Warfarin Key Points• Not listed at top of MAR as per current paper

chart• Needs to be ordered every day- are once only

orders and will fall off MAR after administered– Dr to review INR and place order daily– Warfarin reminder is on the chart to flag that a new

order needs to be written each day after reviewing INR

• Do NOT sign off on warfarin reminder until prescriber has reviewed/charted next dose

through powerplans– Warfarin initiation powerplan– Warfarin recommencement powerplan

• Warfarin when is ordered outside the powerplan it automatically asks if an INR pathology order is needed.

Marevan) it will ask to be substituted to the brand used in hospital (Coumadin)

• If the prescriber wants Marevanspecifically they need to cancel the order and place a new order searching for Marevan by brand

to confirm the last dose before prescribing

this medication will also display in related results for the prescriber to see at point of ordering ie Clozapine/Norclozapine level, WBC, Neutrophils etc.
