If you do not take action during your new hire enrollment window, you will not be able to make any changes to your benefits until the next Annual Enrollment (for the 2021 plan year) unless you have a Qualifying Life Event.
As long as you enroll in benefits within 31 days of your hire date, your benefits will be effective on the 1st of the month following 60 days. You are eligible for benefits if you are a full time employee, working at least 30 hours per week.
Aetna is our medical insurance provider. With Aetna, you can see any in-network provider at any time, no referrals needed. We are also introducing a third medical plan to our offering, providing cost efficient access to high-quality doctors, hospitals and specialists.

ACTION REQUIREDYou are required to enroll in or decline coverage. If you do not enroll in
coverage within 31 days of your hire date, your next opportunity to enroll in coverage will be January 2021, unless you have a Qualified Life Event.

With our online tools, you can:• Earn up to $600 in your HSA plan for participating• Learn about your current health status through a health evaluation and health assessment. • Receive a detailed report which includes your Personal Health Score, Goal and Action Plan. • Complete a Health Risk Assessment or Preventive Screening
Your health plan is committed to helping you achieve your best health. Rewards for participating in a wellness program are available to all employees. If you think you might be unable to complete the Health Risk document or participate in the preventative screening and biometric screening under this wellness program, you might qualify for an opportunity to earn the same reward by a different means. Contact Manna at 760-944-1070, ext. 220. We will work with you (and, if you wish, with your doctor) to develop another way to qualify for the reward.
Note: If you receive the Manna wellness $600 credit in your HSA account, your annual allowance will be reduced by that amount.

Spousal Surcharge
If you enroll your spouse in medical coverage, you will be asked to certify whether he or she is eligible for coverage through their employer. If your spouse is eligible for other employer-sponsored coverage – even if he or she does not elect to enroll in the other coverage– you will pay an additional spousal surcharge of $46.16 per pay period to cover them under a Manna medical plan. The surcharge is paid on a pre-tax basis and does not apply if your spouse also works for Manna Development.
We offer you two great dental plan options through Premier Access, a Base and Buy-Up plan. With the Buy-Up DPPO plan, you can get cleanings up to 3 times per year and have access to orthodontia coverage for both yourself and your dependents!

Manna contributes up to $600 on your behalf each year up if you participate in our Wellness program. You can contribute up to $3,550 annually as an individual or $7,100 for a family. Funds roll over from year-to-year!
Contribute up to $5,000 annually to pay for services in a daycare cen-ter or services for providing care for a qualified dependent. You must use your Flexible Spending Account (FSA) funds in 2020.
If you or a family member have been diagnosed with diabetes or hypertension, you qualify for our new Livongo program. This program includes coaching, a glucose meter and unlimited lancets and test strips - all at no cost to you!
The Progyny benefit is designed to assist covered employees and their partners in all paths to family building. In addition to a comprehensive fertility treatment benefit, Progyny also offers support and navigation of adoption and surrogacy for patients that do not progress to fertility treatments.

PRESCRIPTION DRUG COVERAGEWhen you enroll in medical coverage, you automatically receive prescription drug coverage. MedImpact prescription drug carrier. MedImpact will offer a great lineup of convenient ways to fill prescriptions:• Fill 30-day prescriptions at retail pharmacies• To save money, buy 90-day supplies of most medications:
- At retail pharmacies (excluding Walgreen’s), or- Through MedImpact’s mail order service
Our medical plans are In-Network Only plans. That means you must visit an In-Network provider to receive medical coverage. To find an Aetna provider:

Under “Aetna Premier Care Network Plus” Select “2020 (APCN Plus) Open Access Aetna Select”5. Click “Medical Doctors & Specialists”6.
