Dear Friends,

The honor of being representatives of this extraordinary organization is both challenging and rewarding. The challenges include managing the growth of Alzheimer’s Community Care and maintaining its sustainability in an efficient and effective manner. There have certainly been challenges over this past year due to the escalating numbers of families affected by Alzheimer’s disease and related neurocognitive disorders, and the reality that this fragile population will continue to multiply for the next 10 to 15 years. While we all agree that a cure for the disease, or at least being able to slow its process, would be desirable, there is simply no known drug in the research pipeline. Thus, skilled and compassionate care is paramount for our families, both now and in the future.

With the challenges come the rewards, as evidenced in this Fiscal Year by the wonderful progress made by our Board of Directors and staff in delivering this compassionate care in the following ways:

• Advocating for increased funding to meet the needs of our families. We thank our Advocates who traveled to Tallahassee - they were heard, valued, and they accomplished what they set out to do.

• Establishing our 10th Specialized Alzheimer’s Day Care and Service Center in the Western communities, made possible by the generous financial support from the Leo and Anne Albert Charitable Trust and the James and Claudine Kirkwood Fund of the Community Foundation for Palm Beach and Martin Counties. We also express our deep gratitude to St. Michael Lutheran Church’s congregation in Wellington for opening up their hearts to our families and our vision for the Center, which has become a reality.

• Working with the Florida Department of Elder Affairs, and Broward Health North to expand the Florida Silver Alert Program, to create the Lost on Foot program, successfully launched in July 2015. The program will be rolled out in Palm Beach, Broward and Brevard counties before it goes statewide. The educational video and the Family Risk Guide have become an immediate success, and with those two tools available, the execution of the program promises to be very successful.

• Establishing our Education and Quality Assurance Department has enabled us to develop a Nursing Rotation program with five of our most prestigious institutions. This program will expand a talent pool within their nursing programs, which is critical and necessary to balance the growing needs demanded by this epidemic. The educational opportunities for implementing a robust dementia-specific focus with those institutions’ academic programs have yielded much excitement and a sense of accomplishment.

• Expanding the ID Locator Bracelet Program, which continues to save lives for patients at risk of wandering.

Within the upcoming year Alzheimer’s Community Care will open its 11th Specialized Alzheimer’s Day Care and Service Center, to be known as the Leo and Anne Albert Alzheimer’s Care and Service Center in Port St. Lucie. Again, thank you to the generous and most significantly caring support by the Foundation that allows our organization to expand its services in this most underserved community.

In closing, any donations of time and financial assistance will be warmly appreciated because they will continue to make us successful with our “family-centered” focus, and allow us to succeed in “providing a safety net for our caregivers and families every day.”

About Alzheimer’s Community CareOur organization was founded in 1996, when a group of donors, caregivers and community leaders in Florida came together in response to the growing number of local residents diagnosed with Alzheimer’s disease. There were no specialized treatment services available locally, so our founders established a dementia-specific service organization exclusively dedicated to the needs of patients and caregivers living with the daily challenges of Alzheimer’s disease and related neurocognitive disorders.

The mission of Alzheimer’s Community Care has always included a dual focus on helping both the patient and the caregiver. In this Report, we celebrate our 19th year of providing community-based services and resources, through our Fiscal Year that ended June 30, 2015.

We invite you to learn more about our organization on the pages that follow, including our Specialized Alzheimer’s Care and Service Centers in Palm Beach, Martin and St. Lucie counties, and our Family Nurse Consultants. We are proud to report that in this Fiscal Year, 91 cents of every dollar donated to Alzheimer’s Community Care went to our Services and Resources.

As we draw near to two decades of service to our community, we remain true to our Mission Statement:

With the passing of James Gregory Fragakis in 2015, Alzheimer’s Community Care wishes to pay tribute to Mr. Gregory, along with our other two Founders who preceded him in death, Mr. Gaston Jones and Mr. Fred Kroll. It was these gentlemen, along with our President and CEO, Mary Barnes, who determined that all funds raised would be used locally to provide community-based services, and that both caregivers and patients would receive the highest

quality specialized care delivered with dignity and compassion.In recognition of these visionaries, Alzheimer’s Community Care regularly bestows the

Founders Award on individuals who exemplify the character of James Gregory Fragakis, Gaston Jones and Fred Kroll.

The Board of Directors of Alzheimer’s Community Care is comprised of leaders from local businesses and organizations in the three Florida counties that we serve - Palm Beach, Martin and St. Lucie. Our Board members are steadfast in their commitment to the mission of our organization, and they provide the direction necessary to move

Our Volunteers

Our volunteers are the most treasured asset of Alzheimer’s Community Care. They are our ambassadors who give of their time, experience and various skill sets that

allow us to operate as a true community-based organization.

In Fiscal Year 2015, we were fortunate to have 307 volunteers and five senior companions who generously offered 7,920 hours of their time to our organization,

which is valued at $169,178.

Our volunteers serve on our Board, committees and advisory councils, while also helping to coordinate fundraising events, including the

annual Alzheimer’s Educational Conference.

Specialized Alzheimer’s Care and Service Centers are essential in intervening and providing respite care for caregivers while simultaneously providing dementia-specific,

community-based services for patients.

Our 10 Specialized Alzheimer’s Care and Service Centers are designed to provide the tools for Alzheimer’s patients and family caregivers to maintain their quality of life. In order to

accomplish this, patients receive comprehensive dementia-specific care, which lessens the impact of behaviors associated with Alzheimer’s disease.

Caregivers benefit with essential daily intervention that results in improved personal health, stress reduction and respite from the 24/7 burden of caring for their loved one at home. Each Care

and Service Center provides a safe, supportive and individualized environment for Alzheimer’s patients.

Physical exercise, socialization, cognitive stimulation and medical monitoring are some of the benefits that patients receive. Alzheimer’s Community Care administers eight dementia-specific centers in Palm Beach County, one in Martin County and one in St. Lucie County. The Care and

Service Centers maintain a 5:1 patient-to-staff ratio and are licensed by the Agency for Health Care Administration.

Family Nurse Consultants

The Family Nurse Consultant (FNC) Program serves as a lifeline for families in Palm Beach, Martin and St. Lucie counties who are living with Alzheimer’s disease and related

neurocognitive disorders. Our FNCs provide caregivers with the support, tools and resources needed to continue providing home-based

care for their loved ones.

Through the program, our nurses are specially trained in treating dementia and provide support 24/7, 365 days a year. Our FNCs provide interventions, home visits, needs

assessments and care plans designed to meet the needs of the patient and family caregiver throughout all stages of the disease.

These interventions have proven to reduce caregiver stress, and delay or eliminate the need for nursing home placement. The FNC program also helps reduce physician office and emergency room visits. FNC services are provided without charge to Alzheimer’s

patients and caregivers and are entirely supported by donations and grants.

In Fiscal Year 2015, a total of 2,548 patients and caregivers served by our FNCs were able to continue living at home with family members rather than be

admitted to nursing homes.

24-Hour Crisis Lane

Our 24-Hour Alzheimer’s Crisis Line provides caregivers with understanding and guidance that can help them throughout the many stages of Alzheimer’s

disease and other related neurocognitive disorders.

The Crisis Line serves Palm Beach, Martin and St. Lucie counties. Family Nurse Consultants (FNCs) respond to calls and can connect directly to law

enforcement for assistance during search and rescues.

The Crisis Line is also an excellent crisis management resource for caregivers. In our Fiscal Year 2014-15, our FNCs received a total of 134 calls for

assistance and referrals from family caregivers. In all cases, the on-call FNC responded to the inquiry, followed-up as appropriate, and linked the

family to essential services and resources.

Our Education and Quality Assurance Department, which was established during this Fiscal Year, provides: · dementia-specific education and training· training in regulatory compliance· staff orientation and training· policy and procedure development and maintenance· and oversight of special projects

The Department monitors and maintains ongoing compliance with the organization’s quality, performance, and state and regulatory standards. In this Fiscal Year, the Education and Quality Assurance Department standardized the orientation of Alzheimer’s Community Care’s clinical staff, which includes Family Nurse Consultants, Program Managers, Program Nurses, Program Assistants and Activity Coordinators.

New staff members are also required to demonstrate education and competency in their first days of employment, and in compliance with the latest, high standards of our accrediting body, The Joint Commission. Alzheimer’s Community Care also developed a plan for enhancing the education of local Registered Nursing students by establishing procedures for providing clinical experiences for these future nurses.

The initial plan provided for clinical rotations for students of the Palm Beach Atlantic University Bachelor of Science in Nursing Program. Future plans include extending the program to additional area universities. The Education and Quality Assurance Department also expanded education in the community, providing numerous disease-specific educational programs to community groups, law enforcement, emergency first responders, and related health care providers.

Case Management

Our Case Managers are often called the gatekeepers at Alzheimer’s Community Care. To a family in need, the Case Manager is much more - they are their advocate and support system.

Case Managers work with families with financial need to determine their eligibility for funding Whether this funding is provided by the state, local government entities or through private funds

secured by Alzheimer’s Community Care, these funds are critical to protect the well being of the patients and caregivers.

Alzheimer’s Community Care’s Case Managers work closely with the family, determine their individual needs and bring them together with other agencies that provide additional services

and resources. They also provide much-needed guidance in helping families and caregivers successfully apply for financial assistance through government entities.

ID Locator Bracelet Program

Our ID Locator Bracelet program provides an additional safety net tool for patients with

dementia who may wander away from safe and supervised settings.

This program partners with SafetyNet by LoJack and local law enforcement to assist the

wandering patient and their family. Our pilot project provided 75 bracelets for patients at risk of wandering. An additional 75 bracelets were made available in Fiscal Year 2015 thanks to

generous support from The Jim Moran Foundation.

This is a vital program, as 125,000 persons with Alzheimer’s disease or related neurocognitive disorders wander away from safety each year.

Prompt location of the patient is essential because 80 percent of those lost for more than 72 hours

will not survive the event.

Every year, Alzheimer’s Community Care conducts a series of special events that are intended to further educate the public about issues related to Alzheimer’s disease and related disorders, as well as raise the

funds necessary to further our mission.

The 18th Alzheimer’s Educational Conference, held in the spring of 2015, brought together approximately 1,000 caregivers and professionals for two days of discussions and presentations.

The Palm Beach Luncheon, our signature fundraising event, raised more than $30,000 for the services and resources we provide the community. Alzheimer’s Community Care also advocates on behalf of our

patients and caregivers with an annual trip to the state capitol in Tallahassee.
