Easy Forms ask simple questions in small chunks instead of multiple empty forms.

Easy Forms help people answer:● Are these the right form for my

use?● What parts of each form do I need

to complete?

With Easy Forms, people answer a question once, and information is used in entire form suite.

Go to ILAO● Go to ILAO’s Order of Protection Easy Form launch page:

● Go to ILAO and search for “op”

● Bookmark it or save a shortcut

Get Started● For “anonymous” users who want forms now

● No personal information saved

● Users have to accept Terms of Use

Create Account or Log In● Save answers, make changes, and get forms now & later

● Helpful to avoid having to start over if network problems

Sign up for an LHI account● Account Type: Self-helper State: Illinois

● Enter name, email, username, password, security question & answer

● Click checkbox to accept Terms of Use

How do Easy Forms work?1. The program asks you questions.

2. Your information goes into an Answer File.

3. The program uses the Answer File to build your forms.

4. You can download your forms to print or e-file.

5. You can save your Answer File to use again later.

Interview tips

● Some answers are required (questions are in red text)

● Save every few minutes (if logged in)

● Check for help text at bottom of the screen for definitions of legal terms

● Best results by going through interview screen by screen

How to save answers● Click on the Save icon in the upper-right (must be logged in)

● Edit a name of the answers

● Add a description (optional)

● Click Save Answers

● Click Back to Interview

Click Finish when done● On last page, click Finish or Next to end interview

● Wait for it...

● Screen will go gray while forms are being generated

What would you like to do now?● Once forms are ready, users will see screen with below options

Download Your Forms● Forms can be downloaded in 1 PDF file (PACKET) or individually

● Green check mark appears next to forms already downloaded

What forms does the program make?● Every time:

○ PACKET - PDF with all forms

○ Instructions

○ Petition

○ Emergency Order of Protection

○ Order of Protection

○ Summons

● Optional (based on answers):

○ Additional Incidents of Abuse

○ Additional Case Information

○ Confidential Name & Location of the School or Daycare

○ Affidavit of Parenting Time Supervisor

After downloading● If e-filing from the same computer, save the files to the Desktop

● Save a copy to USB drive or email

● Print forms

● E-file instructions provided by the Clerk

Going back to use saved answers● Go to LHI: https://lawhelpinteractive.org

● Log In (upper-right corner)

● My Answers > Saved Answers

Saved Answer options● Select action using icons on appropriate Saved Answer row

● Get Forms (download)

● Open Interview - restarts interview with answers

● Email forms / document(s) - sends copy of all forms to any email address

● Delete - deletes saved answers
