General Information We welcome visits by family, friends, and even pets. Normal visiting hours are between 9:00 a.m. and 8:00 p.m. daily. Main doors are locked at 10:00 p.m., but later visits can be scheduled with the nurse on duty. We ask that residents sign out when leaving the building. Please make arrangements for off-campus outings at the nurse’s station. Residents are encouraged to personalize their rooms with their own belongings and decorations. We reserve the right to limit or prohibit certain personal items to avoid storage or safety issues. Meals are served three times daily. Snacks are also provided. Alcoholic beverage consumption is allowed as permitted by a resident’s physician. Vancrest Health Care Center is a NON-SMOKING campus. Residents and visitors are not permitted to smoke on campus grounds. Beauty and barber services are available. Appointments can be made through the Social Service Department. Residents are responsible for the costs of services provided. Admissions

For all Admissions To Our Facility:Admission from a hospital is one method of entering our health care facility. Most often, the hospital discharge planner will coordinate efforts to provide us with necessary information and acquire approval for admission from the Area 3 Passport Agency.

Admission from home requires that the family members or the resident provides all medical information, have the resident examined by the physician, and get the Area 3 Passport Agency approval for admission.

On the first day of the health care stay, our Vancrest Team will guide the resident or a person acting on their behalf through the admission process.

Guidelines Include:• The resident’s physician must certify that the resident requires the

services of a health care facility

• The resident must have a history and physical exam within five days prior to health care admission

• The Area 3 Passport Agency must approve health care admission

• Health care admission will be based upon our ability to meet the care needs of the potential resident

• The payment source should be determined and, if necessary, an application for Medicaid must be made prior to admission
