Prof. Dr. Athanasios E. Athanasiou, United Arab Emirates & GreeceUndesirable Tissue Effects Associated With Orthodontic Treatment.

Although the risk of harm in orthodontics is considerably limited compared to other invasive medical and dental therapies, there is nonetheless growing concern about medico-legal issues and risk management within the orthodontics community. The presentation refers to the many potential malpractice problems associated with orthodontic therapy and provides sound advice about how to avoid them. The presentation includes the topics of a) enamel alterations associated with orthodontic treatment, b) orthodontically induced root resorption, and c) damage to tooth-supporting tissues in orthodontics.

Dr. Mohamed Alrayssi, United Arab EmiratesThe Sander II Appliance.

Skeletal class II Malocclusion in children is common and challenging problem, so early treatment with mayo-functional appliances could be necessary to avoid future orthognathic surgery or to decrease the severity of the condition in the future.

Many appliances introduced in literature to treat such problem including monoblock, twinblock and fixed functional appliances. Each of them has advantages and disadvantages, in this speech the presenter going to focus on bite jumping appliance (Sander II) and its efficiency in treating Class II Malocclusions.

Dr. Elie Amm, Lebanon“Efficiency in treatment” of periodontal disorders: the orthodontic perspective.

Description: In the lack of solid evidence-based information, what else can illustrate the interrelationship between orthodontics and periodontics than clinical cases? In our presentation we will point out some aspects of this symbiosis such as the implant site develop-ment by tooth movement, the gingival and alveolar bone align-ment, the treatment of periodontally compromised patients and the gingivoplasty.

Dr. Jorge Ayala, ChileFACE Treatment mechanics update.

Current advances in diagnosis, specifically the use of Cone Beam CT, and new materials, make it necessary to modify some of the concepts in the mechanics of orthodontic treatment. In this presen-tation the most important aspects of these changes, which improve the completion of orthodontic cases and shorten the treatment time will be described.

Woo-Ttum Bittner, GermanyBoosting Efficiency – A paradigm for modern orthodontic treatment.

In today’s world where patients demand a fast, comfortable and discreet treatment, it becomes more and more important to use the most up-to-date orthodontic treatment appliance, which helps the practitioner to be successful in a competitive environment. However, the smaller and the more discreet an orthodontic appliance is, the more difficult its application becomes. In order to identify the point when efficiency and effectiveness begin to develop negatively, it is necessary to comprehensively consider different treatment approaches and practice management issues. The best orthodontic appliance will fail if it is not embedded into an efficient practice organization and treatment protocol. The presenter, as founder of the ADENTICS practices, is going to explain his approach for maintaining the same standard of quality in three practices with 80 team members without losing sight of the patients’ wishes.

Dr. Vittorio Cacciafesta, ItalyExtraction Treatments with 2D Lingual Brackets.

Aim: To describe how extraction mechanics can be performed with the use of 2D Lingual Brackets.

Method: Extractions represent a challenging clinical situation especially in patients who seek an invisible orthodontic treatments. The risk of anchorage and torque loss might compromise the final occlusal and esthetic result.

Results: In the present report the author will illustrate the bio-mechanical aspects and clinical management of the orthodontic treatment with the 2D Lingual System in extraction cases, showing the most appropriatearchwiresequence, the best bracket set-up, the anchorage management, and the clinical tricks and tips to reduce torque loss.

Conclusions: The combination of 2D Plus Lingual Brackets with squared and rectangular ribbon archwires can be a solution to manage the space closure mechanics and torque in extraction cases. After the initial alignment and leveling phase with round Ni-Ti wires, space closure mechanics are performed with .016” x .016” and .022” x .016” SS wires. Finishing and torque control might be achieved with .022” x .016” beta-titanium wires. 2D Lingual Technique repre-sents a valuable and alternative method for invisible orthodontic treatments also when extracting teeth.

Prof. Dr. Nazan Kücükkeles, TurkeyTreatment of open bite with curved NiTi wires and anterior elastics: Can we control the extrusion of molars?

Etiology of open bite is quite complicated. These cases may present overeruption of posterior teeth, deficient vertical growth of anterior teeth (usually related to tongue posture), short ramus, etc. But the treatment mechanics can be briefly classified as posterior intrusion and anterior extrusion in combination with my other apic excersises in children. Skeletal anchorage such as mini plates and micro screws are used for intrusion of posterior teeth. Extrusion of posterior teeth also occur during treatment with incisor extrusion which leads to an increase in lower face height. Dr. Kim in MEAW technique referred to this problem by adding second order bends on posterior teeth for vertical control. We simplified his technique by using curved rectangular NiTi wires and anterior elastics for the correction of anterior open bite without undesirable extrusion of posterior teeth. Treatment effects of this technique was investigated on 2 different group of patients. During the lecture the results of these studies will be presented together with some case reports.

Dr. Domingo Martin, SpainThe Need of Extractions in Orthodontics for Function and Esthetics.

It seems incredible, but it is a fact that once again non extraction treatment is back again in our profession. Ever since Dr. Angle

proposed non extraction treatment as an ideal treatment plan, the profession has gone back and forth from extraction to non extraction for nearly one hundred years … you would think that we would have learned our lesson, unfortunetley this has not occurred. The main reason is that doctors are not aware that non extraction is not a treatment goal but rather a treatment decision and this is where the problem arises, the lack of goals in orthodon-tics today. In my presentation I will talk about “goal oriented treat-ment” and why we need to extract if we want to achieve our goals. I will illustrate my presentation with many examples and cases.

Prof. Dr. Roberto Martina, ItalyAn efficient approach to Class II orthopedic treatment.

Class II malocclusion is one of the most frequent orthodontic prob-lem; as a matter of fact, it occurs in about one third of the popula-tion. Hence, a wide number of therapeutic approacheshave been proposed, over the time, for the treatment of skeletal Angle Class II malocclusion in growing patients. Till today even if 80% of Class II are due to a mandibular retrusion, the most common choice is to distalize the upper dental arch and to advance the lower one.

An orthopaedic-functional treatment should rather be performed in order to stimulate mandibular growth. The efficacy and the dif-ferent effects among the appliances are some of the most debated topics in literature, with the most controversial results. The aim

of this lecture will be to share with the participants a 20 years old clinical experience gained in the Department of Orthodontics of the University of Naples Federico II with bite jumping appliances and to discuss the rationale and the evidence of this treatment approach, with particular regards to the management of patients with different vertical growth patterns.

Prof. Dr. Ravindra Nanda, USA, IndiaSpeeding Orthodontic Treatment: Efficient Use of Simple Mechanics and New Technologies.

This presentation will give an overview of latest information regarding studies related to methods under investigation to speed orthodontic tooth movement (OTM).

Most of the proposed methods still need convincing evidence based data. Status and results of various clinical and experimental studies currently under progress at my department here at the University of Connecticut will be presented along with a critical appraisal of each and every proposed OTM enhancing proposed method. The second part of the presentation will discuss efficient mechanics which can effectively reduce the length of the ortho-dontic treatment.

The presentation will show the role played by careful selection of wires, brackets and TAD’s in managing length of orthodontic treatment.

PD Dr. Christian Sander, GermanyAesthetic correction within the treatment.

The demand for orthodontics is steadily increasing. From an ortho-dontic’s perspective, the objective of a treatment is the shaping a harmonic arch while at the same time creating an ideal mastica-tory function. The patient’s treatment objectives are often “straight and beautiful” teeth. However, both objectives are typically difficult to achieve by the orthodontist. This is especially true with adults, due to frequent demineralization of the teeth, white spots, and attrition before treatment. Therefore, it creates a need for improvement to achieve visually pleasing results. This presentation intends to provide tips and tricks to optimize the aesthetics.

NiTi Gadgets for Severe Treatment Problems such as Rotated or Tipped Teeth.

Impacted or tipped molars and severe rotated teeth are common orthodontic problems. The treatment is not only challenging for the orthodontist, but also time consuming.The purpose of this lecture is to introduce easy solutions for these problems.

Depending on the position of the molars, different types of biomechanics should be applied. The treatment objectives are ideal tooth position and a healthy periodontal condi-tion. In order to achieve these objectives, a proper biome-chanical approach is necessary.

Combining with the right choice of appliances and biome-chanics, molars can easily be uprighted without reactiva-tion procedures. For a severely rotated tooth, a segmented NiTi wire can be used. Teeth that have been rotated about 90° can be corrected with a segmental approach.

This lecture will compare traditional mechanics with a combination of NiTi and stainless steel or NiTi-segments.

Common clinical problems with molar uprighting will be discussed to show biomechanical solutions.
