Transfer Review ProcessTo convert statements of SEN into EHC plans

Approximately 8 weeks before the Transfer Review, Schools/ Colleges write to everyone required to attend the meeting informing them of the date, time and venue and requesting a report from appropriate professionals, the young person and their parents/carers
Schools/ Colleges identify pupils requiring conversion of an existing SEN Statement or Section 139a Learning Difficulty Assessment (LDA)
Education, Health and Care Plan Pathway: Transfer Review ProcessInformation is gathered from the young person, their parents/ carers and professionals. Approximately 2 weeks before the Transfer Review Schools/ Colleges collate and copy all documents collected and produced and send out to anyone who is attending the Transfer Review
A Transfer Review is held
Summary of the outcomes are referred to the EHC Co-ordinator
Resources approved by SEND Commissioning Hub
Draft EHC Plan prepared
Draft returned to EHC Co-ordinators for issue to the young person and their parents/ carers
Comments received from the young person and their parents/ carers. The EHC Plan is amended and final Plan issued

1 week after the Transfer Review Schools/ Colleges collate and copy all documents and compile the pro forma and return to the EHC Co-ordinator who will refer to the SEND Commissioning Hub
SEND Commissioners confirm that resources identified at the Transfer Review are available
Maximum duration for the process is 14 weeks
If a request is made to develop a wiki web site then the development can begin at any point along the stages

Actions Required
• Consider list of pupils provided by the LA • Identify dates for children whose reviews fall within phase 1• Set date for transfer review meeting – allow 8 weeks notice
for all attendees and report contributors• Identify who should be invited to attend or contribute a report• Write to those identified, providing them with relevant
framework for completion• Collate information at week 6 and chase outstanding reports• Prepare for transfer review meeting at week 8• Complete and return summary framework by end of week 9.
