Personnel are required to deep clean all workout equipment used during their workout before putting the equipment away. All sanctioned Volunteer Life Saving Corps events including, but not limited to; special events, fundraisers, socials, competitions, etc. will require BOD President, Medical Director, Training Officer, and VLSC Captain approval. o The Jacksonville Beach Lifeguard Challenge and the USLA SALA Regionals Competition has been canceled for this year. Non-active members, including non-active Retired members are not permitted at the Lifeguard Station, unless on official business or attending a Tuesday night meeting. Active members who are off-duty should limit their Station use to help minimize the possible spread of COVID-19. Abusing this guideline can result in all non-working personnel’s station privileges being revoked until further notice. o VLSC Commissioned Officers, Medical Director, Training Officer, or an Ocean Rescue Officer (on a JBOR day) have the authority to tell a member to leave the Station, if needed.

Volunteer Life Saving Corps SAFETY BULLETIN – COVID-19 OPS

Please fully read this bulletin as it explains our current plans and operations as we continue to work through the COVID-19 pandemic and the start of returning to what soon will be considered the “new normalcy”. We will continue to provide bulletins when major plans, protocols, and/or operations are updated or changed.

Effective immediately, the following policies and procedures are implemented until further notice:

ORGANIZATION AND LIFEGUARD STATION - • Public member access to the Lifeguard Station is prohibited unless they are being

treated as a patient or working in an official capacity. This includes, but not limited to:using the handi-head, non-member events in the Training Room, station tours, etc.

• Public members and non-active members, including non-active Retired Members arenot permitted to use station equipment unless for official use.

• All exterior doors shall remain closed except for the East Boatroom Garage Door toreduce the potential for loitering or unauthorized entrance. The Officer of the Day canallow for the Communication Center’s door to be open.

• Personnel are required to deep clean all workout equipment used during their workoutbefore putting the equipment away.

• All sanctioned Volunteer Life Saving Corps events including, but not limited to; specialevents, fundraisers, socials, competitions, etc. will require BOD President, MedicalDirector, Training Officer, and VLSC Captain approval.

o The Jacksonville Beach Lifeguard Challenge and the USLA SALA RegionalsCompetition has been canceled for this year.

• Non-active members, including non-active Retired members are not permitted at theLifeguard Station, unless on official business or attending a Tuesday night meeting.

• Winter Recruit Class and Training Committee has been reinstated. • Summer Recruit Class is postponed.

o Capt. Atkins is strongly in favor of having a second recruit class this year even if it means ending in August or September.

• Tuesday night meetings will resume back at the Lifeguard Training in the Training Room, starting Tuesday, May 12, 2020 at 7:30 p.m.

o Members are asked to only come to Tuesday night meetings, when they have to for attendance purposes.

o If you have a letter in and you do not need to attend the meeting for attendance purposes, you do not need to be present for your letter to be read. However, if your letter requires further discussion or questioning and you are not in attendance, your letter is subject to be denied or tabled.

o Members are to sit “a chair length” from each other members in attendance. o Members are still encouraged to email the Registrars at

[email protected] before 7:00 p.m. of the Tuesday night meeting if they would like to add a committee report or general message to the minutes.

DWELLING – Dwelling is permitted with restrictions.

• Dwellers are allowed to sleep overnight at the Lifeguard Station, but are to follow the guidelines of not being at the Lifeguard Station unless absolutely needing so, between the hours of 10:00 a.m. and 10:00 p.m.

• Members are to sleep in every other bunk. o Unless a physical barrier is between bunks, members are to sleep head and toe.

For example, one member is to sleep with their head facing West and one member is to sleep with their head facing East.

• Members wishing the stay the night are to shower before entering the upstairs’ Members Only area. This is an important additional step to help prevent the spread of COVID-19 in the living area.

• Members sleeping at the Lifeguard Station are to sanitize the dormitory at a minimum of twice a week in addition to the normal cleaning that is to be done weekly.

PUBLIC OUTREACH –

• Picnic guards through the Volunteer Life Saving Corps will not be scheduled. • The beach wheelchairs service is suspended. • Safety presentations given to community groups at or away from the Lifeguard Station

will not be scheduled. OPERATIONS -

• A new Medical Standard Operating Guideline has been issued. • Personnel are required to use hand sanitizer after any patient contact and then wash

hands at their earliest possible. • At the end of each day, the Lifeguard who is assigned to Beach Equipment for their End

of the Day Job is to sanitize each pair of binoculars, specifically the eye covers, the hand placement area, and the bottom of the binoculars where the users breaths towards when using the binoculars.

• Starting Sunday, May 17, 2020, beach staffing levels will resume back to “normal beach staffing”. All personnel are required to sit their crew days, at minimum. Per Capt. Atkins, one-year members are required to sign every guard sheet until further notice.

• When responding to calls-for-service, response personnel will decrease to one officer

and two guards unless the call-for-service is considered “major” and/or “life-threatening”. Reference additional information under the Patient Care section.

• A Frequently Asked Questions (FAQs) document created by the Chief of Jacksonville Beach Police Department is attached for your review.

• Training has been reinstated, with limitations.

o POOL CLASS – Pool class is still TBD as Duval County Pools have not reopened. Pool class will only be available to those who have completed all of their classes, but Pool due to the COVID-19 Pandemic.

o CPR CLASS – All members who do not have their CPR Class signed off will need to see the Training Officer.

o EMR – All EMR classes have been completed for the 2020 Training Season. o BEACH CLASS – Members MUST complete their online Beach Class in order for

their Beach Class to count. The classroom part is virtual through Socrative. Login to

https://b.socrative.com/login/student/ and type in VLSCTRAINING as your classroom code. You must type in your full name when prompted or your training will not count. If your training is not recorded correctly, you will have to re-take the training.

• Training Season is extended to Memorial Day 2020 at 5PM. o Understand that if personnel do not complete annual refresher training per the

guidelines of the Standard Administrative Procedure, personnel will be marked as incomplete and not eligible to work for both the Volunteer Life Saving Corps and City of Jacksonville Beach.

o Members are still required to pay their dues and turn in their physical, if applicable, by the end of Training Season in order to maintain their active status.

o This Training Season deadline implies to all personnel who are not on a leave of absence. All members who are on a leave of absence that ends after the Training Season ends will have two weeks from the end of their leave of absence to pay their dues, complete a physical, if applicable, and complete their refresh training.

PERSONNEL PRECAUTIONS –

• All on-duty personnel should take measures throughout the day to ensure all surfaces within the Lifeguard Station are decontaminated with an EPA-registered hospital grade disinfectant, routinely.

• Personnel should not take any photos or videos with public members to where they are placed in close contact.

• Personnel are advised to wear rash-guards or shirts and sunglasses, at all times when all possible to minimize splash exposures from the public.

• Personnel are to review the attached CDC guidance on donning and doffing PPE and the N95 Reuse Document.

• Personnel who have traveled to the New York, New Jersey, Massachusetts, Rhode Island, Connecticut, Washington D.C., or the following countries will not be allowed back to the Lifeguard Station and participate in any activities for 16 days from the date of being back home: China, Iran, all of Europe, South Korea, or along the Nile River.

o Personnel should immediately contact the Training Officer and advise that they have been to one of the following areas so a Letter to Staff to be exempt from compulsory duty for 16 days from the date of being back home can be submitted on their behalf. The member will need to show proof on where they went to, when they left, and when they returned.

o Personnel should immediately contact their healthcare provider if they start to experience fever, difficulty breathing, and/or cough while on their 16 days of leave.

• Personnel who start to experience fever, difficulty breathing, and/or cough and have

traveled to: major “hot spots” within the United States, China, Iran, all of Europe, South Korea, or along the Nile River within the past 14 days OR possibly could have been in close contact with someone diagnosed or under investigation for COVID-19, will not be allowed back to the Lifeguard Station or participate in any activities until cleared by a doctor.

o Personnel should immediately contact their healthcare provider for an evaluation.

o Personnel should then contact either the Medical Director or Training Officer to make notification on the situation and so that a Letter to Staff to be placed on Medical Leave can be submitted on their behalf.

PATIENT CARE – The Medical Standard Operating Guidelines - COVID-19 Infection Disease Section has been updated. See attached. When performing a water related call-for-service, personnel should do all that they can to minimize the event of an exposure from a patient experiencing respiratory difficulties. In the event that the patient coughs, throws-up, or bodily fluids make contact onto your face, immediately advise your officer in-charge so they can make notification to the Prevention and Training Officer. When providing patient care, only utilize one person, designated by the officer in charge, to evaluate, treat, and transport, if necessary, patients who are experiencing: fever, flu-like symptoms, cough and respiratory difficulties, recent respiratory illness, and/or who has had contact with a known COVID-19 positive person, or has tested positive for COVID-19. Other personnel on-scene should stay at least 6’ from the patient. When performing an evaluation of all patients, a “COVID-19 Assessment” should be asked as soon as possible, when performing the initial assessment:

1. Has the patient tested positive for COVID-19? 2. Has the patient experienced fevers, recently? 3. Has the patient experienced any respiratory illness, recently? 4. Has the patient had any contact with positive COVID-19 patients? 5. Has the patient recently been evaluated / admitted to a hospital within the past 14

days? If the answer is YES to any of the questions or the patient is unable to answer these questions, then the patient is to be considered a COVID ALERT. Only utilize one rescuer, designated by the officer in charge, to evaluate, treat, and transport the patient, while wearing a N95 mask, sunglasses, and preferably a shirt or rash-guard. If the patient is in critical condition, then additional personnel can be utilized to assist with patient treatment, at the officer-in-charge’s discretion. If requesting Fire and Rescue, the officer should advise dispatch that the patient is a “SIGNAL-13 COVID ALERT”.

• RESPONDING TO CALLS-FOR-SERVICE:

o In efforts to minimize personnel exposure, personnel response to calls-for-service the following will be implemented until further notice: Response personnel will be reduced to one officer and two guards unless

the following which then a standard two officers and four guards will respond:

• The call-for-service is considered “major” and/or “life-threatening”

• Additional personnel are requested by the officer-in-charge • The call-for-service is a significant traumatic call-for-service that is

not caused by a medical illness

• TRANSPORTING PATIENTS: o If transporting a patient on the back of the truck that is a COVID ALERT only one

rescuer is to be on the back of the truck with the patient unless additional are absolutely necessary to perform proper patient care.

• MAINTAINING PROPER PERSONAL PROTECTIVE EQUIPMENT (PPE):

o All personnel providing direct patient care will wear a surgical mask and their sunglasses. Once a member is provided a surgical mask, they will keep their surgical mask in a ziplock bag to be reused, when needed. Once a member is issued a surgical mask, they should keep it on them to be used in the event that they have to provide patient contact. In the event that the mask is soiled or ruined, the member should immediately discard the mask and be reissued a new one, when needed.

o When treating a patient who is experiencing respiratory distress, respiratory arrest, or cardiac arrest, all personnel proving patient care will wear N95 mask and sunglasses. Personnel providing patient care are preferred to also be wearing a shirt, rash-guard, jacket, or gown.

• FIRST-AID ROOM USAGE:

o A COVID-19 Assessment must be performed BEFORE any patient or person is to enter the first-aid room. If a patient is a COVID ALERT or is experiencing a respiratory emergency,

the patient is not to be treated in the first-aid room. Any call-for-service that has a positive COVID ALERT or possible positive COVID ALERT should document appropriately on the patient care report under the treatment section. The officer-in-charge should also advise the Officer of the Day who then will make notification to the Training Officer. When any personnel have been exposed to a suspected infectious disease patient and/or COVID ALERT patient, the officer in-charge should immediately advise the Officer of the Day. The Officer of the Day and the officer in-charge should write a brief report on how the exposure occurred and immediately contact the Prevention and Training Officer. If no contact is able to be made, leave a message and/or a text to their cell phone and then attempt to contact the Medical Director. For any questions, comments, or clarification, please contact a VLSC Commissioned Officer, VLSC Training Officer, VLSC Medical Director, or VLSC BOD President.

Jacksonville Beach Ocean Rescue Volunteer Life Saving Corps COVID-19 Infection Control Guideline Update Updated May 18, 2020 Purpose: This is an update to the March 2020 COVID-19 Infection Control Guideline Update and is meant to provide guidance to JBOR/VLSC responders while treating patients on duty. These guidelines are meant for the treatment of all beach patrons. Initial Patient Encounter – The following treatment guidelines should be adhered to for all patients:

• If initiating a water rescue, take all efforts to utilize a rear approach and rescue • No in-water ventilations should be given • Ventilations should only be given using a bag-valve-mask (BVM)

o If it is in the opinion of the rescuer that delaying ventilations until the arrival of a BVM will greatly affect the outcome of the patient, the rescuer may use their discretion with the knowledge that this may present an increased risk of infection if the patient is infected with COVID-19

o Rescuers opting to delay ventilations until the arrival of a BVM will be practicing within the scope of JBOR/VLSC MSOGs

• A minimum number of responders should be on scene, as per other guidelines in this document

• All responders providing direct care should don, at a minimum, a surgical mask, gloves, and sunglasses. Other PPE specific to patient condition should be donned as appropriate.

• Providers wishing to utilize additional PPE from home specific to COVID-19 may do so after clearance from the Safety Officer or Medical Director

• All responders providing direct care should, when feasible based on patient condition, maintain a distance of at least 6 feet from the patient

Screening Criteria: Responder should ask the patient, if feasible based on patient condition, the following questions:

• Exposure o Have you been diagnosed with COVID-19 in the past 14 days? o Do you live with or interact closely with anyone diagnosed with COVID-19 in the

past 14 days? • Symptoms (past 72 hours)

o Temperature is or has been greater than 100.4 degrees Fahrenheit (38 Celsius) o Coughing o Shortness of breath or difficulty breathing

o Chills o Repeated shaking with chills o Muscle pain o Headache o Sore throat o New loss of taste or smell

COVID Alert – Patients meeting any of the above criteria will be deemed a “COVID Alert”

• This alert does not change management of the patient, but is for documentation and communication to responding EMS, if necessary

Procedures: • Any disposable equipment making direct contact with any patient during treatment

should be handled with gloves and disposed of immediately • Any non-disposable equipment making direct contact with any patient during treatment

should be cleaned properly. • Upon the activation of a COVID Alert, officer of the day should alert the Safety Officer

and Medical Director for further guidance or patient disposition. Disposition: Patients who are deemed a “COVID Alert” should be dispositioned as following:

• Patients who appear well with normal respirations and oxygen saturation off supplemental oxygen should be advised to return home, contact primary care physician, and seek guidance on self-quarantine.

• Patients who are deemed candidates for home isolation should not be advised to go to the emergency department, unless clinically indicated, and should not be transported by EMS. These patients may choose to go to the emergency department on their own, but this will be on their own power without EMS.

• Patients who appear sick, display tachypnea, or require supplemental oxygen should have EMS activated. Upon activating EMS, dispatch should be told that this is a “COVID Alert”. If these patients refuse EMS transport, they should be advised of the need to seek treatment at an emergency department. If patient states they will self-transport to an emergency department, enquire as to which hospital and make efforts to contact that hospital. Also advise patient to alert the triage staff that they have screened positive as a COVID alert.

BEACH

- No restrictions except for 6 foot social distancing; and - Groups smaller than 10; and - Dogs are allowed on the beach, on leash, from 5pm to 9am ONLY (Regular

hours per city ordinance). PARKING

- All public parking is open 24/7; and - Paid parking is in effect in the designated paid parking lots.

CITY PARKS - The skate park will be open, social distancing will remain in affect; - Open from sunrise to sunset; - Dog park is open; - Exercise and utilizing playground equipment allowed; and - 6 foot social distancing required.

RESTAURANTS - Must derive more than 50% gross revenue from food sales to be considered a

Reuse refers to the practice of using the same N95 respirator for multiple encounters with patients but removing it (‘doffing’) after each encounter. Respirator Reuse Recommendations There is no way of determining the maximum possible number of safe reuses for an N95 respirator as a generic number to be applied in all cases. The recommendations below are designed to provide practical advice so that N95 respirators are discarded before they become a significant risk for contact transmission or their functionality is reduced.

Discard N95 respirators following use during aerosol generating procedures (nebulizer, CPAP, or intubation).

Discard N95 respirators contaminated with blood, respiratory or nasal secretions, or other bodily fluids from patients.

Discard N95 respirators following close contact with any patient co-infected with an infectious disease requiring contact precautions.

Store respirators in a breathable container such as a paper bag between uses. To minimize potential cross-contamination, store respirators so that they do not touch each other and the person using the respirator is clearly identified. Storage containers should be disposed of or cleaned regularly.

Clean hands with soap and water or an alcohol-based hand sanitizer before and after touching or adjusting the respirator (if necessary for comfort or to maintain fit).

Avoid touching the inside of the respirator. If inadvertent contact is made with the inside of the respirator, discard the respirator and perform hand hygiene as described above.

Use a pair of clean (non-sterile) gloves when donning a used N95 respirator and performing a user seal check. Discard gloves after the N95 respirator is donned and any adjustments are made to ensure the respirator is sitting comfortably on your face with a good seal.
