Our Setting - Geauga County, OH 95,000 residents over 408 square miles. Historically rated in top 3 of all Ohio counties for average family income & public health indicators. Fourth best place to raise a family in the US (Forbes 2008). DDC Clinic is in the geographic center of the worlds 4th largest Amish settlement - 16,500+ with estimated population doubling rate of less than 20 years. ~50% of Geauga County’s developmental disabilities are represented in just 15% of our population. Just 30 miles yet a world away from world-class major medical centers.

Patient Assistance Program • Launched in 2008 to help 1 needy family with medication for

seizure disorders. • Program managed by our business office and nursing • Provides much needed navigation services for families • Insures quality of life for area patients • Has grown to save local community health funds considerable

resources. Will serve a projected 95 families in 2015

Holmes County Outreach Clinics

• Born of a tremendous community need coupled with strong ties between the Geauga & Holmes settlements.

• Launched in earnest in 2004 with the support of a five- year award from the Robert Wood Johnson Foundation.

• Support was expanded through 2015 to include both patient care and research with five-year combined commitments from the Ohio Development Disabilities Council, Holmes County Training Center, and area parents.
