Sub: Immediate submission of Form 11 and KYC Documents by EPF members forUAN2 based EPF transactions.

With reference to the letters as cited above,Form II, for verification by BSNLabout new employment of EPF members and generation of EPF's UAN (UniqueAccount Number) or for linking of existing UAN Number (already allotted to new EPFmember/BSNL employees during their previous employments), were called for from

newly joined BSNL employees in MH Circle (EPF No: 2946 onwards).

However, even after instructions and reminder issued by this office, only fewForms have been received in this office. The correctly filled Forms have already beenverified by this office for generation of UAN by EPFO in such cases, which membersthemselves can directly check with EPF Portal.

In this regard, the officers of this office have recently met twice with theRegional Provident Fund Commissioner Mumbai to resolve this issue of UNAgeneration or linking of old UAN without submission of Form II and KYC documentsby EPF members of BSNL. However, after much discussion and deliberations, theRegional Provident Fund Commissioner Mumbai has instructed that it ismandatory on the part of EPF member to fill and submit, Form 11 and KYC

- paymentlupdation and other transactions very shortly and, failure to immediately submitForm l I on the part of EPF members will lead to non-allotment of UAN Numbers andin absence of the same, monthly contributions cannot be made/updated for suchmember.

In this situation, non-submission of Form II by new EPF members (EPF No:~ ~ ~~ 2946 onwards) on immediate basis may led to complications with respect to their EPF

Further, vide this office latest latter dt Dt 31/08/2016, KYC documents werecalled for from EPF members (old and new) by 15/09120.16. However, till date no anyKYC Documents, as per the instructions issued by this office, are received in this office.In this regards, the EPF members may again be instructed to seed their KYC detailswith Aadhar, PAN and Bank Account Details and submit the KYC documents to

~their SSA's EPF Nodal Officer on most immediate basis as per instructions in letterdt 31/08/2016.

'IAs submission of Form 11 to employer and, seeding of KYC with Aadhar, PAN

and Bank Details on EPF Portal and submitting of KYC documents to employer aremandatory requirements from EPF members underEl'F & MP Act 1952, the BSNLemployees who are covered under EPF, are therefore, duty bound to comply with thesame, being Central Government PSU employees governed by service rules thereunder.

In view of the above, all EPF members may be instructed to submit Form 11 andKYC documents (as per instructions already issued), to their SSA's EPF Nodal Officerfor further necessary actions so that the EPF Nodal Officer of the SSA can forwardingthe same to this office so as to reach this office on or before OS/IO/201y

In this regard, all SSA Heads and IFAs are requested to bestow upon theirpersonal attention in. implementation of this Employees' Welfare Scheme procedurewhich is mandatory on part of EPF members to follow.

Further, for non-compliance of EPF's mandatory requirement by EPFmembers, it shall solely be responsibility of fPF member concerned for anycomplications and resulting consequences thereof.
