Arizona State Veteran Home - Phoenix

One of two Arizona tate !eteran Homes is located in "hoeni# at $%$% &. 'rd treet. (t is a )** bed+s,illed care nursing facility operated and maintained by the Arizona -epartment of !eterans ervices.This is a tate owned and operated facility built ne#t to the !eterans Administration /edical 0enter andteele (ndian chool "ar,. A!H serves the long term care and rehabilitative needs of the veterans ofArizona. The )** beds are divided into one hundred fifty long term care beds+ a twenty five bed memorycare unit and a twenty five bed /edicare1rehab focused unit. A!H provides rehabilitative therapy

seven days a wee,. "hysical therapy is designed to improve movement and function. Occupationaltherapy is designed to improve daily living s,ills and peech therapy to improve memory and swallowingdisabilities. A noezelen room+ originated and developed in Holland+ provides a 2uiet therapeuticstimulating visual environment for dementia residents. The facility provides a free private telephone forlocal calls at bed side for each resident and free cable T!. Each resident room contains a T! and a -!-

player. The facility has a one thousand -!- movie library. 3esidents can en4oy the internet throughoutthe building using facility provided 5(F(.

Honorably separated !eterans including their spouses and widows are eligible for admission. A!H hasan energetic and caring staff that provides nursing care+ therapeutic recreation and social services to thosewho have served us. A !eterans Benefits 0ounselor is available onsite to assist the veteran resident inobtaining their veteran benefits1pension. ocial ervices are available to provide psychosocial support anddischarge planning for residents and their families. !eteran seniors and their spouses who choose long6term care en4oy a vibrant+ independent lifestyle with customized services to fit their needs. 3esidentveterans en4oy comfort+ dignity and warm social connections. A variety of activities are planned andpromoted+ increasing the sense of community for each resident. 0aregivers are respectful+ well6trainedand attentive.

Arizona State Veteran Home - Tucson

The other Arizona tate !eteran Home is located ne#t to outhern Arizona !A Health 0are ystem 7!AHospital8 at 999 East A4o 5ay Tucson+ A: ;9 79)*8 =';6)%9?. This%'*+***6s2uare6foot %)* s,illed bed facility offering many amenities to Arizonas veterans such as allprivate rooms+ full baths+ and selective dining in a homeli,e environment. The facility provides a range ofmedical aid+ including respiratory rehabilitation and speech+ occupational and physical therapy

Applicants for either home are eligible for admission if they are an honorable separated veteran or thespouse of a veteran. The applicant must re2uire s,illed nursing care as determined by a physician. Theremust be documented evidence of the absence of tuberculosis. The Arizona -epartment of !eteran ervices Fiduciary -ivision provides fiduciary services toveterans+ spouse and minor children of veterans in the most efficient and cost effective manner possible.The -ivision charges minimum fees for its fiduciary services that are less than alternative fiduciaries maycharge. Age Gimit An honorably discharged veteran shall be eligible for employment preference+ rights+ andprivileges under any merit system in the state in the state or any political subdivision thereof+ regardlessof age+ if otherwise 2ualified.

)iil Serice*!eterans who pass an e#amination for employment by the state+ county or city will have 9

points added to their certification score. The veteran must have served for more than si# 7=8 months andbe separated under honorable conditions. !eterans entitled to compensation for a service6connecteddisability will have %* points added to their certification score. 0ertain spouses or surviving spouses shallbe given a 9 point preference if the veteran died of a service6connected disability.

Fire De!artment*/embers of any fire company inducted into the military establishment of the nitedtates for military training are authorized reinstatement to their previous rating after discharge frommilitary service.

Police an$ Fire De!artment*The period of military service shall be included in computing the length ofservice of the employee to determine eligibility for retirement.

%esere Status./ar mergenc"* Appointive officers or employees of the state or of a politicalsubdivision will be reinstated to their former position upon completion of military service to which 7s8 hewas inducted or ordered during time of war or was called to service because of their status as an active orinactive member of the 3eserves.

A veteran or eligible dependent who has applied for educational benefits under the .(. A former member of the Arizona &ational uard who received a purple heart citation on or after

eptember %%+ )**% 7who was medically discharged because of a service6connected in4ury8 is eligible fora tuition waiver. A tuition waiver for Arizona community colleges and universities is also available to adependent of the national guard member or member of the nited tates armed forces who was a residentof the state of Arizona or stationed in Arizona and who was ,illed in the line of duty or who died fromin4uries suffered in the line of duty while traveling to or from duty.

5hen a veteran or a surviving spouse dies without sufficient means for funeral e#penses+ the 0ountyBoard of upervisors is responsible to ensure that burial will not be in a portion of ground usede#clusively for burial of paupers. A suitable plot will be used+ and the county may apply to the ..-epartment of !eterans Affairs for e#penses not to e#ceed C%9*.**.

5hen the county buries an indigent veteran+ the county cler, will ma,e an application to the ..

Arizona State Veteran )emeter"

Burial in the !eterans 0emetery is open to all members of the Armed Forces+ and with certain e#ceptions+veterans discharged from active duty under conditions other than dishonorable. Their spouses+unremarried widow or widower+ minor children+ and under certain conditions+ unmarried adult dependentchildren are also eligible for burial. Also eligible for burial are members of the reserve components of theArmed Forces+ &ational uard+ and 3eserve Officer Training 0orps who die while on active duty fortraining or performing service+ or who have )* years of service in reserve components of the ArmedForces creditable for retired pay.There is no charge for interment for veterans and a one6time nominal fee for eligible spouses anddependents.
