“An innovative approach to help troubled teens realize their dreams”

MISSION:

The Mission of Delaware HOOPS 4 Life Inc. is to enhance and

develop coping skills, self-respect, accountability and self-sufficiency for adolescence aging out of Foster Care entering adulthood.

STRATEGY:

The Delaware HOOPS 4 Life Inc. will provide structure, promotes

self-discipline, attention to detail, awareness and teaching each participant to embrace the concept of Team (Together Everyone Achieves More), while making healthy life style choices with an emphasis on academic success, accountability, self-pride and leadership.

METHOD OF ATTAINMENT:

Each Delaware HOOPS 4 Life Inc. member will identify

deficiencies and or areas of strengths they would like to enhance. Members will have an individual “HOOPS Pathway” i.e. map created to highlight their landmark progress made and or setbacks (lessons learned while transitioning) that will enable them to acknowledge individual growth, development and overall Team accomplishments.

“DE HOOPS 4 Life Goals”

The goal of the Delaware HOOPS 4 Life Inc. is to; positively impact

the lives of each participant while teaching, modeling and demonstrating quality leadership and coping skills for Teens in Foster Care via the experience of practicing, learning and playing competitive Basketball in a

controlled, safe and fun environment.

OUTCOME MEASURES: The tools used to quantify

programmatic goals are as follows; School Attendance,

Grades, Attitude, Behavior, Monitoring of Social Interactions (Home, Community, Public) and comparing “Life Choices” prior to

participation in the Delaware HOOPS 4 Life Inc. program.

PROGRAM OVERVIEW

The majority of young men in foster

care have had limited opportunities to

assume leadership responsibilities and or

model other men in leadership roles.

Delaware HOOPS 4 Life Inc. is a

program designed to allow young men

ample occasions to learn the value of

“Playing Smart; espousal integrity by

making sound decisions, Playing Hard;

being persistent & persevering during

difficult times. Playing Together; learning

the value of Team Work i.e. “family” while

grasping the fundamentals of competitive

Basketball and developing lifelong coping

skills in a controlled, safe, and fun

environment.

Delaware HOOPS 4 Life Inc. program

will enhance each young man’s ability to

comprehend the difference between one

that follows and one who creates his own

path by using his mind to make sound

judgment for himself and his Team,

during the course of the program and for

life going forward.

Each active participant in the Delaware

HOOPS 4 Life Inc. will require a 110%

(unconditional) commitment throughout

the duration of the curriculum, with the

expectation of total excellences in the

area of academics, daily living, self care

and social relationships. These attributes

will be the governing structure used to

uplift, empower and ultimately become

the guiding force in each Players life!

Scope of Services

SELF~CARE: DE HOOPS 4 Life Inc. promotes responsibility self

awareness, discipline and Healthy Life Styles. There is a zero tolerance for drug, alcohol or tobacco usage. Any player(s) who is found in violation of terms and participation conditions of this program, will be asked to voluntarily leave that month & must remain drug, alcohol & tobacco free prior to his return, no exceptions!

DAILEY LIVING: Players will be held accountable for maintaining good

grades, school attendance and unquestionable behavior. A contract will be signed at the onset of the program reflecting the above participation conditions. Players with initial poor grades, attendance or behavioral

problems, will be required to have their weekly DE HOOPS 4 Life

Progress Report signed by each teacher and or program administrator

prior to any practice and or game. If a Player(s) can’t produce his progress report or is found to be out of compliance with the program rules and or their contractual obligations, said player will not practice, play or travel with the Team. Player(s) will be given two reviews to make improvements before being dismissed from the Team.

SOCIAL RELATIONSHIPS: DE HOOPS 4 Life Inc. Players will be

provided the opportunity to learn leadership responsibilities and skills, while modeling the attributes of their Coaches & special guest speakers (Social Service Professionals, Police Officers, etc.) during the course of the program. Players will also assist in the development of the “HOOPS Pathway” project, that will serve as a (constant) visual reminder of each

players individual growth and development.

Operational Cost:

DE HOOPS 4 Life Inc. program fees will cover Gym Rental (4 per

month), Uniforms, Warm~Up Shirts, Practice Equipment, Tournament Fees (2 per Month, 1 over-night, to include Lunch Dinner, Breakfast), Player Travel Coverage (Gas & Transportation), Hotel / Motel Room Fees, Tournament & Team Awards, Administrative duties, (Logistics & Program Planning), College Tours, SAT Prep / TEST from April ~ July 2014. Individual Player Sponsorships are encouraged.

PLAYER OPTIONS:

Monthly Program Fees [$120.00] Full Time Player

2-3 Tournaments (4 - 8 Games), 2 Scrimmage(s), 4 Practices

Monthly Program Fees [$75.00] Part Time Player

1-2 Tournaments (2 - 6 Games), 1 Scrimmage, 2 Practices

EXPERIENCE SUMMARY

Jr. High School Boys Head Basketball Coach, Holy Cross School Responsibilities included establishing a fair and equitable screening process to select players, enhanced player fundamentals via Basketball Drills, introduced man to man defensive concept, half court trapping defenses, various out of bounds sets, sideline inbounds sets, offensive sets, full court press formations and press break formations, spearheaded fund raising efforts to procure new Basketball Uniforms for the Girls & Boys Programs, ordered and tracked warm up gear, establishing electronic real time communication with parents and facilitated awards ceremony.

JV Girls Head Basketball Coach, Saint Thomas More Academy As the Head Basketball Coach for STMA Basketball Team, my duties consisted of developing the “Skill Set” of Scholar Athletes by repetitiously facilitating drills to enhance each player’s Basketball knowledge of the game and natural abilities. Provided players with credible instruction that enhanced their overall basketball “IQ”, published playbooks with diagrams, illustrations and written definitions explaining the unique basketball vernacular used to describe Offensive Half Court Sets, Defensive Strategies, Player Positioning, Half and Full Court Presses, Press Breaks and Inbounds Plays. Employed strength and conditioning drills along with weekly organized video tape film review & instruction to correct player inaccuracies and strengthen team tactics. Implemented player evaluations at the onset of the season to quantify the effectiveness of programmatic goals and measure the progress of individual players, while identifying areas of improvement prior to the start of the next Basketball season. Managed and organized JV and Varsity practices, scheduled games, trained and supervised Assistance Coaches/ Volunteers, established strong working relationships with school administrators, parents and program boosters, while adhering to DIAA Rules and supporting of the Mission of both the Varsity Head Coach and Saint Thomas More Academy.

Program Consultant, President Dover Basketball Assoc. (DBA) JIMU Basketball, Health & Sports Ministry, Camden, Delaware JIMU is an acronym for “Jesus in Me & You”… JIMU Public Health & Wellness Ministry provides instruction to individual Basketball Players primarily from Holy Cross School & STMA Programs. It’ Mission is to enhance the life of each participant while teaching Basketball fundamentals in a controlled safe and fun environment, via sound communication, interactive video instruction, strength & conditioning drills. Formed multiple Basketball Travel Team(s) and Leagues to build the confidence and character of Scholar Athlete, while preparing each player with tenable Basketball resources to play High School Basketball.

Basketball League Commissioner, Upward Basketball Inc. My scope of services included; communicating effectively with parents, boosters and community leaders, implementing team structure, encouraged player and staff inclusion while building consensus with coaches, program officials and volunteers, provided both pre and post seasonal visual presentations, read and discussed scriptures from the Bible with player(s).

Program Director, Belcamp Basketball Coalition Responsibilities as Belcamp Parks & Recreation Program Operations Manager included; enrollment of players, recruitment of coaches and sponsors. Facilitated meetings, provided training for Coaches, Referees, repetitively taught Basketball fundamentals in a fun, safe learning environment, organized fund raisers, strengthened community partnerships & involvement, designed brochures, flyers, game and practice schedules, procured uniforms, awards, created sponsorship letters, registration documents, supervised coalition officers, practices / game day activities, collaborated with area High Schools & Community Colleges, organized / initiated Basketball Tournaments and Training. Facilitated Community Summer Basketball Camps and Coached Championship Basketball Teams.

Organizations wishing to donate time (volunteering man hours presenting) and or other resources (Hotel / Motel Cost, Food, Drinks, and Vehicles Access) are also welcomed. DE HOOPS 4 Life Inc. is a not for profit organization. Our focus is to provide Teens (who have been victims of circumstances beyond their control) transitioning into adulthood, with the tools to becoming contributors to society & one day good parents to their families.
