INTRODUCTION
Shaft Sinkers have a number of sites where we
intensively use trackless equipment and
personnel together in a confined area.
This in itself is therefore a risk!
The question is then how do we get around it to
ensure that no person underground is exposed
in this trackless environment to life threatening
situations.

PAST TRACKLESS FATAL ACCIDENTS

These fatal accidents occurred within Shaft
Sinkers.
• 2007 - Fatal injury occurred when a pedestrian
was crushed by an LHD against the side wall.
• 2007 - Fatal injury occurred when a face crew
member was squatting whilst cleaning his
jacket. A LHD ran over him.
• 2008 - Fatal injury occurred where the UV
operator was crushed between the mobile
crane and the UV.

Why SNAKES for a trackless environment?
Python - low risk
Cobra - medium risk
Adders - fatal risk

SOME HAZARDS, LIKE PYTHONS, ARE HARMLESS WHEN SMALL IF LEFT UNTREATED, THEY WILL GROW AND THEN YOU NEED TO WATCH OUT!
• Oil leaks
• Faulty safety devices
• Using pirate parts
• Inexperience operators
• Inexperience artisans

• Ignorance to start of shift and end of shift
procedures
• Failure to report incidents and accidents
• Failure to use correct parking procedures – not
using stop blocks.
• Failure to conduct NDT tests on stop blocks,
tow bars, supporting sprags and trestles.

• Occupational illnesses – Expose to noise, dust,
vibration, heat, etc
• Fatigue, work stress – cause errors which will
eventually...
• Personal problems – focus not on the task at
hand
• Unsafe behaviour – taking short cuts

• Negligent operator behaviour
• Tyre damages
• Disregarding safe travelling distances between
people & machines and machines & machines
• Key control
• Failure to do proper pre-use inspections
• Smoking engines

SOME HAZARDS IN THE WORKSPACE ARE LIKE PUFF ADDERS. THEY LIE THERE
INVISIBLE UNTIL YOU STEP ON THEM
• Operators under the influence of drugs or alcohol
• Not following maintenance procedures – fail to
lock out
• Not following pre-use inspection procedures – fail
to test the brakes, operating with ‘A’ Class
Hazards
• Not following operating procedures – operate with
faulty anti collusion unit (Booyco)

• Operators not trained for the task – multiple
machines
• Inadequate operating risk assessments – not
identifying ALL relevant hazards
• Not isolating stored energy before maintenance
is done on machines
• Not following operating procedures – fail to
warn, sounding the horn.

CONCLUSION
Safety is a state of mind.....
So be cautious of your next step.....
And be aware of the SNAKES.....
