returned after the Intensive There are no exceptions to this rule There will be no refunds for balances left on the meal card

ORIENTATION MEETING

uml THE ORIENTATION MEETING IS ON SUNDAY JULY 2 AT 415 PM

The orientation meeting is mandatory for all students Information about location will be provided at check in Parents are welcome to attend Staff will be introduced at this meeting The orientation meeting will be followed by the studentsrsquo first dinner

Students live with a roommate and share a community bathroom in Moore-Hill Dormitory Although requests cannot be guaranteed students may indicate a preferred roommate on the attached registration form If no preference is indicated an effort will be made to match students of the same age Roommate assignments are subject to change and are at the discretion of the UT office staff

All dorm rooms have microwaves and small refrigerators The dormitory also provides internet accessibility to those bringing personal computers There are no public-use computers available to students

Students are advised to keep their room locked and to leave non-essential and valuable personal items at home Never leave personal items unattended The University of Texas cannot be responsible for studentsrsquo money or other personal items

Mentors will be assigned to oversee groups of students for the ABT Summer Intensive The mentors will live in the dorm escort students to the cafeteria classes rehearsals and recreational activities The mentors and supervisors will be available in case of emergencies

All students will be housed in Moore-Hill Dormitory Boys are not allowed on the girlsrsquo floors at any time nor will the girls be allowed on boysrsquo floors All resident mentors and supervisors will strictly enforce this policy

Mail Center Participants can send and receive letters and packages from July 2-July 28 In order to receive a package the studentrsquos name and the program name (ABT Summer Intensive) MUST be included Deliveries sent via FedEx UPS etc are delivered to students Monday through Friday only Moore-Hill Dormitory cannot accept and deliver any mail or packages on weekends

Dining Center Students who reside in Moore-Hill Dormitory purchase three meals a day in Jester Centerrsquos 2nd Floor Dining Area (J2) using their meal card The recently renovated cafeteria offers a contemporary take on traditional buffet dining Jester Center (201 E 21 St) is located directly across from Moore-Hill Dormitory

Day students are encouraged to purchase a meal card at utabtorg These meal cards will allow day students to purchase lunches during the Intensive

Jester City Market (Convenience Store) The convenience store sells everything from postcards greeting cards mailing supplies and school supplies to UT t-shirts hats and umbrellas personal toiletries laundry supplies pain relievers paper products snack foods soft drinks health drinks etc The meal card may not be used in Jester City Market Parents will be required to pay for costs incurred on their studentrsquos meal card for inappropriate purchases

Class Placement Placement classes will be Monday July 3 2017 Audition schedules will be provided to students on the evening of July 2 Please remember that placement reflects ABTrsquos training philosophy focusing on both traditional and classical methods Determinations are based on the sole judgment of the American Ballet Theatre artistic staff

Dress Code Policy PLEASE NOTE Any student not adhering to the dress code will be dismissed from class and may only return when properly attired Dancers may bring leg warmers t-shirts shorts and sweat pants for warm-up and between classes but may not wear those items in class

Female Dancers Hair must be up and neatly secured at all times during class

Photographers are often invited to ABTs Summer Training Programs for publicity purposes Your childs name and image may be used in promotional materials including brochures newspaper articles books andor television and for commercial purposes

Scheduling is tentative Scheduling and faculty changes are at the discretion of the American Ballet Theatre Summer Intensive

CLOSING SPECIAL EVENTS

Parent Observation Day We are scheduling two observation classes that provide parents the opportunity to observe a warm-up class taught by ABT faculty The following observation times are scheduled

uml Thursday July 27 2017 - 915 am to 1045 am uml Friday July 28 2017 - 915 am to 1015 am

Observation seats will need to be reserved beforehand (at no cost) Beginning the week of July 17 students will be given the opportunity to register their parentguardianteacher to attend an observation class

Tickets go on sale Monday July 17 2017 Tickets may be pre-ordered online at utabtorg If you choose this method of purchase your tickets will be held at the McCullough Theatre Will Call and you can collect them on the day of the performance You will also be able to purchase tickets on the day of the performance by cash or check only The box office opens 90 minutes before the 1200 pm performance and 60 minutes before the 230 pm performance There are no refunds for performance tickets purchased in advance Those arriving on campus for the performance are advised to park in the Manor Garage The Manor Garage is located at 2017 Robert Dedman Dr Austin TX 78712 Information about parking for the event can be found at wwwutexaseduparkingmapsvisitorindexhtml Performance DVDs Videotaping and flash photography of the final performance are strictly prohibited However for your convenience Take One Productions will professionally record the performances DVDs may be pre-purchased or purchased on the day of the performance The ABT Summer Intensive will be sending information about purchasing DVDs closer to the Intensive

CHECK OUT AND DEPARTURE You may check out of the dorm following the final performances beginning at 400 pm on Friday July 28 or on Saturday July 29 by 1000 am Students may not be checked out prior to 400 pm on Friday July 28

Moore-Hill Dormitory will close at 1000 am on Saturday July 29 and all students must be checked out by this time Students are to leave the room as it was originally found on July 2 If any student leaves their room in disarray they will be required to pay a $5000 cleaning fee A supervisor will inspect each room upon checkout and the student must turn in their key to that supervisor If the key is not properly returned the student will be charged a lost key fee of $7500

The University of Texas will not be responsible for returning any items left by students

If the studentrsquos flight leaves after check out time on Saturday July 29 it is the parentsrsquo responsibility to arrange accommodations andor transportation off-campus Parents are responsible for arranging their childs transportation in advance (Please see the Travel form in DocuSign)

The last shuttle from the University of Texas to Austin-Bergstrom International Airport will be at 1000 am on July 29 2017 The University of Texas is not responsible for any accommodations andor airport transportation after this time

Students are expected to acquaint themselves with the rules procedures and standards of conduct established by the American Ballet Theatre Summer Intensive at The University of Texas at Austin A student who does not fulfill the responsibilities set out by such rules procedures and standards of conduct may be subject to disciplinary action including dismissal from the program

Students not fulfilling the rules procedures and standards of conduct as outlined below may be subject to the following disciplinary actions as per the discretion of the American Ballet Theatre Summer Intensive at The University of Texas at Austin Department of Theatre and Dance program coordinator and residential staff

uml Written Disciplinary Action form and a phone call to parentlegal guardian andor disciplinary meeting with program coordinator

uml Dismissal from program

Rules and Standards of Conduct

uml Students are required to participate in all scheduled classes activities and the final performance(s) The program coordinator must approve any exceptions

uml Students should immediately report any accident or illness to their mentor andor teacher

uml If a student must leave the program early due to a medical or family emergency the parent or legal guardian must contact the program coordinator to sign the student out

uml Students are required to wear their nametags in a clearly visible manner at all times

uml Students should eat all meals at Jester Center in their designated dining area

uml Students must be escorted by a mentor or residential staff at all times when walking between the dorm and campus buildings

uml Students must follow all applicable policies outlined in the forms in DocuSign including the Quiet Time and Day Student forms

uml Students must obey the strict ldquoLights Outrdquo rule at 1000 pm nightly and stay in the dorm room assigned to them

uml Disruptive conduct

uml Destroying or defacing institutional property

uml Theft

uml Creating a condition hazardous to another person on the premises

uml Disorderly conduct horseplay harassment of other dancers or use of abusive language on the premises

uml Fighting encouraging a riot or threatening or attempting to cause injury to another person on the premises

uml Elevator misbehavior and

uml Violations of safety rules or accepted safety practices

Students who are dismissed will be sent home at the expense of the parent or legal guardian If a student is dismissed for reasons of unacceptable conduct there is no refund on Tuition or Room and Board Any student dismissed under these conditions will not be allowed to return to ABTUT in subsequent summers It is the responsibility of the program coor dinator to make decisions in this area

HEALTH ARE AND ERVICES C S All ABT Summer Intensive participants receive medical care at University Health Services (UHS) UHS is located on the UT campus in the Student Services Building at 100 W Dean Keeton Street Austin Texas 78712 The summer operating hours for the University Health Services are from 800 am - 530 pm Monday through Friday

Participants under the age of 18 in need of medical attention outside of UHSrsquos hours andor those with potentially serious or life-threatening conditions will be taken to Dell Childrenrsquos Medical Center of Central Texas The Dell Childrenrsquos Medical Center is located at 4900 Mueller Boulevard Austin Texas 78723

Participants who are age 18 at the time of the Intensive and in need of medical attention outside of UHSrsquos hours andor those with potentially serious or life-threatening conditions will be taken to St Davidrsquos Medical Center The St Davidrsquos Medical Center is located at 919 E 32nd Street Austin Texas 78705

UHS is a full-service ambulatory healthcare clinic with on-site laboratory radiology and pharmacy facilities All ABT Summer Intensive participants are seen in the UHS Urgent Care Clinic A physician and supporting clinical staff provide treatment in this area If any student is seriously ill or injured it is the policy of UHS that the doctor on the case will contact the parents Individuals with potentially serious or life-threatening conditions will be taken directly to a hospital emergency room

Students who require medical attention will be accompanied to the University Health Services by the residential coordinator supervisor or mentor All medical costs incurred for treatment will be the responsibility of the parents or guardian of the individual treated

Some health maintenance services are available for program participants with special needs such as frequent allergy injections as long as they can be administered at University Health Services during the regular hours of operation Participants or parents of minors should contact the residential coordinator by phoning 512-232-7099 to make arrangements with University Health Services for these services

Consent for treatment of a minor is required by law in order for University Health Services to render any medical treatment except in life-threatening situations thus no student will be registered for the ABT Summer Intensive without a Consent for Treatment form (Please see form in DocuSign) The consent form requires four signatures by the parentlegal guardian ndash one giving permission to release health history information one releasing liability in regards to administering medication one giving permission for the minor to receive treatment and the other acknowledging that they have received a copy of the UHS ldquoNotice of Privacy Practicesrdquo

University Health Services Charges

All summer program participants who go to UHS for medical care are assessed a $6000 office visit charge There are additional charges for many of the procedures clinicians may need to perform in order to diagnose and treat illnesses or injuries Samples of services with additional charges include

uml Lab tests X-rays uml Casts splints crutches and dressings uml Injections and IVs uml Nebulizer or other asthma treatments uml Medications and pharmacy supplies

The participant will receive a bill from UHS immediately upon being treated The participant or parentlegal guardian must pay this invoice by the end of the next business day following the participantrsquos treatment UHS accepts payment in form of cash and MasterCard at this time

UHS does not accept outside insurance for medical services The UHS staff can provide information that may be needed for patients to file claims with their own insurance companies if their insurance plans allow them to do so

Please be sure to provide a copy of your childrsquos medicalprescription card This will be required at check in on July 2 In the event your child needs medical treatment from St Davidrsquos Medical Center Dell Childrenrsquos Medical Center or a specialist we will present this insurance card on your behalf

Physical Therapy Services

Students will have access to a physical therapist after being evaluated by a medical provider at University Health Services The physical therapist will be on site Tuesdays and Thursdays from 530 pm to 730 pm for the assessment and treatment of musculoskeletal dysfunction and or pain and for the assessment and treatment of functional loss Students will have to reserve a 15-minute timeslot by signing up in the morning of on Tuesday or Thursday (log is located at the ABT daily commuters desk at Moore-Hill Dormitory available until 830 am)

When University Health Services recommends therapeutic treatment for an injury the program coordinator will contact the parentlegal guardian of the student to inform himher and to ask for a signed ldquoConsent for Therapeutic Proceduresrdquo form This way we ensure a real-time information flow between the program the student and the parentlegal guardian The program coordinator will provide this form on an as needed basis These services are provided to the students at no cost

No student will be seen by the physical therapist if the Consent for Therapeutic Procedures form is not received by the program coordinator before 500 pm on the day of treatment

ORIENTATION MEETING

uml THE ORIENTATION MEETING IS ON SUNDAY JULY 2 AT 415 PM

The orientation meeting is mandatory for all students Information about location will be provided at check in Parents are welcome to attend Staff will be introduced at this meeting The orientation meeting will be followed by the studentsrsquo first dinner

Students live with a roommate and share a community bathroom in Moore-Hill Dormitory Although requests cannot be guaranteed students may indicate a preferred roommate on the attached registration form If no preference is indicated an effort will be made to match students of the same age Roommate assignments are subject to change and are at the discretion of the UT office staff

All dorm rooms have microwaves and small refrigerators The dormitory also provides internet accessibility to those bringing personal computers There are no public-use computers available to students

Students are advised to keep their room locked and to leave non-essential and valuable personal items at home Never leave personal items unattended The University of Texas cannot be responsible for studentsrsquo money or other personal items

Mentors will be assigned to oversee groups of students for the ABT Summer Intensive The mentors will live in the dorm escort students to the cafeteria classes rehearsals and recreational activities The mentors and supervisors will be available in case of emergencies

All students will be housed in Moore-Hill Dormitory Boys are not allowed on the girlsrsquo floors at any time nor will the girls be allowed on boysrsquo floors All resident mentors and supervisors will strictly enforce this policy

Mail Center Participants can send and receive letters and packages from July 2-July 28 In order to receive a package the studentrsquos name and the program name (ABT Summer Intensive) MUST be included Deliveries sent via FedEx UPS etc are delivered to students Monday through Friday only Moore-Hill Dormitory cannot accept and deliver any mail or packages on weekends

Dining Center Students who reside in Moore-Hill Dormitory purchase three meals a day in Jester Centerrsquos 2nd Floor Dining Area (J2) using their meal card The recently renovated cafeteria offers a contemporary take on traditional buffet dining Jester Center (201 E 21 St) is located directly across from Moore-Hill Dormitory

Day students are encouraged to purchase a meal card at utabtorg These meal cards will allow day students to purchase lunches during the Intensive

Jester City Market (Convenience Store) The convenience store sells everything from postcards greeting cards mailing supplies and school supplies to UT t-shirts hats and umbrellas personal toiletries laundry supplies pain relievers paper products snack foods soft drinks health drinks etc The meal card may not be used in Jester City Market Parents will be required to pay for costs incurred on their studentrsquos meal card for inappropriate purchases

Photographers are often invited to ABTs Summer Training Programs for publicity purposes Your childs name and image may be used in promotional materials including brochures newspaper articles books andor television and for commercial purposes

Scheduling is tentative Scheduling and faculty changes are at the discretion of the American Ballet Theatre Summer Intensive

CLOSING SPECIAL EVENTS

Parent Observation Day We are scheduling two observation classes that provide parents the opportunity to observe a warm-up class taught by ABT faculty The following observation times are scheduled

uml Thursday July 27 2017 - 915 am to 1045 am uml Friday July 28 2017 - 915 am to 1015 am

Observation seats will need to be reserved beforehand (at no cost) Beginning the week of July 17 students will be given the opportunity to register their parentguardianteacher to attend an observation class

Performance Tickets uml ADMISSION IS $1500 PER PERFORMANCE FOR ADULTS AND $10 FOR CHILDREN AGES 5-18 THE THEATRE IS OPEN SEATING

Tickets go on sale Monday July 17 2017 Tickets may be pre-ordered online at utabtorg If you choose this method of purchase your tickets will be held at the McCullough Theatre Will Call and you can collect them on the day of the performance You will also be able to purchase tickets on the day of the performance by cash or check only The box office opens 90 minutes before the 1200 pm performance and 60 minutes before the 230 pm performance There are no refunds for performance tickets purchased in advance Those arriving on campus for the performance are advised to park in the Manor Garage The Manor Garage is located at 2017 Robert Dedman Dr Austin TX 78712 Information about parking for the event can be found at wwwutexaseduparkingmapsvisitorindexhtml Performance DVDs Videotaping and flash photography of the final performance are strictly prohibited However for your convenience Take One Productions will professionally record the performances DVDs may be pre-purchased or purchased on the day of the performance The ABT Summer Intensive will be sending information about purchasing DVDs closer to the Intensive

CHECK OUT AND DEPARTURE You may check out of the dorm following the final performances beginning at 400 pm on Friday July 28 or on Saturday July 29 by 1000 am Students may not be checked out prior to 400 pm on Friday July 28

Moore-Hill Dormitory will close at 1000 am on Saturday July 29 and all students must be checked out by this time Students are to leave the room as it was originally found on July 2 If any student leaves their room in disarray they will be required to pay a $5000 cleaning fee A supervisor will inspect each room upon checkout and the student must turn in their key to that supervisor If the key is not properly returned the student will be charged a lost key fee of $7500

The University of Texas will not be responsible for returning any items left by students

If the studentrsquos flight leaves after check out time on Saturday July 29 it is the parentsrsquo responsibility to arrange accommodations andor transportation off-campus Parents are responsible for arranging their childs transportation in advance (Please see the Travel form in DocuSign)

The last shuttle from the University of Texas to Austin-Bergstrom International Airport will be at 1000 am on July 29 2017 The University of Texas is not responsible for any accommodations andor airport transportation after this time

CONDUCT OLICY P

Students are expected to acquaint themselves with the rules procedures and standards of conduct established by the American Ballet Theatre Summer Intensive at The University of Texas at Austin A student who does not fulfill the responsibilities set out by such rules procedures and standards of conduct may be subject to disciplinary action including dismissal from the program

Students not fulfilling the rules procedures and standards of conduct as outlined below may be subject to the following disciplinary actions as per the discretion of the American Ballet Theatre Summer Intensive at The University of Texas at Austin Department of Theatre and Dance program coordinator and residential staff

uml Written Disciplinary Action form and a phone call to parentlegal guardian andor disciplinary meeting with program coordinator

uml Dismissal from program

Rules and Standards of Conduct

uml Students are required to participate in all scheduled classes activities and the final performance(s) The program coordinator must approve any exceptions

uml Students should immediately report any accident or illness to their mentor andor teacher

uml If a student must leave the program early due to a medical or family emergency the parent or legal guardian must contact the program coordinator to sign the student out

uml Students are required to wear their nametags in a clearly visible manner at all times

uml Students should eat all meals at Jester Center in their designated dining area

uml Students must be escorted by a mentor or residential staff at all times when walking between the dorm and campus buildings

uml Students must follow all applicable policies outlined in the forms in DocuSign including the Quiet Time and Day Student forms

uml Students must obey the strict ldquoLights Outrdquo rule at 1000 pm nightly and stay in the dorm room assigned to them

uml Students should carefully read the daily bulletin board It contains up-to-the minute information on rehearsals classes and special activities

uml Students may not drive or have cars on campus during Intensive Due to university parking restrictions UT does not offer parking permits

uml Possession or use of drugs alcohol fireworks firearms and tobacco

uml Visitation to rooms or halls of students of the opposite sex

uml Fraternization with students or adults on campus who are not associated with the ABT Summer Intensive

uml Unexcused tardiness or absences from attendance checks classes rehearsals performances or other scheduled activities

uml Inappropriate use of the meal card such as the purchase of merchandise or other Jester City Market products

uml Gambling participating in lotteries or any other games of chance on the premises at any time

uml Disruptive conduct

uml Destroying or defacing institutional property

uml Theft

uml Creating a condition hazardous to another person on the premises

uml Disorderly conduct horseplay harassment of other dancers or use of abusive language on the premises

uml Fighting encouraging a riot or threatening or attempting to cause injury to another person on the premises

uml Elevator misbehavior and

uml Violations of safety rules or accepted safety practices

Students who are dismissed will be sent home at the expense of the parent or legal guardian If a student is dismissed for reasons of unacceptable conduct there is no refund on Tuition or Room and Board Any student dismissed under these conditions will not be allowed to return to ABTUT in subsequent summers It is the responsibility of the program coor dinator to make decisions in this area

HEALTH ARE AND ERVICES C S All ABT Summer Intensive participants receive medical care at University Health Services (UHS) UHS is located on the UT campus in the Student Services Building at 100 W Dean Keeton Street Austin Texas 78712 The summer operating hours for the University Health Services are from 800 am - 530 pm Monday through Friday

Participants under the age of 18 in need of medical attention outside of UHSrsquos hours andor those with potentially serious or life-threatening conditions will be taken to Dell Childrenrsquos Medical Center of Central Texas The Dell Childrenrsquos Medical Center is located at 4900 Mueller Boulevard Austin Texas 78723

Participants who are age 18 at the time of the Intensive and in need of medical attention outside of UHSrsquos hours andor those with potentially serious or life-threatening conditions will be taken to St Davidrsquos Medical Center The St Davidrsquos Medical Center is located at 919 E 32nd Street Austin Texas 78705

UHS is a full-service ambulatory healthcare clinic with on-site laboratory radiology and pharmacy facilities All ABT Summer Intensive participants are seen in the UHS Urgent Care Clinic A physician and supporting clinical staff provide treatment in this area If any student is seriously ill or injured it is the policy of UHS that the doctor on the case will contact the parents Individuals with potentially serious or life-threatening conditions will be taken directly to a hospital emergency room

Students who require medical attention will be accompanied to the University Health Services by the residential coordinator supervisor or mentor All medical costs incurred for treatment will be the responsibility of the parents or guardian of the individual treated

Some health maintenance services are available for program participants with special needs such as frequent allergy injections as long as they can be administered at University Health Services during the regular hours of operation Participants or parents of minors should contact the residential coordinator by phoning 512-232-7099 to make arrangements with University Health Services for these services

Consent for treatment of a minor is required by law in order for University Health Services to render any medical treatment except in life-threatening situations thus no student will be registered for the ABT Summer Intensive without a Consent for Treatment form (Please see form in DocuSign) The consent form requires four signatures by the parentlegal guardian ndash one giving permission to release health history information one releasing liability in regards to administering medication one giving permission for the minor to receive treatment and the other acknowledging that they have received a copy of the UHS ldquoNotice of Privacy Practicesrdquo

University Health Services Charges

All summer program participants who go to UHS for medical care are assessed a $6000 office visit charge There are additional charges for many of the procedures clinicians may need to perform in order to diagnose and treat illnesses or injuries Samples of services with additional charges include

uml Lab tests X-rays uml Casts splints crutches and dressings uml Injections and IVs uml Nebulizer or other asthma treatments uml Medications and pharmacy supplies

The participant will receive a bill from UHS immediately upon being treated The participant or parentlegal guardian must pay this invoice by the end of the next business day following the participantrsquos treatment UHS accepts payment in form of cash and MasterCard at this time

UHS does not accept outside insurance for medical services The UHS staff can provide information that may be needed for patients to file claims with their own insurance companies if their insurance plans allow them to do so

Please be sure to provide a copy of your childrsquos medicalprescription card This will be required at check in on July 2 In the event your child needs medical treatment from St Davidrsquos Medical Center Dell Childrenrsquos Medical Center or a specialist we will present this insurance card on your behalf

Physical Therapy Services

Students will have access to a physical therapist after being evaluated by a medical provider at University Health Services The physical therapist will be on site Tuesdays and Thursdays from 530 pm to 730 pm for the assessment and treatment of musculoskeletal dysfunction and or pain and for the assessment and treatment of functional loss Students will have to reserve a 15-minute timeslot by signing up in the morning of on Tuesday or Thursday (log is located at the ABT daily commuters desk at Moore-Hill Dormitory available until 830 am)

When University Health Services recommends therapeutic treatment for an injury the program coordinator will contact the parentlegal guardian of the student to inform himher and to ask for a signed ldquoConsent for Therapeutic Proceduresrdquo form This way we ensure a real-time information flow between the program the student and the parentlegal guardian The program coordinator will provide this form on an as needed basis These services are provided to the students at no cost

No student will be seen by the physical therapist if the Consent for Therapeutic Procedures form is not received by the program coordinator before 500 pm on the day of treatment

ORIENTATION MEETING

uml THE ORIENTATION MEETING IS ON SUNDAY JULY 2 AT 415 PM

The orientation meeting is mandatory for all students Information about location will be provided at check in Parents are welcome to attend Staff will be introduced at this meeting The orientation meeting will be followed by the studentsrsquo first dinner

Students live with a roommate and share a community bathroom in Moore-Hill Dormitory Although requests cannot be guaranteed students may indicate a preferred roommate on the attached registration form If no preference is indicated an effort will be made to match students of the same age Roommate assignments are subject to change and are at the discretion of the UT office staff

All dorm rooms have microwaves and small refrigerators The dormitory also provides internet accessibility to those bringing personal computers There are no public-use computers available to students

Students are advised to keep their room locked and to leave non-essential and valuable personal items at home Never leave personal items unattended The University of Texas cannot be responsible for studentsrsquo money or other personal items

Mentors will be assigned to oversee groups of students for the ABT Summer Intensive The mentors will live in the dorm escort students to the cafeteria classes rehearsals and recreational activities The mentors and supervisors will be available in case of emergencies

All students will be housed in Moore-Hill Dormitory Boys are not allowed on the girlsrsquo floors at any time nor will the girls be allowed on boysrsquo floors All resident mentors and supervisors will strictly enforce this policy

Mail Center Participants can send and receive letters and packages from July 2-July 28 In order to receive a package the studentrsquos name and the program name (ABT Summer Intensive) MUST be included Deliveries sent via FedEx UPS etc are delivered to students Monday through Friday only Moore-Hill Dormitory cannot accept and deliver any mail or packages on weekends

Dining Center Students who reside in Moore-Hill Dormitory purchase three meals a day in Jester Centerrsquos 2nd Floor Dining Area (J2) using their meal card The recently renovated cafeteria offers a contemporary take on traditional buffet dining Jester Center (201 E 21 St) is located directly across from Moore-Hill Dormitory

Day students are encouraged to purchase a meal card at utabtorg These meal cards will allow day students to purchase lunches during the Intensive

Jester City Market (Convenience Store) The convenience store sells everything from postcards greeting cards mailing supplies and school supplies to UT t-shirts hats and umbrellas personal toiletries laundry supplies pain relievers paper products snack foods soft drinks health drinks etc The meal card may not be used in Jester City Market Parents will be required to pay for costs incurred on their studentrsquos meal card for inappropriate purchases

Class Placement Placement classes will be Monday July 3 2017 Audition schedules will be provided to students on the evening of July 2 Please remember that placement reflects ABTrsquos training philosophy focusing on both traditional and classical methods Determinations are based on the sole judgment of the American Ballet Theatre artistic staff

Dress Code Policy PLEASE NOTE Any student not adhering to the dress code will be dismissed from class and may only return when properly attired Dancers may bring leg warmers t-shirts shorts and sweat pants for warm-up and between classes but may not wear those items in class

Female Dancers Hair must be up and neatly secured at all times during class

Photographers are often invited to ABTs Summer Training Programs for publicity purposes Your childs name and image may be used in promotional materials including brochures newspaper articles books andor television and for commercial purposes

Scheduling is tentative Scheduling and faculty changes are at the discretion of the American Ballet Theatre Summer Intensive

Parent Observation Day We are scheduling two observation classes that provide parents the opportunity to observe a warm-up class taught by ABT faculty The following observation times are scheduled

Students not fulfilling the rules procedures and standards of conduct as outlined below may be subject to the following disciplinary actions as per the discretion of the American Ballet Theatre Summer Intensive at The University of Texas at Austin Department of Theatre and Dance program coordinator and residential staff

Students who are dismissed will be sent home at the expense of the parent or legal guardian If a student is dismissed for reasons of unacceptable conduct there is no refund on Tuition or Room and Board Any student dismissed under these conditions will not be allowed to return to ABTUT in subsequent summers It is the responsibility of the program coor dinator to make decisions in this area

HEALTH ARE AND ERVICES C S All ABT Summer Intensive participants receive medical care at University Health Services (UHS) UHS is located on the UT campus in the Student Services Building at 100 W Dean Keeton Street Austin Texas 78712 The summer operating hours for the University Health Services are from 800 am - 530 pm Monday through Friday

Participants under the age of 18 in need of medical attention outside of UHSrsquos hours andor those with potentially serious or life-threatening conditions will be taken to Dell Childrenrsquos Medical Center of Central Texas The Dell Childrenrsquos Medical Center is located at 4900 Mueller Boulevard Austin Texas 78723

Participants who are age 18 at the time of the Intensive and in need of medical attention outside of UHSrsquos hours andor those with potentially serious or life-threatening conditions will be taken to St Davidrsquos Medical Center The St Davidrsquos Medical Center is located at 919 E 32nd Street Austin Texas 78705

UHS is a full-service ambulatory healthcare clinic with on-site laboratory radiology and pharmacy facilities All ABT Summer Intensive participants are seen in the UHS Urgent Care Clinic A physician and supporting clinical staff provide treatment in this area If any student is seriously ill or injured it is the policy of UHS that the doctor on the case will contact the parents Individuals with potentially serious or life-threatening conditions will be taken directly to a hospital emergency room

Students who require medical attention will be accompanied to the University Health Services by the residential coordinator supervisor or mentor All medical costs incurred for treatment will be the responsibility of the parents or guardian of the individual treated

Some health maintenance services are available for program participants with special needs such as frequent allergy injections as long as they can be administered at University Health Services during the regular hours of operation Participants or parents of minors should contact the residential coordinator by phoning 512-232-7099 to make arrangements with University Health Services for these services

Consent for treatment of a minor is required by law in order for University Health Services to render any medical treatment except in life-threatening situations thus no student will be registered for the ABT Summer Intensive without a Consent for Treatment form (Please see form in DocuSign) The consent form requires four signatures by the parentlegal guardian ndash one giving permission to release health history information one releasing liability in regards to administering medication one giving permission for the minor to receive treatment and the other acknowledging that they have received a copy of the UHS ldquoNotice of Privacy Practicesrdquo

University Health Services Charges

All summer program participants who go to UHS for medical care are assessed a $6000 office visit charge There are additional charges for many of the procedures clinicians may need to perform in order to diagnose and treat illnesses or injuries Samples of services with additional charges include

uml Lab tests X-rays uml Casts splints crutches and dressings uml Injections and IVs uml Nebulizer or other asthma treatments uml Medications and pharmacy supplies

The participant will receive a bill from UHS immediately upon being treated The participant or parentlegal guardian must pay this invoice by the end of the next business day following the participantrsquos treatment UHS accepts payment in form of cash and MasterCard at this time

UHS does not accept outside insurance for medical services The UHS staff can provide information that may be needed for patients to file claims with their own insurance companies if their insurance plans allow them to do so

Please be sure to provide a copy of your childrsquos medicalprescription card This will be required at check in on July 2 In the event your child needs medical treatment from St Davidrsquos Medical Center Dell Childrenrsquos Medical Center or a specialist we will present this insurance card on your behalf

Physical Therapy Services

Students will have access to a physical therapist after being evaluated by a medical provider at University Health Services The physical therapist will be on site Tuesdays and Thursdays from 530 pm to 730 pm for the assessment and treatment of musculoskeletal dysfunction and or pain and for the assessment and treatment of functional loss Students will have to reserve a 15-minute timeslot by signing up in the morning of on Tuesday or Thursday (log is located at the ABT daily commuters desk at Moore-Hill Dormitory available until 830 am)

When University Health Services recommends therapeutic treatment for an injury the program coordinator will contact the parentlegal guardian of the student to inform himher and to ask for a signed ldquoConsent for Therapeutic Proceduresrdquo form This way we ensure a real-time information flow between the program the student and the parentlegal guardian The program coordinator will provide this form on an as needed basis These services are provided to the students at no cost

No student will be seen by the physical therapist if the Consent for Therapeutic Procedures form is not received by the program coordinator before 500 pm on the day of treatment

Students who are dismissed will be sent home at the expense of the parent or legal guardian If a student is dismissed for reasons of unacceptable conduct there is no refund on Tuition or Room and Board Any student dismissed under these conditions will not be allowed to return to ABTUT in subsequent summers It is the responsibility of the program coor dinator to make decisions in this area

HEALTH ARE AND ERVICES C S All ABT Summer Intensive participants receive medical care at University Health Services (UHS) UHS is located on the UT campus in the Student Services Building at 100 W Dean Keeton Street Austin Texas 78712 The summer operating hours for the University Health Services are from 800 am - 530 pm Monday through Friday

Participants under the age of 18 in need of medical attention outside of UHSrsquos hours andor those with potentially serious or life-threatening conditions will be taken to Dell Childrenrsquos Medical Center of Central Texas The Dell Childrenrsquos Medical Center is located at 4900 Mueller Boulevard Austin Texas 78723

Participants who are age 18 at the time of the Intensive and in need of medical attention outside of UHSrsquos hours andor those with potentially serious or life-threatening conditions will be taken to St Davidrsquos Medical Center The St Davidrsquos Medical Center is located at 919 E 32nd Street Austin Texas 78705

UHS is a full-service ambulatory healthcare clinic with on-site laboratory radiology and pharmacy facilities All ABT Summer Intensive participants are seen in the UHS Urgent Care Clinic A physician and supporting clinical staff provide treatment in this area If any student is seriously ill or injured it is the policy of UHS that the doctor on the case will contact the parents Individuals with potentially serious or life-threatening conditions will be taken directly to a hospital emergency room

Students who require medical attention will be accompanied to the University Health Services by the residential coordinator supervisor or mentor All medical costs incurred for treatment will be the responsibility of the parents or guardian of the individual treated

Some health maintenance services are available for program participants with special needs such as frequent allergy injections as long as they can be administered at University Health Services during the regular hours of operation Participants or parents of minors should contact the residential coordinator by phoning 512-232-7099 to make arrangements with University Health Services for these services

Consent for treatment of a minor is required by law in order for University Health Services to render any medical treatment except in life-threatening situations thus no student will be registered for the ABT Summer Intensive without a Consent for Treatment form (Please see form in DocuSign) The consent form requires four signatures by the parentlegal guardian ndash one giving permission to release health history information one releasing liability in regards to administering medication one giving permission for the minor to receive treatment and the other acknowledging that they have received a copy of the UHS ldquoNotice of Privacy Practicesrdquo

University Health Services Charges

All summer program participants who go to UHS for medical care are assessed a $6000 office visit charge There are additional charges for many of the procedures clinicians may need to perform in order to diagnose and treat illnesses or injuries Samples of services with additional charges include

uml Lab tests X-rays uml Casts splints crutches and dressings uml Injections and IVs uml Nebulizer or other asthma treatments uml Medications and pharmacy supplies

The participant will receive a bill from UHS immediately upon being treated The participant or parentlegal guardian must pay this invoice by the end of the next business day following the participantrsquos treatment UHS accepts payment in form of cash and MasterCard at this time

UHS does not accept outside insurance for medical services The UHS staff can provide information that may be needed for patients to file claims with their own insurance companies if their insurance plans allow them to do so

Physical Therapy Services

Students will have access to a physical therapist after being evaluated by a medical provider at University Health Services The physical therapist will be on site Tuesdays and Thursdays from 530 pm to 730 pm for the assessment and treatment of musculoskeletal dysfunction and or pain and for the assessment and treatment of functional loss Students will have to reserve a 15-minute timeslot by signing up in the morning of on Tuesday or Thursday (log is located at the ABT daily commuters desk at Moore-Hill Dormitory available until 830 am)

When University Health Services recommends therapeutic treatment for an injury the program coordinator will contact the parentlegal guardian of the student to inform himher and to ask for a signed ldquoConsent for Therapeutic Proceduresrdquo form This way we ensure a real-time information flow between the program the student and the parentlegal guardian The program coordinator will provide this form on an as needed basis These services are provided to the students at no cost

No student will be seen by the physical therapist if the Consent for Therapeutic Procedures form is not received by the program coordinator before 500 pm on the day of treatment

Dress Code Policy PLEASE NOTE Any student not adhering to the dress code will be dismissed from class and may only return when properly attired Dancers may bring leg warmers t-shirts shorts and sweat pants for warm-up and between classes but may not wear those items in class

Female Dancers Hair must be up and neatly secured at all times during class

Photographers are often invited to ABTs Summer Training Programs for publicity purposes Your childs name and image may be used in promotional materials including brochures newspaper articles books andor television and for commercial purposes

Scheduling is tentative Scheduling and faculty changes are at the discretion of the American Ballet Theatre Summer Intensive

CLOSING SPECIAL EVENTS

Parent Observation Day We are scheduling two observation classes that provide parents the opportunity to observe a warm-up class taught by ABT faculty The following observation times are scheduled

uml Thursday July 27 2017 - 915 am to 1045 am uml Friday July 28 2017 - 915 am to 1015 am

Observation seats will need to be reserved beforehand (at no cost) Beginning the week of July 17 students will be given the opportunity to register their parentguardianteacher to attend an observation class

Final Performances

uml THE TWO FINAL PERFORMANCES ARE FRIDAY JULY 28 AT 1200 PM AND 230 PM

The performances will be held at the McCullough Theatre which is located in the Performing Arts Center (PAC) at 2375 Robert Dedman Dr Austin TX 78705 on the UT campus httpsmapsutexaseduutm All students are expected to be present for both performances

Performance Tickets uml ADMISSION IS $1500 PER PERFORMANCE FOR ADULTS AND $10 FOR CHILDREN AGES 5-18 THE THEATRE IS OPEN SEATING

Tickets go on sale Monday July 17 2017 Tickets may be pre-ordered online at utabtorg If you choose this method of purchase your tickets will be held at the McCullough Theatre Will Call and you can collect them on the day of the performance You will also be able to purchase tickets on the day of the performance by cash or check only The box office opens 90 minutes before the 1200 pm performance and 60 minutes before the 230 pm performance There are no refunds for performance tickets purchased in advance Those arriving on campus for the performance are advised to park in the Manor Garage The Manor Garage is located at 2017 Robert Dedman Dr Austin TX 78712 Information about parking for the event can be found at wwwutexaseduparkingmapsvisitorindexhtml Performance DVDs Videotaping and flash photography of the final performance are strictly prohibited However for your convenience Take One Productions will professionally record the performances DVDs may be pre-purchased or purchased on the day of the performance The ABT Summer Intensive will be sending information about purchasing DVDs closer to the Intensive

CHECK OUT AND DEPARTURE You may check out of the dorm following the final performances beginning at 400 pm on Friday July 28 or on Saturday July 29 by 1000 am Students may not be checked out prior to 400 pm on Friday July 28

Moore-Hill Dormitory will close at 1000 am on Saturday July 29 and all students must be checked out by this time Students are to leave the room as it was originally found on July 2 If any student leaves their room in disarray they will be required to pay a $5000 cleaning fee A supervisor will inspect each room upon checkout and the student must turn in their key to that supervisor If the key is not properly returned the student will be charged a lost key fee of $7500

The University of Texas will not be responsible for returning any items left by students

If the studentrsquos flight leaves after check out time on Saturday July 29 it is the parentsrsquo responsibility to arrange accommodations andor transportation off-campus Parents are responsible for arranging their childs transportation in advance (Please see the Travel form in DocuSign)

The last shuttle from the University of Texas to Austin-Bergstrom International Airport will be at 1000 am on July 29 2017 The University of Texas is not responsible for any accommodations andor airport transportation after this time

Students are expected to acquaint themselves with the rules procedures and standards of conduct established by the American Ballet Theatre Summer Intensive at The University of Texas at Austin A student who does not fulfill the responsibilities set out by such rules procedures and standards of conduct may be subject to disciplinary action including dismissal from the program

Students not fulfilling the rules procedures and standards of conduct as outlined below may be subject to the following disciplinary actions as per the discretion of the American Ballet Theatre Summer Intensive at The University of Texas at Austin Department of Theatre and Dance program coordinator and residential staff

uml Written Disciplinary Action form and a phone call to parentlegal guardian andor disciplinary meeting with program coordinator

uml Dismissal from program

Rules and Standards of Conduct

uml Students are required to participate in all scheduled classes activities and the final performance(s) The program coordinator must approve any exceptions

uml Students should immediately report any accident or illness to their mentor andor teacher

uml If a student must leave the program early due to a medical or family emergency the parent or legal guardian must contact the program coordinator to sign the student out

uml Students are required to wear their nametags in a clearly visible manner at all times

uml Students should eat all meals at Jester Center in their designated dining area

uml Students must be escorted by a mentor or residential staff at all times when walking between the dorm and campus buildings

Students are expected to acquaint themselves with the rules procedures and standards of conduct established by the American Ballet Theatre Summer Intensive at The University of Texas at Austin A student who does not fulfill the responsibilities set out by such rules procedures and standards of conduct may be subject to disciplinary action including dismissal from the program

Students not fulfilling the rules procedures and standards of conduct as outlined below may be subject to the following disciplinary actions as per the discretion of the American Ballet Theatre Summer Intensive at The University of Texas at Austin Department of Theatre and Dance program coordinator and residential staff

uml Written Disciplinary Action form and a phone call to parentlegal guardian andor disciplinary meeting with program coordinator

uml Dismissal from program

Rules and Standards of Conduct

uml Students are required to participate in all scheduled classes activities and the final performance(s) The program coordinator must approve any exceptions

uml Students should immediately report any accident or illness to their mentor andor teacher

uml If a student must leave the program early due to a medical or family emergency the parent or legal guardian must contact the program coordinator to sign the student out

uml Students are required to wear their nametags in a clearly visible manner at all times

uml Students should eat all meals at Jester Center in their designated dining area

uml Students must be escorted by a mentor or residential staff at all times when walking between the dorm and campus buildings

uml Students must follow all applicable policies outlined in the forms in DocuSign including the Quiet Time and Day Student forms

uml Students must obey the strict ldquoLights Outrdquo rule at 1000 pm nightly and stay in the dorm room assigned to them

uml Students should carefully read the daily bulletin board It contains up-to-the minute information on rehearsals classes and special activities

uml Students may not drive or have cars on campus during Intensive Due to university parking restrictions UT does not offer parking permits

uml Possession or use of drugs alcohol fireworks firearms and tobacco

uml Visitation to rooms or halls of students of the opposite sex

uml Fraternization with students or adults on campus who are not associated with the ABT Summer Intensive

uml Unexcused tardiness or absences from attendance checks classes rehearsals performances or other scheduled activities

uml Inappropriate use of the meal card such as the purchase of merchandise or other Jester City Market products

uml Gambling participating in lotteries or any other games of chance on the premises at any time

uml Disruptive conduct

uml Destroying or defacing institutional property

uml Theft

uml Creating a condition hazardous to another person on the premises

uml Disorderly conduct horseplay harassment of other dancers or use of abusive language on the premises

uml Fighting encouraging a riot or threatening or attempting to cause injury to another person on the premises

uml Elevator misbehavior and

uml Violations of safety rules or accepted safety practices

Students who are dismissed will be sent home at the expense of the parent or legal guardian If a student is dismissed for reasons of unacceptable conduct there is no refund on Tuition or Room and Board Any student dismissed under these conditions will not be allowed to return to ABTUT in subsequent summers It is the responsibility of the program coor dinator to make decisions in this area

HEALTH ARE AND ERVICES C S All ABT Summer Intensive participants receive medical care at University Health Services (UHS) UHS is located on the UT campus in the Student Services Building at 100 W Dean Keeton Street Austin Texas 78712 The summer operating hours for the University Health Services are from 800 am - 530 pm Monday through Friday

Participants under the age of 18 in need of medical attention outside of UHSrsquos hours andor those with potentially serious or life-threatening conditions will be taken to Dell Childrenrsquos Medical Center of Central Texas The Dell Childrenrsquos Medical Center is located at 4900 Mueller Boulevard Austin Texas 78723

Participants who are age 18 at the time of the Intensive and in need of medical attention outside of UHSrsquos hours andor those with potentially serious or life-threatening conditions will be taken to St Davidrsquos Medical Center The St Davidrsquos Medical Center is located at 919 E 32nd Street Austin Texas 78705

UHS is a full-service ambulatory healthcare clinic with on-site laboratory radiology and pharmacy facilities All ABT Summer Intensive participants are seen in the UHS Urgent Care Clinic A physician and supporting clinical staff provide treatment in this area If any student is seriously ill or injured it is the policy of UHS that the doctor on the case will contact the parents Individuals with potentially serious or life-threatening conditions will be taken directly to a hospital emergency room

Students who require medical attention will be accompanied to the University Health Services by the residential coordinator supervisor or mentor All medical costs incurred for treatment will be the responsibility of the parents or guardian of the individual treated

Some health maintenance services are available for program participants with special needs such as frequent allergy injections as long as they can be administered at University Health Services during the regular hours of operation Participants or parents of minors should contact the residential coordinator by phoning 512-232-7099 to make arrangements with University Health Services for these services

Consent for treatment of a minor is required by law in order for University Health Services to render any medical treatment except in life-threatening situations thus no student will be registered for the ABT Summer Intensive without a Consent for Treatment form (Please see form in DocuSign) The consent form requires four signatures by the parentlegal guardian ndash one giving permission to release health history information one releasing liability in regards to administering medication one giving permission for the minor to receive treatment and the other acknowledging that they have received a copy of the UHS ldquoNotice of Privacy Practicesrdquo

University Health Services Charges

All summer program participants who go to UHS for medical care are assessed a $6000 office visit charge There are additional charges for many of the procedures clinicians may need to perform in order to diagnose and treat illnesses or injuries Samples of services with additional charges include

uml Lab tests X-rays uml Casts splints crutches and dressings uml Injections and IVs uml Nebulizer or other asthma treatments uml Medications and pharmacy supplies

The participant will receive a bill from UHS immediately upon being treated The participant or parentlegal guardian must pay this invoice by the end of the next business day following the participantrsquos treatment UHS accepts payment in form of cash and MasterCard at this time

UHS does not accept outside insurance for medical services The UHS staff can provide information that may be needed for patients to file claims with their own insurance companies if their insurance plans allow them to do so

Please be sure to provide a copy of your childrsquos medicalprescription card This will be required at check in on July 2 In the event your child needs medical treatment from St Davidrsquos Medical Center Dell Childrenrsquos Medical Center or a specialist we will present this insurance card on your behalf

Physical Therapy Services

Students will have access to a physical therapist after being evaluated by a medical provider at University Health Services The physical therapist will be on site Tuesdays and Thursdays from 530 pm to 730 pm for the assessment and treatment of musculoskeletal dysfunction and or pain and for the assessment and treatment of functional loss Students will have to reserve a 15-minute timeslot by signing up in the morning of on Tuesday or Thursday (log is located at the ABT daily commuters desk at Moore-Hill Dormitory available until 830 am)

When University Health Services recommends therapeutic treatment for an injury the program coordinator will contact the parentlegal guardian of the student to inform himher and to ask for a signed ldquoConsent for Therapeutic Proceduresrdquo form This way we ensure a real-time information flow between the program the student and the parentlegal guardian The program coordinator will provide this form on an as needed basis These services are provided to the students at no cost

No student will be seen by the physical therapist if the Consent for Therapeutic Procedures form is not received by the program coordinator before 500 pm on the day of treatment

HEALTH ARE AND ERVICES C S All ABT Summer Intensive participants receive medical care at University Health Services (UHS) UHS is located on the UT campus in the Student Services Building at 100 W Dean Keeton Street Austin Texas 78712 The summer operating hours for the University Health Services are from 800 am - 530 pm Monday through Friday

Participants under the age of 18 in need of medical attention outside of UHSrsquos hours andor those with potentially serious or life-threatening conditions will be taken to Dell Childrenrsquos Medical Center of Central Texas The Dell Childrenrsquos Medical Center is located at 4900 Mueller Boulevard Austin Texas 78723

Participants who are age 18 at the time of the Intensive and in need of medical attention outside of UHSrsquos hours andor those with potentially serious or life-threatening conditions will be taken to St Davidrsquos Medical Center The St Davidrsquos Medical Center is located at 919 E 32nd Street Austin Texas 78705

UHS is a full-service ambulatory healthcare clinic with on-site laboratory radiology and pharmacy facilities All ABT Summer Intensive participants are seen in the UHS Urgent Care Clinic A physician and supporting clinical staff provide treatment in this area If any student is seriously ill or injured it is the policy of UHS that the doctor on the case will contact the parents Individuals with potentially serious or life-threatening conditions will be taken directly to a hospital emergency room

Students who require medical attention will be accompanied to the University Health Services by the residential coordinator supervisor or mentor All medical costs incurred for treatment will be the responsibility of the parents or guardian of the individual treated

Some health maintenance services are available for program participants with special needs such as frequent allergy injections as long as they can be administered at University Health Services during the regular hours of operation Participants or parents of minors should contact the residential coordinator by phoning 512-232-7099 to make arrangements with University Health Services for these services

Consent for treatment of a minor is required by law in order for University Health Services to render any medical treatment except in life-threatening situations thus no student will be registered for the ABT Summer Intensive without a Consent for Treatment form (Please see form in DocuSign) The consent form requires four signatures by the parentlegal guardian ndash one giving permission to release health history information one releasing liability in regards to administering medication one giving permission for the minor to receive treatment and the other acknowledging that they have received a copy of the UHS ldquoNotice of Privacy Practicesrdquo

University Health Services Charges

All summer program participants who go to UHS for medical care are assessed a $6000 office visit charge There are additional charges for many of the procedures clinicians may need to perform in order to diagnose and treat illnesses or injuries Samples of services with additional charges include

uml Lab tests X-rays uml Casts splints crutches and dressings uml Injections and IVs uml Nebulizer or other asthma treatments uml Medications and pharmacy supplies

The participant will receive a bill from UHS immediately upon being treated The participant or parentlegal guardian must pay this invoice by the end of the next business day following the participantrsquos treatment UHS accepts payment in form of cash and MasterCard at this time

UHS does not accept outside insurance for medical services The UHS staff can provide information that may be needed for patients to file claims with their own insurance companies if their insurance plans allow them to do so

Physical Therapy Services

Students will have access to a physical therapist after being evaluated by a medical provider at University Health Services The physical therapist will be on site Tuesdays and Thursdays from 530 pm to 730 pm for the assessment and treatment of musculoskeletal dysfunction and or pain and for the assessment and treatment of functional loss Students will have to reserve a 15-minute timeslot by signing up in the morning of on Tuesday or Thursday (log is located at the ABT daily commuters desk at Moore-Hill Dormitory available until 830 am)

When University Health Services recommends therapeutic treatment for an injury the program coordinator will contact the parentlegal guardian of the student to inform himher and to ask for a signed ldquoConsent for Therapeutic Proceduresrdquo form This way we ensure a real-time information flow between the program the student and the parentlegal guardian The program coordinator will provide this form on an as needed basis These services are provided to the students at no cost

No student will be seen by the physical therapist if the Consent for Therapeutic Procedures form is not received by the program coordinator before 500 pm on the day of treatment

Parent Observation Day We are scheduling two observation classes that provide parents the opportunity to observe a warm-up class taught by ABT faculty The following observation times are scheduled

Observation seats will need to be reserved beforehand (at no cost) Beginning the week of July 17 students will be given the opportunity to register their parentguardianteacher to attend an observation class

Final Performances

uml THE TWO FINAL PERFORMANCES ARE FRIDAY JULY 28 AT 1200 PM AND 230 PM

The performances will be held at the McCullough Theatre which is located in the Performing Arts Center (PAC) at 2375 Robert Dedman Dr Austin TX 78705 on the UT campus httpsmapsutexaseduutm All students are expected to be present for both performances

Performance Tickets uml ADMISSION IS $1500 PER PERFORMANCE FOR ADULTS AND $10 FOR CHILDREN AGES 5-18 THE THEATRE IS OPEN SEATING

Tickets go on sale Monday July 17 2017 Tickets may be pre-ordered online at utabtorg If you choose this method of purchase your tickets will be held at the McCullough Theatre Will Call and you can collect them on the day of the performance You will also be able to purchase tickets on the day of the performance by cash or check only The box office opens 90 minutes before the 1200 pm performance and 60 minutes before the 230 pm performance There are no refunds for performance tickets purchased in advance Those arriving on campus for the performance are advised to park in the Manor Garage The Manor Garage is located at 2017 Robert Dedman Dr Austin TX 78712 Information about parking for the event can be found at wwwutexaseduparkingmapsvisitorindexhtml Performance DVDs Videotaping and flash photography of the final performance are strictly prohibited However for your convenience Take One Productions will professionally record the performances DVDs may be pre-purchased or purchased on the day of the performance The ABT Summer Intensive will be sending information about purchasing DVDs closer to the Intensive

CHECK OUT AND DEPARTURE You may check out of the dorm following the final performances beginning at 400 pm on Friday July 28 or on Saturday July 29 by 1000 am Students may not be checked out prior to 400 pm on Friday July 28

Moore-Hill Dormitory will close at 1000 am on Saturday July 29 and all students must be checked out by this time Students are to leave the room as it was originally found on July 2 If any student leaves their room in disarray they will be required to pay a $5000 cleaning fee A supervisor will inspect each room upon checkout and the student must turn in their key to that supervisor If the key is not properly returned the student will be charged a lost key fee of $7500

The University of Texas will not be responsible for returning any items left by students

If the studentrsquos flight leaves after check out time on Saturday July 29 it is the parentsrsquo responsibility to arrange accommodations andor transportation off-campus Parents are responsible for arranging their childs transportation in advance (Please see the Travel form in DocuSign)

The last shuttle from the University of Texas to Austin-Bergstrom International Airport will be at 1000 am on July 29 2017 The University of Texas is not responsible for any accommodations andor airport transportation after this time

uml Students are required to wear their nametags in a clearly visible manner at all times

uml Students should eat all meals at Jester Center in their designated dining area

uml Students must be escorted by a mentor or residential staff at all times when walking between the dorm and campus buildings

uml Students must obey the strict ldquoLights Outrdquo rule at 1000 pm nightly and stay in the dorm room assigned to them

Participants under the age of 18 in need of medical attention outside of UHSrsquos hours andor those with potentially serious or life-threatening conditions will be taken to Dell Childrenrsquos Medical Center of Central Texas The Dell Childrenrsquos Medical Center is located at 4900 Mueller Boulevard Austin Texas 78723

Participants who are age 18 at the time of the Intensive and in need of medical attention outside of UHSrsquos hours andor those with potentially serious or life-threatening conditions will be taken to St Davidrsquos Medical Center The St Davidrsquos Medical Center is located at 919 E 32nd Street Austin Texas 78705

UHS is a full-service ambulatory healthcare clinic with on-site laboratory radiology and pharmacy facilities All ABT Summer Intensive participants are seen in the UHS Urgent Care Clinic A physician and supporting clinical staff provide treatment in this area If any student is seriously ill or injured it is the policy of UHS that the doctor on the case will contact the parents Individuals with potentially serious or life-threatening conditions will be taken directly to a hospital emergency room

Students who require medical attention will be accompanied to the University Health Services by the residential coordinator supervisor or mentor All medical costs incurred for treatment will be the responsibility of the parents or guardian of the individual treated

Some health maintenance services are available for program participants with special needs such as frequent allergy injections as long as they can be administered at University Health Services during the regular hours of operation Participants or parents of minors should contact the residential coordinator by phoning 512-232-7099 to make arrangements with University Health Services for these services

Consent for treatment of a minor is required by law in order for University Health Services to render any medical treatment except in life-threatening situations thus no student will be registered for the ABT Summer Intensive without a Consent for Treatment form (Please see form in DocuSign) The consent form requires four signatures by the parentlegal guardian ndash one giving permission to release health history information one releasing liability in regards to administering medication one giving permission for the minor to receive treatment and the other acknowledging that they have received a copy of the UHS ldquoNotice of Privacy Practicesrdquo

When University Health Services recommends therapeutic treatment for an injury the program coordinator will contact the parentlegal guardian of the student to inform himher and to ask for a signed ldquoConsent for Therapeutic Proceduresrdquo form This way we ensure a real-time information flow between the program the student and the parentlegal guardian The program coordinator will provide this form on an as needed basis These services are provided to the students at no cost

No student will be seen by the physical therapist if the Consent for Therapeutic Procedures form is not received by the program coordinator before 500 pm on the day of treatment

HEALTH ARE AND ERVICES C S All ABT Summer Intensive participants receive medical care at University Health Services (UHS) UHS is located on the UT campus in the Student Services Building at 100 W Dean Keeton Street Austin Texas 78712 The summer operating hours for the University Health Services are from 800 am - 530 pm Monday through Friday

Participants under the age of 18 in need of medical attention outside of UHSrsquos hours andor those with potentially serious or life-threatening conditions will be taken to Dell Childrenrsquos Medical Center of Central Texas The Dell Childrenrsquos Medical Center is located at 4900 Mueller Boulevard Austin Texas 78723

Participants who are age 18 at the time of the Intensive and in need of medical attention outside of UHSrsquos hours andor those with potentially serious or life-threatening conditions will be taken to St Davidrsquos Medical Center The St Davidrsquos Medical Center is located at 919 E 32nd Street Austin Texas 78705

UHS is a full-service ambulatory healthcare clinic with on-site laboratory radiology and pharmacy facilities All ABT Summer Intensive participants are seen in the UHS Urgent Care Clinic A physician and supporting clinical staff provide treatment in this area If any student is seriously ill or injured it is the policy of UHS that the doctor on the case will contact the parents Individuals with potentially serious or life-threatening conditions will be taken directly to a hospital emergency room

Students who require medical attention will be accompanied to the University Health Services by the residential coordinator supervisor or mentor All medical costs incurred for treatment will be the responsibility of the parents or guardian of the individual treated

Some health maintenance services are available for program participants with special needs such as frequent allergy injections as long as they can be administered at University Health Services during the regular hours of operation Participants or parents of minors should contact the residential coordinator by phoning 512-232-7099 to make arrangements with University Health Services for these services

Consent for treatment of a minor is required by law in order for University Health Services to render any medical treatment except in life-threatening situations thus no student will be registered for the ABT Summer Intensive without a Consent for Treatment form (Please see form in DocuSign) The consent form requires four signatures by the parentlegal guardian ndash one giving permission to release health history information one releasing liability in regards to administering medication one giving permission for the minor to receive treatment and the other acknowledging that they have received a copy of the UHS ldquoNotice of Privacy Practicesrdquo

University Health Services Charges

All summer program participants who go to UHS for medical care are assessed a $6000 office visit charge There are additional charges for many of the procedures clinicians may need to perform in order to diagnose and treat illnesses or injuries Samples of services with additional charges include

uml Lab tests X-rays uml Casts splints crutches and dressings uml Injections and IVs uml Nebulizer or other asthma treatments uml Medications and pharmacy supplies

The participant will receive a bill from UHS immediately upon being treated The participant or parentlegal guardian must pay this invoice by the end of the next business day following the participantrsquos treatment UHS accepts payment in form of cash and MasterCard at this time

UHS does not accept outside insurance for medical services The UHS staff can provide information that may be needed for patients to file claims with their own insurance companies if their insurance plans allow them to do so

Please be sure to provide a copy of your childrsquos medicalprescription card This will be required at check in on July 2 In the event your child needs medical treatment from St Davidrsquos Medical Center Dell Childrenrsquos Medical Center or a specialist we will present this insurance card on your behalf

Physical Therapy Services

Students will have access to a physical therapist after being evaluated by a medical provider at University Health Services The physical therapist will be on site Tuesdays and Thursdays from 530 pm to 730 pm for the assessment and treatment of musculoskeletal dysfunction and or pain and for the assessment and treatment of functional loss Students will have to reserve a 15-minute timeslot by signing up in the morning of on Tuesday or Thursday (log is located at the ABT daily commuters desk at Moore-Hill Dormitory available until 830 am)

When University Health Services recommends therapeutic treatment for an injury the program coordinator will contact the parentlegal guardian of the student to inform himher and to ask for a signed ldquoConsent for Therapeutic Proceduresrdquo form This way we ensure a real-time information flow between the program the student and the parentlegal guardian The program coordinator will provide this form on an as needed basis These services are provided to the students at no cost

No student will be seen by the physical therapist if the Consent for Therapeutic Procedures form is not received by the program coordinator before 500 pm on the day of treatment
