Triple Aim: improving experience, health and cost
• Population health management – Addressing biggest breakdowns in care process
• Demographic shift – 2043 projections: Minorities to majorities
– WA ranked with both largest LEP population and growth
• Increased coverage – Racial and ethnic populations disproportionately represented in
Medicaid expansion
• Organizational strategy – Move towards pay-for-performance contracts, new
accreditation requirements, etc.

A call to action
• The sooner we address disparities in care, the better we will be positioned in the rapidly changing landscape of population health management and shifting demographics.
• Rather than creating a new initiative, successful strategies to improve disparities often involve incorporating a disparities-in-care “lens” throughout the organization.
• The Alliance hopes that these findings will provide a call to action to effective quality improvement efforts to improve the health of all the people in our state.
