• Approximately 7.4% of Tennessee’s seniordrivers have been involved in some typeof fatal or injury crash.
• Anyone can be in a crash or havea medical emergency.
• The Program is a simpleway to make sure you are prepared.

How to use theProgram
Place the Photo on MedicalInformation FormPlace the photograph in the space indicated onthe emergency medical information form. Thephotograph should be a clear, recent photo ofjust the Yellow DOT participant.
Attach DecalPlace the decal outside on theleft side of the rear-facing window. This willalert first responders to look in the glovecompartment for your emergency medicalinformation.

Complete Medical FormComplete the emergency medical form, includingthe participant’s name, emergency contacts,personal physician, medical conditions, recentsurgeries and medications.

Place Informationin Your GloveCompartmentThe final step is to place themedical information form with photoattached in the folder andplace folder in your glove compartment wheretrained first responders will look when theysee a on the rear window.
