Complete the free application for Student Aid (FAFSA) found online at www.fafsa.ed.gov to determine eligibility forstate/federal grants. Financial aid is initially based off of your previous two years’ earnings. The QVCC School Code is: 010530.

You will need an Email address and credit or debit card to complete the application.• Submit your high school transcript, diploma or GED• Submit your immunization records:

Proof of immunization for measles, mumps and rubella if born after 1956 and for varicella, if born after 1979 (waived forstudents who are part-time and non-degree)

• Submit official transcripts from prior colleges attended or from military service. Transcripts should be evaluated priorto course selection. For military transcripts for Army, Coast Guard, Marine Corps, or Navy, please contacthttps://jst.doded.mil. Air Force transcripts can be requested through www.au.af.mil/au/ccaf/transcripts

• Students must sign and submit a Request of Veterans Benefits Form each semester. Your classes will notbe certified with the VA until this form is received.
