• The pupil was eligible to receive high school credit for

participation in the WBLE.

• Written training agreement in place on count day included:

• Pupil’s personal information, including name, home address,

telephone number, date of birth, and emergency contact

information.

• School’s name, address, telephone number, and contact person.

• Employer’s name, address, telephone number, and contact person.

• The responsibilities of employer, school, and pupil.

• Beginning and ending dates of agreement.

• Daily hours to be worked, including beginning and ending times.

• Beginning rate of pay, if paid work-based learning experience.

• Appropriate safety instruction was provided.

• Employer has worker’s disability compensation and general liability

insurance.

• Required signatures.

• Nondiscrimination statements of assurance.

• Written training plan in place on count day included:

• Verification by the certified teacher that the pupil’s career or

education goals as outlined in his or her education development

plan relate to the placement.

• A list of performance elements that contribute to the pupil’s

progress toward a career objective.

• For WBLE Non-CTE programs, identification of the academic

courses that generate credit toward a high school diploma in

which the pupil is currently enrolled or was previously enrolled

that relates to and prepares the pupil for job placement.

• Required signatures.

• Employer or certified teacher/coordinator maintained and verified

records of the pupil’s attendance throughout the duration of the

training agreement.

• Safety instruction appropriate to the placement was provided by

the district and documented in either the training plan or training

agreement.

• WBLE did not generate more than one-half (.50) of the pupil’s FTE.

• Employment did not exceed the maximum hours set by the

district.

• The pupil met grade eligibility.

• Regular visitation plan is documented.

• Monitored by certified instructors.

• Additional requirements in Pupil Accounting Manual are met.
