1. INTRODUCTION

Now provision has been given to easily access the Patient Portal Application from your smart phones. Using this Patient Portal Application, you can conveniently schedule an appointment; view their Lab/Radiology results; view and compose messages; send refill request - the regular Patient Portal features.

To access the Patient Portal Application, you need to download the ‘PrognoCIS My Health Records’ application on your smart phone. If you are an Android or iPhone user then you can download it for FREE from Play Store or iTunes store respectively.

2. LOGIN SCREEN

After the ‘PrognoCIS My Health Records’ application is installed, the Application

launcher icon: displays on your device.

2.1 LOGIN FOR REGISTERED USER

To login to the Patient Portal application:

1. Tap the ‘PrognoCIS My Health Records’ App launcher icon: . The Login screen displays. Refer to figure 1.

2. Enter the Patient Portal’s Username and Password provided by the clinic.

3. Enter the Clinic ID.

4. Tap login. The Change Password screen displays.

Note: The Change Password screen displays only if you are accessing the Patient Portal for the first time else the Agreement screen displays. Refer to figure 2.

5. Enter the Patient Portal’s Current Password, New Password, and Confirm Password newly entered.

Note: The New Password must contain at least one alphabet and a number. Also, the password length should be between 8 to 10 characters.

6. Enter the Secret Question and Answer for the entered question. This helps you retrieve the Password, in case you forget.

7. Tap proceed. The Password Request Submitted confirmation message displays. Refer to figure 3.

2.2 FORGOT PASSWORD?

To access the Patient Portal application if the password is forgotten:

1. On the Login screen, enter your Username and Clinic ID.

2. Tap the Forgot Password? link. The Forgot Password screen displays. Refer to figure 6.

8. Enter the Current Password received in your email, your New Password, and Confirm Password.

Note:

The New Password must contain at least one alphabet and a number. Also, the password length should be between 8 to 10 characters.

You can also change your Secret Question and Answer, if required.

2.3 SIGN UP FOR NEW USER

If your details are registered in Clinic, however you haven’t signed up for Patient Portal then follow the steps below to sign up for the Patient Portal Application. After signing up, you will receive an email about your Patient Portal’s username and Password to login the ‘PrognoCIS My Health Records’ App.

To sign up for the Patient Portal Application:

1. Tap the ‘PrognoCIS My Health Records’ App launcher icon: . The Login screen displays. Refer to figure 10.

3. Enter your Legal First Name and Legal Last Name, as registered in the Clinic.

4. Enter your DOB (Date of Birth) in MM-DD-YYYY format, as registered in the Clinic.

5. Enter the registered Email address.

Note: On tap of submit, if the entered Email address doesn’t match with the Email address registered in the Clinic then the Enroll Request Failed confirmation message and the Chart Number field displays. Refer to figure 12.

6. Enter the Cell Phone number.

7. Enter the Clinic ID provided by the Clinic.

8. Enter your Chart Number, as registered in the Clinic.

Note: The Chart Number field displays only if the entered Email address doesn’t match with the Email address registered in the Clinic.

9. Tap submit. The Enroll Request Submitted confirmation message displays. Refer to figure 13.

The Home screen of ‘PrognoCIS My Health Records’ App is similar to your Patient Portal’s Home page (Refer to figure 18). The Home screen displays your name along with your picture as registered in the Clinic. It also displays your Physician’s name and Dashboard consisting of the following details:

3.6.4 CHANGE PASSWORD

From the Change Password screen, you can change your login password.

To change your password, tap Change Password on the Personal Info screen. The Change Password screen displays. Refer to figure 89.

To change your Login Password:

1. Enter your Current Password, your New Password, and Confirm Password.

Note:

The New Password must contain at least one alphabet and a number. Also, the password length should be between 8 to 10 characters.

You can also change your Secret Question and Answer, if required.

From Authorized Users screen, you can view a list of your Authorized Users and add a new Authorized Users to access your Patient Portal. You can also delete any required defined Authorized User to restrict that respective User from accessing your Patient Portal.

To view a list of your Authorized Users, tap Authorized Users on the Personal Info screen. The Authorized Users screen displays. Refer to figure 91.
