26. Each day before use, the license shall perform visual and operational checks of equipment refer application and letter dated November 17, 2000, conditions 21 and 22 to ensure that:

A. the equipment is in good working condition; B. required labeling is present and legible; and C. survey instrumentation is responding to radiation.

If any inspection reveals damage to labeling or components critical to radiation safety, the equipment shall be removed from service at the time the damage is discovered and until repairs have been made. Records of equipment inspection, maintenance and problems shall include unique identification of equipment labels, apparent condition, repair or replacement notations, date of inspection and signature. Rec required by this condition shall be maintained for inspection by the Agency for three years.

A. Prior to conducting licensed activities,. radiation surveys shall be made and recorded for each where radioactive materials are used.

B. If the licensee has reason to believe that a sealed source of radiation has become damaged or is le the licensee shall immediately conduct a radiation survey, including a contamination survey, during an after the completion of licensed activities.
