Lauren with a Side of Lewy by Lauren U, PAC Core Team

People often ask me what hasbeen most difficult aboutadjusting to life with dementia. In the past I've thought it waslosing my independence, lackof a driver's license, beingunable to work, my inability tosleep well, or myhallucinations, delusions, orparanoia. It recently occurredto me that the most disturbingchange has been my lack of confidence. I used tothink of myself and present myself as quiteconfident. I felt certain of my relationships, my art, managing money, working as an RN, and myinteractions with the world in general. I could stepforward in situations, confident in my ability toappropriately handle whatever came my way.

A Thank You to Everyone whoworks to better the lives ofpeople living with dementiaby Reverend Linn Possell,PAC Speaker and Lead Mentor As we begin to roll up oursleeves and get ready to domore intentional work in 2019,I thought this would be a goodtime to celebrate all of thepeople, communities, andorganizations that are strivingto a common goal... to lift upthose loving someone livingwith dementia and those livingwith dementia. Recently aPAC team was at Rochester Presbyterian Home inRochester, NY. This community is doing great workand wanted to enhance what they are already doingwell with some PAC training. My Thursdays with Claude: Part 3Putting PAC Skills into Practiceby Leslie Carney, LCSW, PAC OutreachAdministrative Coordinator Putting your agenda in yourback pocket, as Teepa says, isn't always easy, especiallyif there are many things toget accomplished in thecourse of the day. Even involunteering it's hard not tohave an agenda. I mentallyhad a game plan for skills I wanted to practice anddementia knowledge to be mindful of, but Claudehad his own agenda, and I was getting ready to getschooled. I still used my skills, just not the ones Iplanned on using. Claude was sitting alone at an empty table. Iintroduced myself in Positive Approach to Care(PAC™) fashion and asked permission to sit downwith him. He kind of huffed in acknowledgement ofme, and I could see from his demeanor thatsomething wasn't right.

Immigrants and refugees in care communities facea host of challenges and complexities, as do theindividuals caring for them. It can be difficult enoughto engage with and understand the desires ofsomeone with dementia who speaks your nativelanguage, is accustomed to the food you cook, theholidays you celebrate, and the pop culturereferences you make. But those who requirememory care and speak little English or areseparated from their cultural norms experience aunique kind of isolation and loss of social capital.

As someone who has worked with both refugeeand ageing populations, I have seen firsthand theimportance of reminiscence therapy and life storywork. Apart from recalling memories, these effortsacknowledge and validate one's cultural identity, helping to create essential connections betweenthe care partner and recipient or their family.

stage dementia. He is wobblyon his feet. He sometimesdrools. He can't speak. He isincontinent. But here we are, eight milesfrom home. We have riddenhere on our tandem trike withan electric motor that helps us keep up with ourbike friends. We stopped to use the restroom. Nowhe has to lift his foot about 2.5 feet high to step overthe trike to straddle it and then sit on the seat. Istand next to him ready to support him if hestumbles, and I just wait silently so he can processthe next move. Then he does it; he lifts his leg overwithout a problem. I strap his feet onto the pedals, and we are ready to continue our ride, sometimesriding 40 miles in a day. This activity has helped us remain healthy, strong, and social while George lives with dementia and Ipartner with him to provide care. All of that hashelped me maintain some sanity. Exercise actuallyincreases endurance and daily energy availability,so you have more energy to deal with dementia.
