Excused Sick Industrial (RX)

and Sick Industrial (SI)

y  When an employee is injured on the job, the employees time should beentered as Excused Sick Industrial (RX) anytime during the first 15calendar days. On the 16th calendar day, their time should then be

entered as Sick Industrial (SI). However, before an employee can useSI, it is mandatory that all medical documentation and the Personnel Action form (PAF) be submitted to the HR Manager for processingbefore that 16th day. If the medical documentation or PAF is notprocessed, payroll will not accept time coded with SI codes.

y SI will automatically end 90 calendar days after injury. However, it willbe the responsibility of the employee to notify the Risk Manager,Debbie Jo Maust, if they need to extend SI time. After 90 days theemployee will no longer be able to use SI unless approved by the RiskManager.
