• Receives $1000 incentive

Tier I: Full Participation

• Submits the following measures on the K-HEN Measures tab for Jan, Feb, Mar 2014 and submits or attests to one process measure per topic area on the Process Measures tab by Apr 30:

– ADE-12; 13; & 111– CAUTI-18; 19; &113 (NHSN)– CLABSI-24; 25 & 122 (NHSN)– Falls-37 & 39 (Claims Based)– EED (OB-40)– OB-54 & 55 (AHRQ); 118 & 120– PrU-58 & 61 (AHRQ)– READ-75 (Jan/Feb only)– SSI-89 (4 or more Procedures) (Jan/Feb only)– VAE-96d; 96e & 96f– VTE-104 & 105 (AHRQ)

• Submits same measures meeting the Data submission schedule for each month (4Q is only 2 months)

• Receives $150/month – $450 each for 2Q & 3Q– $300 for 4Q

Hospital B 2Q

• Submits same measures meeting the Data submission schedule for each month (4Q is only 2 months)

• Receives $250/month – $750 each for 2Q & 3Q– $500 for 4Q

• Total Possible: $4000

Tier I Quarterly Submission

Hospital A 2Q, 3Q & 4Q

• Submits same measures with all three months completed by July 31

• Receives $250 each quarter

• Total Possible: $1750

Hospital B 2Q, 3Q & 4Q

• Submits same measures with all three months completed by July 31

• Receives $500 for each quarter

• Total Possible: $3500

Tier II & III

Tier II• Submits at least one HAB report for each quarter• $250/quarter ($1000 possible)

Tier III• Achieves 40/20% reduction from baseline in

applicable topics by Nov 1 OR sustains rate at or below benchmark

• $1000 incentive
