Spirit Platform™ Bed Warranty The warranty information for the Spirit Platform™ bed manufactured by CHG Hospital Beds Inc. (herein after referred to solely as “CHG”) is as follows:

CHG warrants to the original purchaser of its products that the product or any part thereof, in the judgment of CHG, will not be defective in material or workmanship for five (5) years from date of original purchase.

Such defects will be repaired or replaced (at the CHG’s discretion) free of charge for parts only, provided the defective product is shipped to the factory or an authorized depot at the customer’s own expense. Shipping charges for return of the product to the customer will also be at the customer’s own expense. This warranty does not include any labour or shipping charges incurred in replacement part installation or repair of any such part. CHG’s sole obligation and your exclusive remedy under this warranty shall be limited to such repair and/or replacement. Parts, in the judgment of CHG, found to be damaged through normal wear and tear, impact, abuse, overloading, fire, lightening, water damage or power surges, brown-outs, improper supply voltage or use of other manufacturer’s components and/or accessories will not be covered.

Any alteration made to and/or performed on the bed and/or one of its components voids this warranty.

A Return Goods Authorization number (RGA) must be obtained before returning any products or parts to the factory. We regret that no products will be accepted at the factory without an RGA number. No other warranty, expressed or implied by CHG or its agents shall be considered valid.

Important Precautions

WARNING

Safe Working Load of Spirit Platform™ bed

454 kg (1000 lbs) DO NOT overload the bed. The combined weight of patients, visitors, mattress, additional equipment/accessories, and bedding should not

exceed 454 kg (1000 lbs)

WARNING Possible ENTRAPMENT, SUFFOCATION, and/or APHIXIATION HAZARD exists when bed is used with patient restraint mechanisms of any type. Patient restraint mechanisms can cause SERIOUS INJURY or DEATH if not affixed and/or adjusted properly. CHG Hospital Beds does not recommend and/or condone the use of patient restraints. However a qualified healthcare practitioner, leveraging their experienced medical judgment and following established healthcare best practices, believes that the patient may attempt to do harm to themselves and/or others, patient restraints may be prescribed as being medically necessary to safeguard the wellbeing of the patient and/or staff. Patient restraints shall be used, affixed, and/or adjusted ONLY by qualified healthcare practitioners after the patient has been clinically assessed as requiring specialized treatment and/or restricted mobility.

WARNING A Spirit Platform™ Bed is a customer requested bed configuration that is intended to perform a specialized function and has several unique differences from regular hospital and/or long term care facility beds.

The Spirit™ Platform Bed is a non-powered bed with no moving parts. Emergency functions such as Trendelenburg and reverse Trendelenburg are NOT available.

A Spirit™ Platform Bed is NOT intended for normal ward use.

WARNING Before any patient is placed in the Spirit Platform™ Bed, CHG Hospital Beds recommends that a qualified healthcare practitioner thoroughly assess the needs of every patient on a case-by-case basis to determine the level of care a patient requires.

If a qualified healthcare practitioner, leveraging their experienced medical judgment and following established healthcare best practices, believes there is a potential for the patient to become physically violent and/or to inflict harm on themselves and/or others, CHG Hospital Beds recommends that floor anchors be used to prevent unauthorized bed motion to safeguard the wellbeing of patients and/or staff.

Mattress Specifications A mattress is not included with the Spirit Platform™ Bed. A CHG Hospital Beds’ mattress is recommended. It is recommended that the length and width of the mattress suitably fits the mattress deck. It is also recommended that the mattress has a thickness of 6" (152 mm).

WARNING DO NOT use the Spirit Platform™ Bed without a mattress having a thickness of at least 6" (152 mm) and of recommended dimensions.

The mattress MUST entirely rest upon the mattress deck and MUST fit snugly between the head and foot boards.

The mattress MUST fit snugly within the mattress keepers (when so equipped).

WARNING Possible patient ENTRAPMENT Hazard or FALL RISK exists if using a non-specified mattress.

Patient entrapment may result in SERIOUS INJURY or DEATH. Use only a mattress of recommended specifications with the Spirit Platform™ Bed. CHG Hospital Beds will not be responsible for any injury to patient and/or staff and/or damage to bed that may result with use of non specified mattress.

WARNING CHG Hospital Beds recommends that the customer perform a thorough patient assessment to determine if the bed system and mattress selection is appropriate for the patient on the basis of their clinical needs, fall risk, and mental capacity. CHG Hospital Beds is of the belief that to ensure maximum patient safety, there is simply no substitute for frequent patient monitoring by qualified healthcare practitioners. CHG Hospital Beds also recommends that the customer conduct and document a patient entrapment risk assessment for every mattress combination that is intended for use on the Spirit™ Platform Bed to identify and address any exposure to areas of potential patient entrapment and/or fall risk as part of a comprehensive and proactive bed safety program.

Lowering the Spirit Platform™ Bed

Your Spirit Platform™ bed may be shipped vertically on a skid. Adhere to the following to safely lower.

Beds shown above in typical position when being transported and/or stored in pairs. 1. Cut all of the shipping

straps holding the two beds together.

2. Remove all of the wood used to secure the beds to the shipping skid.

3. Carefully slide one bed away from the other bed so they are not touching.

4. Slip the bed bags off both beds.

IMPORTANT: BED IS HEAVY!!! Never attempt to lower the bed without assistance. CHG recommends that the bed is most safely lowered with at least 2 suitable people as shown in the following steps. Maintain your hold on both beds at all times. The bed not being lowered may fall if not steadied during the lowering process.

5. Rotate bed around so that frame is accessible as shown above.

IMPORTANT: Prior to lowering the bed, ensure that you have adequate room. At least 8 feet is required to safely lower the bed to the floor from the upright transportation and/or storage position.

6. Walk the bed so that it is partially hanging over the edge of the shipping skid.

7. Position a piece of cardboard packaging as shown above (arrow) to protect the lower edge of the bed as you carefully slide the bed off of the shipping skid.

8. Begin lowering the bed by gently pulling the bed from the upright position and slowing walking backwards.

IMPORTANT: It may be necessary to reposition your grip to allow further lowering of the bed. Communicate with the other person before adjusting your grip!

9. When appropriate, maintain your grip, lock your arms, and lower the bed completely to the floor by bending your knees.

IMPORTANT: Do not drop bed!

WARNING DO NOT drop the bed when lowering during storage and/or transport. If the bed is dropped, permanent damage to bed components may occur.

DO NOT use a bed that has been dropped. Non obvious damage may have occurred that may expose staff and/or patients to unforeseen safety hazards.
