Reaching adulthood The first step

From the age of 16 we will support you to start thinking about your journey to independence. Once you turn 18 you will have lots of important decisions to make.

Where will you live when you can no longer stay with your foster carer or in your children’s home? How will you earn money to pay your rent, bills and to do the things you enjoy? Who will support you when you need help?

To help you answer some of these questions your social worker or leaving care worker will help you to fill out this booklet. This is your pathway plan. You can fill it out, keep it and look at it again with your worker when you need to.

Your leaving care worker or social worker is .....................................................................................

5The purpose of this plan is to help you make the most of all the opportunities and support available to you.

This is important so that you can enjoy learning and feel confident. We want to make sure that when you leave our service you can be your own person and will be able to do the things that interest you. At the back of this booklet you will find a star like the one below for you to complete each time you revisit this plan. It will keep us focused on the areas we need to work on to ensure things keep improving for you.

Your relationshipsBeing connected to trusted adults and supportive friends is key in establishing a good circle of support. Those connections will help ensure you grow and achieve in your community.

This scale is about feeling connected and having people to support you when you need it. These might be family, friends, local people, people from your cultural or religious community or people you share interest with. Mark on the scale (right) to show how connected you feel to the people in your life and community.

5 I mostly feel supported and connected. I have friends or other people outside the service who are good for me, so I don’t need professional support in this area.

4 I am getting to know new people or spending more time with people who are good for me, but I need support from workers too.

3 I try to spend time with people outside the service who are good for me but it’s difficult. People knock me back or I miss my old friends.

2 At least one worker in the service is helping me and I feel supported by them. My friends or family are mostly not helpful..

1 You are isolated but don’t want to talk about it.

4 I look after my health well enough in some ways but there are areas where I need reminders or support from the service.

3 I am trying to look after my health better, such as by going to the doctor if I am ill or eating more healthy food. It’s difficult and I often slip back into old habits.

2 I am harming or risking my health but know this needs to change and I am talking to someone about it.

1 I am harming or risking my health by not getting treatment for physical or mental health problems, sexual risk-taking or not eating well enough.

Your choices and behavioursWe think it is important for you to have the opportunity to participate socially in the pursuit of your talents and interests. Regularly being sociable with others and taking part in physical activity helps to reduce or avoid depression and anxiety. We want you to feel healthy, connected, safe and secure in your daily life. If you are experiencing circumstances that involve a high level of risk your worker must complete a full risk assessment.

12 There may already have been times when you have run out of money before you’ve received your wage or benefit and had to borrow from others or manage without. This is why it is really important to plan how you will manage the money you have, how you will stretch your budget and how you might save for the future.

Do you currently have a:

Child Trust Fund Savings Fund Pre paid card Bank account Savings account

If you don’t have any of these then your worker can help you to set one up.

13When you begin to live independently you will need to think about practical things like

• looking after yourself and staying safe

• keeping your own environment to a good standard of hygiene including cleaning, laundry, and household tasks

• cooking and shopping for healthy nutritious food

• contacting agencies

• making your own health appointments

• attending meetings/appointments on time

• reporting repairs and complaints

• knowing how to access local facilities, and act as a responsible member of the community

• asking for support when needed
