Step 1 – Review, Change, Add

REVIEW

This year, your current benefit elections will be populated in our Enrollment Module.

Please review your existing 2015 -2016 elections, covered dependents and cost.

Note: Click on the Toggle full screen viewing mode in the top-right side of your inbox action to view the full-screen. If you do not wish to make any changes to your current elections, click Continue to move through the screens to the final

Submit option.

CHANGE

If you wish to change from family to individual coverage in your health plan for 2016, select the prompt for new coverage type

Medical, dental and vision enrollment. The choices that you have are each listed and you will need to decide if you want the premiums pretax or

post tax, please review DCSD’s Benefit Summary to determine which is best for you.

Once you review the Benefit Summary and know what plans you would like to select, you will click on elect, select the Coverage and if enrolling

dependents you will need to add your dependents. It will then list the Employee Cost and Employer Contribution. The Medical-Cigna/Allegiance

HDHP Post Tax was elected and when you click on the icon for Coverage, you will need to select the coverage that you need.

Employee plus Spouse was elected. If you’re dependents are not listed, you will need to create them.

Click on Create and then click on Add Dependent.

If you have an Existing Beneficiary or Emergency Contact, you can click on the Icon and add that person as a dependent or you can click on New

Dependent and click OK at the bottom of the page. If you want to use this dependent as a beneficiary on your life insurance, you will need to check

the box for Use as Beneficiary.

You will need to add their name, click on Contact Information and you only need to add the Address.

If you should click on any of the others such as Phone and you don’t want to add the information, click on the minus sign to get out of that screen.

You will also need to add their National ID. You will click on the National ID tab and then click on the + icon.

You will need to add the Country, Type and Identification #. Then click on OK at the bottom of the screen. Once this is completed they will be added

to your medical plan. You will need to complete this process for each dependent you are adding to your benefits.

Once the dependents are added you do not need to add them again when enrolling them in the dental or vision plans

You can use the Use Existing Address if the dependent has the same address as yourself or another dependent. Otherwise, you will need to add

the address.

You will also need to add the Usage Type. Once this is completed, you will click OK at the bottom of the screen and they will be added to the

benefit.

Once you have made your medical, dental and vision elections, you will click on Continue at the bottom of the page and this will take you to Step 2.

Step 2

If you did not add the National ID in the previous step, you will now need to complete this step. If it is not available, you will need to put the reason

why. Click on Continue at the bottom of the page to take you to Step 3.

Step 3

If you are enrolled in the HDHP plans and eligible for an HSA and you wish to receive the employer contribution, you will need to elect the HSA.

If you don’t want to contribute to it, you can put in 0 for your contributions. Click on Continue at the bottom of the page to take you to Step 4.

This provides you information about Life Insurance and Disability Coverage provided by DCSD. You also have the option to elect additional

Voluntary Life Coverage. Click on Continue at the bottom on the page to take you to Step 6.

If you elect life insurance over the Guaranteed Issue amount, you will need to complete the Evidence of Insurance form which will be sent to you.

You must designate a beneficiary to your life insurance to continue your benefit elections, if you stop at this step, you will not have benefits. To add

a beneficiary, you will need to click on the + icon.

If you have a beneficiary person already added, then click on Beneficiary Persons and add them and this also is the same for Trusts. If you don’t

have a beneficiary added, you will need to create one. Click on Create

Click on Add Beneficiary, you will only need to add the relationship, name, address and usage.

The Primary Beneficiary and Percentage are required for each life insurance benefit option that you have. For spouse life insurance, you are

automatically the beneficiary. The Contingent Beneficiary and Percentage are not required.

To add another beneficiary, you will click on the + icon and go through the same steps as above. If you clicked on the + icon and decide not to add

another beneficiary, you will need to click on the – icon. Once completed click on Continue at the bottom on the page to take you to Step 8.

This is a Summary of the Benefits you elected.

If you elected life insurance that requires an Evidence of Insurability, you will receive a task in Workday Inbox.

Please review your benefit elections, the benefits that you waived, and your beneficiaries. After reviewing, you will need to click on the I Agree, this is your Electronic Signature, click submit. We encourage you to Print your benefit elections. Then click Done to complete your benefit elections.

After electing benefits you have the opportunity to review your enrollment or make changes during the open enrollment

window.

Your Benefits Worklet will have an exclamation point (this will be present during the open enrollment window MAY 5th – MAY 27th).

You may click on the Benefits icon on the homepage and open the Change Open Enrollment event and follow the same steps. This option will be

available during the Open Enrollment Window.
