DEAR DR. DONOHUE: I have had heartburn for many years. I take Prilosec for it, and it works fine for me.

My doctor scheduled me for a gastroscope exam. I haven’t had one for quite a while. The doctor who did the scope called me and my family doctor to say that I had changes seen in the biopsy he took. He says I have Barrett’s esophagus, which can turn into cancer. Naturally, this has me worried.

Will you elaborate on Barrett’s esophagus and what I have to look forward to? Neither of my doctors has suggested a change in medicines. Do you? -- T.M.

ANSWER: Barrett’s esophagus is a change in the kind of cells that line the lowermost part of the esophagus, the part that attaches to the stomach. That change brings with it a chance of further, cancer changes. The risk is quite small, about 0.5 percent per year.

Barrett’s esophagus happens to people who have GERD, gastroesophageal reflux disease, more popularly called heart-burn. You take a medicine, Prilosec, that suppresses the pro-duction of stomach acid. Medicines that act similarly some-times can retard the progression of these cell changes into cancer cells. They most definitely control heartburn. Barrett’s also can happen to people who do not have GERD.

The chance of cancer developing depends on a number of criteria. One is the kind of cells that have evolved. If they show low-grade changes, the likelihood of cancer is not as great as it would be if they show high-grade changes. The length of the area involved with these changes is another factor in determining the cancer risk.

You must not have had signs that raise the risk for cancer, or the doctor would have recommended immediate treat-ment. About the only thing you need to do is comply with the suggested follow-up scope exams so the doctor can see if any procedures are needed to eradicate these new cells. You don’t have to sit around and worry. You have not received a death sentence.

Hiding Income OffshoreSome people try to avoid paying taxes by illegally hiding income in offshore bank and brokerage accounts. They also use offshore:• Debit and credit cards • Wire transfers • Foreign trusts • Employee-leasing schemes • Private annuities or life insurance plansThe IRS guides auditors on dealing with those hiding income offshore in undisclosed accounts. The IRS draws a clear line between people with offshore accounts who voluntarily come forward and those who fail to come forward.2.) Identity Theft and PhishingIdentity thieves use this technique to acquire your personal data to gain access to financial accounts. They send phony e-mails, usually posing as representatives of a financial company or the IRS itself.A typical tax fraud e-mail notifies you of an outstanding refund or audit. The e-mail then urges you to click on a link and enter your Social Security and credit card numbers. Don’t click on this link, and don’t send your private numbers.The reality is the IRS never uses e-mail to start contact with you regarding your accounts. If you receive unsolicited e-mail that claims to be from the IRS, you can forward the message to [email protected]. The only official IRS website is www.irs.gov.3.) Return-Preparer FraudDishonest return preparers make their money by:• Skimming a portion of their clients' refunds • Charging inflated fees for tax-preparation servicesSome preparers promote the filing of fraudulent claims for refunds on items like fuel tax credits to recover taxes paid in prior years. You should be careful when hiring a tax preparer, especially one who promises something that seems too good to be true.To increase confidence in the tax system and improve compliance with the tax law, the IRS requires tax preparers to:• Register with the IRS • Have a preparer tax identification number (PTIN) • Pass competency tests • Take ongoing continuing professional education coursesPaid tax preparers must apply for a PTIN before preparing any federal returns. Paid preparers can be attorneys, CPA’s, or enrolled agents. 4.) Filing False or Misleading FormsScam artists will file false or misleading returns to claim refunds they’re not entitled to. Frivolous-information returns are used to legitimize illegitimate refund claims. Ex: Form 1099-OID (Original Issue Discount) claiming false withholding credits The new scam evolved from an earlier scam that a "straw-man" bank account had been created for each citizen. Under this scam, individuals filed a false

Zero WagesIn this scam, taxpayers attach 1 of these forms that shows no or little income:• Form 4852: Substitute for Form W-2: Wage and Tax Statement • Form 1099R: Distributions From Pensions, Annuities, Retirement or Profit-Sharing Plans, IRAs, Insurance Contracts, etc. • Corrected Form 1099These taxpayers include a statement refuting information they previously submitted to the IRS. Ex: They might cite on Form 4852 "statutory language behind IRC 3401 and 3121." They also might include some reference to their employers refusing to issue a corrected Form W-2 for fear of IRS retaliation.They usually attach Form 4852 or 1099 to a 0 return.9.) Misuse of TrustsFor years, scammers have urged taxpayers to transfer assets into trusts. They promise:• Reduction of income subject to tax • Deductions for personal expenses • Reduced estate or gift taxesHowever, some trusts don’t deliver the promised tax benefits. Also, the IRS has seen an increase in the improper use of private annuity trusts and foreign trusts to divert income and deduct personal expenses. You should seek the advice of a trusted professional before entering into a trust arrangement.10.) Fuel-Tax Credit ScamsThe IRS penalizes those who make unreasonable claims for the fuel tax credit. Some people — like farmers who use fuel for off-highway business purposes — might be eligible for the fuel tax credit. But some people claim the credit for nontaxable uses of fuel when their jobs or income levels make the claims unreasonable.Fraud involving the fuel tax credit is considered a frivolous tax claim. Those who improperly claim the credit might have to pay a $5,000 penalty.

Through preparation, time, money and mechanical skills, your vehicle survived the winter. Spring has arrived, and once again, you will need to prepare your vehicle for the upcoming season. Many people will take their vehicle to a shop and say, “Call me when it’s done,” while others won’t maintain their vehicle at all. But you, you’re different. You’re going to pop the hood and get your trusty steed ready to take to the road.

Checking the tire pressure and wiper blades and cleaning the interior are maintenance 101 for you. You’re more concerned with the inner workings of the vehicle.

Start with the battery. Check the posts and connections. Make sure they are free of corrosion and making good contact. The same goes for the spark plugs-clean them if necessary. Spark plugs can fire three million times every 1,000 miles, so proper operation is crucial.

Check the oil and oil filter. Your vehicle’s manual will have recommendations on how often to change the oil and filter. A good rule of thumb is every 3,000 to 5,000 miles. Make sure you are using the correct grade of oil. The outside temperature is now warmer, and as you probably know, oil gets thicker when it’s cold and thinner when it’s hot. Using an oil grade of 10W40 means that the oil will flow at a 40 viscosity rate during the warmer spring months.

Another filter to check is your vehicle’s air filter. The air

filter keeps dust and other debris from getting into the engine’s moving parts. Dust and debris in your vehicle’s engine can cause it to be less efficient and underperform. It is a good idea to replace the air filter annually or every 12,000 to 15,000 miles.

Next, check the coolant level. The radiator on a cold engine should be completely full. The reservoir should be at the “cold” level. Again, check the vehicle’s owner’s manual to determine when the system should be flushed and fresh coolant added.

The next level to check is the brake fluid level. If the level has fallen below the “low” mark, it may indicate excessive brake wear or a leak somewhere in the system. Unless you are certified to repair brake systems, you should take the vehicle to someone who is. This function of the vehicle is too important to not be overseen by a professional.

telephone. We need to beware.You know all your routines, and if something

new comes up, be suspicious. If the “bank” calls and wants to talk about your account, hang up and call them back to be sure you’re really talking to your bank and not a scammer. The same goes if “Medicare” or “Social Security” calls you and asks for personal information, or if the “electric com-pany” says your power will be cut off if you don’t make a payment over the phone. Don’t be fooled. They’re not likely to call you on the phone.

Two of your biggest weapons again scam-

mers: Caller ID on your phone and a shredder. By knowing who’s calling before you even answer the phone, you can protect yourself against aggressive thieves. And with a shredder you can destroy any mail or documents that have your name on them before they go in the trash.

Remember the “granny scam,” where someone posing as a grandchild calls up and says he or she is in trouble and needs emergency money? Far too many grandparents have fallen for that scam and wired money to thieves. One state is now making it a law that companies who do the wire trans-fers notify the sender about where the money is actually ending up. Call your state senators and representatives and suggest that this be a law in your state as well.

If you’re near a senior center, ask it to put to-gether a program to tell seniors about scams and how to handle those who call and pressure you about giving personal information on the phone.

If you’re ever in doubt about who’s calling you and asking for information, hang up.

Q: The antique door to our bathroom is solid oak. Unfortunately, it’s no longer closing straight -- the door latch is lower than the hardware on the frame. It’s also scraping the floor when it’s opened and closed. How can we repair this? -- Cliff, West Hartford, Conn.

A: Fixing this problem can be as simple as adjusting the hinge pins of the door, or as complex as resetting them (which isn’t too complex, it’s just a two-person job).

Sticking, dragging doors -- or doors that won’t close completely -- can have a number of causes. In newer houses, a door’s hinges might have been placed incor-rectly, or the mortise of the door might not be deep enough (or too deep). Checking the depth of the mortise is simple: run a finger along the hinge leaf that’s set into the door. If the leaf isn’t flush with the wood, the trouble may lie there. The new wood might have swollen slightly, as well.

Because you’re in an older home, the door probably opened and shut for years with no problem, meaning the hinges are (or were) set just fine. However, the door frames of aging homes typically settle as time passes, and the frame could be slightly warped. The hinges also might have worked loose.

The first (and easiest) fix is to check all the hinges -- tap them down into place if necessary -- and tighten the screws holding the hinges to the door. If a screw won’t

tighten down, remove it, insert a wooden toothpick (or several) into the hole, and reinsert the screw.

If the door still sags, try putting a shim between the bottom hinge and the door. Remove the door completely from its hinges (this is where that second person comes in handy). Then, unscrew the bottom hinge leaf from the door. Using the leaf (the flat part) as a template, cut a piece of noncompressible cardboard, particle board or sheet brass (available in various precut sizes at the hardware store) to match. Punch out screw holes in the same area as on the leaf. Lay the shim against the door first, then fasten the hinge leaf to it. Rehang the door and check the fit.

Conversely, if the door latch were higher than it should be, you would place a shim in the top hinge.

Now, what if the latch lines up but the door still scrapes or sticks? Try covering the sticking edge with chalk, and open and close the door. Mark spots on the door edge where the chalk has rubbed away. Lightly sticking doors can be repaired by sanding the contact points. Don’t sand too much, or the door won’t fit tightly.

HOME TIP: To easily sand away sticky spots from the bottom of the door, tape a piece of sandpaper to the floor and briskly move the door back and forth across it. Elevate the sandpaper with thin pieces of cardboard as the sanding process continues.
