Find a Future produces a range of published materials and online information. We take photographic/video images and use case studies of subjects in order to enhance and illustrate our materials to raise awareness of our products and services.

This consent form covers any use of the images and personal details (such as first name) by Find a Future.

Legal clauses

1. Signing this form gives consent for the individual/venue to be included in photographic/video images for use by Find a Future, thereby giving consent to the full use of material by Find a Future, and its approved contractors and organisations in any of its media applications, which reasonably promote or advertise the services of Find a Future.

2. The images and personal details will not be used for any other purposes than those highlighted above. You agree to notify the Find a Future before speaking to the press/broadcast media concerning this case study and photographic/video recording.

3. No personal data of individuals under 18 shall be disclosed or used. 4. The copyright of any materials generated as a result of this photographic/video

recording shall rest with Find a Future. 5. Find a Future retains the right to correct images (i) if it is felt necessary to protect the

anonymity of the individuals captured in the images or (ii) to avoid endorsing other brands.

6. The Data Protection Act stipulates that you are entitled to withdraw your consent at any time in the future should you wish to. You may do this by sending an email to kmoloughney@findafuture.org.uk
