* To qualify for a Young Person’s Season Ticket you must be under the age of 22 as of August 1st 2017. Juniors are under 18 as of 1st August 2017 and Seniors must be over 65 as of 1st August 2017. Please include a photocopy of your driving licence / passport with your application.

Section 4 Home Cup Tie Scheme

I would like to take advantage of the Home Cup Tie Scheme (You wish to attend every Home Cup Tie played at Craven Cottage during the 2017/18 season)

In taking advantage of the above Home Cup Tie Scheme, I authorise Fulham Football Club to charge my debit/credit card (account details below) with the relevant cost of the Home Cup Ticket for each 2017/18 Home Cup Tie as and when they occur. In signing up to the above scheme you agree that you will inform the Club of any change to the number or expiry date on the nominated card. You understand this agreement cannot be cancelled, nor a refund made in respect of any games that you do not attend for any reason.

Section 5 Donate to the Fulham FC Foundation

The Fulham FC Foundation uses the power of sport to deliver an extensive programme that engages, motivates and educates over 25,000 young people a year. This includes many disabled and disadvantaged young people in Fulham FC’s community.

I authorise my donation to be charged to the card detailed in section 6 of my renewal.

I enclose a separate cheque for my donation made payable to the Fulham FC Foundation - Registered charity number 1111639.

I am a UK taxpayer and I want the Fulham FC Foundation to treat all donations I’ve made for the six years prior to this year (but no earlier than 06/04/2000) and all donations I make from the date of this declaration as Gift Aid donations, until I notify you otherwise.
