I-9 FORMS (electronic)

• The I-9 form must be completed before you can be hired

• It is electronic – must be completed with one of our HR reps in the lobby

• AFTER YOUR DRUG TEST, stop at the I-9 table and complete your

electronic I-9

• EXCEPT—during the Sleep Deprivation session—you must be in the

meeting room and sign in (1:20 PM)

• There will be several people available to help; there will be plenty of

time for all of you. The I-9 people will not leave until everyone has

completed.

• TAKE YOUR DL AND SOCIAL SECURITY CARD (or passport)

• J-1’s: I-94 card, passport, DS-2019

• REMEMBER, if you do not complete the I-9 electronic form, you will not be hired!

Long Distance Calling Number

• You are issued a personal number for long distance patient-related calls only

• You need this number to call from patient-care areas

• Charges are monitored

• Personal calls are not allowed

• If you need to do this, call the Hospital Operator for your long distance access number

UCard Office

• You should have already had your picture taken.

• Location: SOM A level (between B & C elevators).

• Open until 7:00 PM.

• You cannot get your ID card without the blue

authorization sheet.

• Pick up your blue sheet (or your finished card) as

you leave Orientation today. The UCard Office

will not issue ID cards! GME needs them to get

the numbers. Get them from GME tomorrow!

Rotating at Other Training Sites:What Nametag Do I Wear?

1. University of Utah ID card at all University of Utah Hospital or Hospital-owned facilities

2. Other Hospital ID cards when there

3. University of Utah ID card when rotating off-site on an approved rotation that is non-hospital

4. Always wear a name tag

Free Transit Pass

• Your U ID Card is programmed to be used as a UTA free pass for riding UTA buses or TRAX.

• The programming is automatic, but may take 2-3 days (up to a week) to be activated.

• Problems? UCard Office – 581-5955

Commuter Services• Commuter Services will be available at the lunch break to

get you signed up for a parking permit—all electronic.

• You need an “A” permit. With an “A” permit, residents may park in the South Terrace, top level ONLY. If you try to park in any other terrace, or any other level of the South Terrace, you will be ticketed.

• An “A” permit gets you the closest surface lots.

• The Commuter Services table will have parking maps.

• If you park where you are not allowed, you will get ticketed.

• If you don’t resolve your tickets, your vehicle will be booted and/or towed.

• Commuter Services needs your license plate number(s). When you register your plate number(s) online, the cost will be payroll deducted.

• Pick up pager at Pager table during lunch break—or register your

Smart phone for Mobile Connect. Pager Services will explain

Mobile Connect and have sign-up info at table.

• No pager numbers assigned. All paging through Smart Web

(more info during presentation and flyer in folder).

• If you had a pager previously and notified the GME Office, you

keep the same pager – DON’T PICK UP A NEW ONE

• No deposit--$35 replacement if lost/stolen/broken

• DON’T PICK UP A PAGER

• New licensing requirement this year!• 4-hour CME course on prescribing opioids• License renewal only• LOOK AT YOUR LICENSE—what is the expiration date?

• January 31, 2016—MUST take opioid course to renew• After January 31, 2016—not applicable until renewal

OPTIONS:• UMA online course--$30—find at www.utahmed.org• U of U working on an online course• Not available yet—no course fee info yet• Hopefully available this fall—GME will send out email
