2. Register and track your claim details online by logging on to Autolite.com/Special-Offers and clicking the

3. Fill in this mail-in rebate form. Cut out the Autolite® Spark Plug UPC symbol(s) from the package(s), circle price(s) paid on your dated cash register receipt and mail all items to the address below. PLEASE RETAIN A COPY OF YOUR CASH REGISTER RECEIPT AND UPC FOR PURCHASED ITEM(S) FOR YOUR RECORDS.

4. Envelope with rebate request must be postmarked no later than January 31, 2020 and requests received after February 15, 2020 will not be honored.
