National Partnership to Improve Dementia Care
In 2012, CMS launched the National Partnership to Improve
Dementia Care.
As a nation to reduce use by: WI US

Phase II: Six month Implementation Survey
At the beginning of July 2015, Phase II participants received a six month
implementation survey to find out how the program is going, and if we
can help in any way.
Completion of the survey is mandatory as part of your facility’s
agreement to participate in evaluations.
We have designed it so it is easy to complete. The submission date has
been extended to August 13th.
Upon completion of the survey, we will send you a laminated door sign
proclaiming that your nursing home is a Music & Memory certified
facility.
