Who can use the center?All Perrigo employees and interns. Spouses and
dependent children (twelve years of age and older) who
are enrolled in a Perrigo medical plan. Perrigo Medical
Center (PMC) is conveniently located next to the Perrigo
Security Office.

Premise Health and is staffed by qualified healthcare
professionals that know you, the jobs you perform, and
the healthcare needs of you and your family. Everyone
on your center’s team has been fully screened and
selected by Premise Health for their leading-edge
knowledge and skills.
We encourage you to choose the Perrigo Medical Center
(PMC) as your Primary Care Provider. We will work
alongside your current medical team, and if you ever
need a referral to a specialist, we’re right here for you.
Your privacy is our priority.We take your privacy seriously. Premise Health operates
the Perrigo Medical Center (PMC) and maintains all of
your records, and follows the same state and federal
laws as any healthcare organization. Premise Health
can’t share any information without your permission,
unless it pertains directly to a workplace injury or illness.
We believe being healthy is about more than stopping the sniffles. It’s about being at your best, every day – and knowing where to turn when you don’t feel quite right.
That’s why Perrigo has partnered with Premise Health to operate the Perrigo Medical Center (PMC). If you are a Perrigo employee or intern, you’re all set – just make an appointment. The PMC is also available to spouses and dependent children (twelve years of age and older) who are enrolled in a Perrigo medical plan.

No more running around town for checkups or tests. No more scrambling for an appointment when you’re sick. The Perrigo Medical Center (PMC) will handle it all. PMC charges a $10 copay for those enrolled in PPO and those not enrolled in a Perrigo medical plan, which can be paid through payroll deduction. Those enrolled in the CDH plan; claims submitted to BCBSM and they are billed for applicable deductible and coinsurance.

Better health, online.Visit perrigomedicalcenter.goandbewell.com to view your Health Portal. To register you will need your primary email address (corporate or personal), employee ID, and date of birth. Upon completion you will have access to your health records, visit history and test results. It’s also a convenient way for you to make appointments, request medication refills, complete new patient forms, email providers and more.Mobile Access: • To download the app - search “Premise Health”• In the patient portal, get your mobile activation
