Student – Athlete Code of Conduct I am a UW-Manitowoc Blue Devils student-athlete;

I. I will maintain a high scholastic average and display leadership in classroom attendance and participation. II. I will carry a minimum of 9 credits and maintain a GPA of 2.0 in order to remain academically eligible. III. I understand that participation in intercollegiate athletics is an opportunity and a service provided by UW-

Manitowoc and the Wisconsin Collegiate Conference. My participation is a privilege, not a right. IV. I am a representative of UW-Manitowoc in the community and while visiting other communities related to

intercollegiate contests. I will uphold UW-Manitowoc in a positive manner that will help to maintain or improve the image and respect in the community.

V. I will refrain from “trash talking” in any manner including using profanity, obscene, racial, or sexual language or gestures, or physical or verbal assault since those actions have no part in an athletic program and will not be tolerated.

VI. I have worked diligently to achieve this level of athletic performance. I understand the negative effects that alcohol, tobacco, and other drugs have on my academic and athletic performance. Therefore, I choose not to use these substances. I will encourage my teammates to be substance free.

VII. I understand that there will be no use of tobacco products, drinking of alcoholic beverages, or use of illegal drugs in connection with any varsity athletic event. This includes during travel, overnight, and at the contest. Conference sponsored events along with all intercollegiate competition is covered by these rules. As stated in the Student Rights and Regulations handbook, “Violation of these provisions by a student may lead to the imposition of a disciplinary sanction, up to and including suspension or expulsion, under s. UWS 17.03(l)(b), Wis. Adm. Code.”

VIII. I am committed to the Blue Devils and to the team’s success. I will be a contributor to the team. I intend to participate for the duration of the season. I will make every effort to attend all practices and games. I will communicate with the coaching staff in advance if for any reason I will not be able to attend a scheduled team meeting, practice, or contest.

IX. I take pride in my team, my coaching staff, my campus, and my community. I wear the Blue Devils uniform; I am a part of a team. I will be team oriented and will strive for the success of the team over my individual success.

X. If I have an issue with any member of the team, including the coaching staff, I will address the issue with respect and professionalism.

XI. I understand that team issued uniforms, warm-ups, practice gear, and equipment are the property of UW-Manitowoc. I will keep track of and care for the items with diligence. I will take responsibility for any items that are damaged, lost, stolen, or misused while under my watch and care.

XII. I will return all team issued gear within one week of the completion of the season.

I have read the Code of Conduct and I willingly agree to conduct my actions according to the twelve codes.

Student – Athlete Medical Certification The University of Wisconsin Colleges requires that all individuals provide written proof that they are physically qualified to participate in intercollegiate athletics. This medical certification form must be signed and dated by medical doctor and returned to the Athletic Director prior to athletic participation. This document is good for two seasons of intercollegiate participation. Wisconsin Collegiate Conference UW-Manitowoc Blue Devils Hold Harmless Agreement

The undersigned hereby acknowledges that s/he understands that participation in any intercollegiate sport at the University of Wisconsin – Manitowoc is purely voluntary. The undersigned hereby releases the WCC, the University of Wisconsin – Manitowoc, and the University of Wisconsin Board of Regents, its successors, assigns, officers, agents, and employees from any and all claims, demands, and causes of action whatsoever in any way growing out of or resulting from the undersigned student’s participation in the activities of ___________________.
