OBSERVATIONSi
1. The bridge is a pre-stressed concrete bridge having 7 spans of 36.0 mtr. eacnwith 4 girders & 5 diaphram in each span. The bridge is completed except forcomponents of crash railing, expansion joint, wearing coat and approaches,work of which are going on. The provisional extension for 2 months havebeen given by the Executive Engineer and work is slated for completion by30.10.2017.
2. The whole range of concrete grades have been used. M25 in foundation ofpiers, M30 in foundation of abutment, M4O in sub structure upto bearing cap,M45 in superstructore girders & diaphragm and M35 in deck slab.
3. The mix design has been got done from private laboratory house which is
required to be approved by Employer/Engineer. The following ratios ofingredients of concrete are requjred to be used as per design which is
mentioned below. High quality of e,nforcement of quality control is requiredas rich mix has been used.

6. Fe500 grade of steel conforming to lS:1786 is being used, test certificate ofwhich was also not available at site which is also required to be recorded in
quality control register.
7. Since the cement is being arranged by the contractor, it is necessary thatindependent testing of cement for every consignment should be ensured.
8. POT and PTFE bearings are being used, test certificate of which is required tobe obtained and recorded in quality control register.
9. Honeycombing has been noticed in diaph.agm and railings which is required
to be rectified.
10. lt is found that the approach is being constructed in long length by providrng
gabion structure and doing the earth filling. lt is advised to ensure thatgabion structures are properly designed and filling and compaction is done as
per the specification mentioned in the agreement. The gabion provided in
the approaches needs improvement from workmanship point of view as theyare not found in line and plumb.
11. lt was found that two electric poles of H.T. line have been raised on gabion
wall at two locations and there is every chance of its damage due tosettlement as they have not been raised from the firm strata. lt is advised toraise these poles from the firm strata outside the gabion wall to avoid thepossible damage.
The field officers are advised to ensure the compliance on abovepoints and follow the quality assurance mechanism properly so that desiredquality is obtained.

. OBSERVATIONS:-

The bridge is a pre-stressed concrete bridge having 11 spans of 36.0 mtr.
each with 4 girders & 5 diaphram. The work was required to be completed
by 30.08.2017, however, the progress of the work is not satisfactory. The
provisional time extension has been given by the Executive Engineer upto
31.12.2017 but it seems that with the present pace of work it is unlikely to be
compleled by 37.72.2077. The main reason for delay as explained by the
Executive Engineer and the contractor are late handing over of land and
problem in well sinking encountered due to hard strata.
It was noticed at site that there is some problem as sinking in cutting edge
being used does not seem to be designed for the strata encountered at site.
There seems to be some problem with the soil investigation done eadier
which is not showing the actual strata as encountered at site. The piers
already sunk have also tilted and they are required to be set right. The
problem need to be settled by the employer as the cutting edge being used
does not seem to be suitable and is difficult to replace at this stage when part
sinking has already been done. ln case, the issue is not settled the project
may witness long delay with added lmplications.
3. The whole range of concrete grades have been used. M25 in foundation ofpiers, M30 jn foundatlon of abutment, M40 in sub structure upto bearing cap,
M45 in superstructure girders & diaphragm and M35 in deck slab.

11. Since lot of the work project remains to be done, it is advised that for timelycompletion of the project, a resource based working programme is obtainedfrom the contractor and target fixed should be closely rnonitored.

ll. OBSERVATIONST
A) Generalr
This is a pre-stressed concrete bridge constructed and completed about 43years back. The bridge has 3 spans of 58.0 mtr. each with 2 girders and diaphragmsacross the girders. lt has 2 abutments on Renuka side and Dadahu side respectivelyand 2 piers in between the abutments. The overall condition of the bridge is good.However, the following pojnts noticed at site need consideration.
B) Sub Structure :-
{i) Both abutments are resting on hard strata and there is no sign of djstress onboth the abutments.

The bed block on Dadahu side needs protection as water is flowing from thesides which may endanger the abutment.Retaining wall/Return wall of Dadahu side needs to be repaired as plasterhas peeled off and if timely repair is not done it may further deteriorate andoamage the structure.
The neoprene bearings (8 layers) have been provided which have not beenrepaired/ replaced since completion of the brjde. The bearing life generajlyis not more than 25 years and needs to be thoroughly examined by theexpert as some deflection is also noticed in the super structure when theheavy vehicles passes over it.
C) Super Structure & Approachesr
(i) The bridge is a single lane with 3.278 mtr. Carriageway and additional 1mfoot path on both sides.
(ii) The appfoaches on both sides are in ascending grade and surface waterfrom approaches is flowing into the bridge. The same is required to bechannelized safety towards the river. Cross drain is requjred to beconstructed on both sides just before the approach slab which should becovered with grating and will help the surface water to drain offthe roadinto the bridge by channelizing the same.
(iii) On Dadahu side, the water from nallah js flowing towards the bridge deckand sides of abutment. This water is hitting the abutment sides ano neeosproper channelization as it may erode the hill and cause damage to theabutment,
(iv) The expans;on joints at all the four locations are damaged and needsreplacement.
(v) R.C.C. railing of the bridge is damaged at few locations and they are alsorequired to be repaired urgently.
(vi) lt was found that water supply pipeline is passing through the bridge betowthe footpath which is leaking and water coming down to the abutment sidesand eroding the hill. The same is required to be rectified immediately.
(vii) The drainage spouts are existing but gratings are missing which are alsorequired to be provided.
(viii) Lot of weeds and grass have grown along the railing and sides of deck slabwhich should be routinely cleared.
(ix) Telephone cable has been laid over the footpath and is covered with C.C. insome portion which is damaged. The same should be repaired.

(xi) The routine white washing and painting ofthe bridge should be done atleastonce in a Vear.
As such it is recommended to ensure that above points areattended to immediately and it should be ensured that inspection of bridge is
routinely done by the Executive Engineer twice in a year and by Senior Officers(Atleast Superintending Engineerl and mechanical wing once in a year.

ll. OBSERVATIONS:-
This is a prestigious building work which is not likely to be completed by the stipirlated
date as lot of work of finishing is left. In fact, Aiditorium roof work is yet to be
completed and then after other finishing work like ceiling which is a typical one
involving construction of catwalks will start. As per the discussion held at site unilateral
extension of time of contract for one month upto 31.10.2017 has been granted which
in my view is not sufficient to complete the work. The Chief Engineer / Superintending
Engineer concerned need to closely monitor the project.
For completing such prestigious work in time a strict contact management system in
conformjty wjth the agreement provision are required to be followed. There seems to
be no working programme which cou d be monitored vis-a- vis the targets fixed. As
such it is advised that a realistic working programme is given by the contractor in
consultation with Employer/Engineer whlch should be resource based so that proper
contract management and target fixed are monitored and achieved. This is allthe more

important as all the drawings of finishing are said to be available and there can be no
reason for anY delaY.
The Superintending Engineer concerned and Executive Engineer intimated that work rs
likely to be deviated. Necessary approval of item and deviation are also requlred to be
expedited.
As per the structural design M-20 grade of concrete mix is being used which in my
opinion should have been of richer grade to economise the structure The design mix
has been got done through Private Agency but has not approved by Employer/Fngineer'

It has been found that field lab. ls not established at site which in my opinion should
have been kept at site to ensure the testing of material at site itself The tests
conducted are at Circle laboratory by sending the samples to the laboratory lt is
advised to establish proper laboratory at site and ensure that tests like workability,
cube strength and physical properties of aggregate are got done at site itself The tests
like workability of concrete cannot be done at a place away from the site as it is going
to give misleading results. The tests conducted at site should be properly documented
and inspecting officers should also exercise a check while on visit to the site
Rain water harvesting is required to be designed properly keeping in view its
requirement as the same is proposed to be used for flushing of toilets.
Sanitary P.V.C. pipes are being used in the toilets instead to C l. pipes, however,
com plete specifications of the same were not available at the site which is required to
be obtained from the office of Architect in Chief and ensure the use of the same as per
speclfication.
8. lt was found that P.V.C. pipes are laid below the soffit without depressing the slab
which will have to be covered with ceiling However, it was noticed that door bends
were not being provided which may cause problern in case of blockage of pipes lt is
recommended to examine this aspect in consultation with Archltect-in Chief
9. The concrete balcony laid in Auditorium is not in proper curvature ltisadvisedto
improve it to conform to the curvature shown in the Architectural Drawing
10. Concrete work at few places is showing honeycombing thereby reflecting on the bad
quaJity of work. lt is recommended to correct such deficiency immediately

tt. OBSERVATIONS:-
A) General:-
(i) This is a single lane carriageway stiffened suspension bridge over river
Chenab {Chandra Bhaga) on link road to village Salpat | & ll.(ii) The bridge is having two abutnlents and four towers on which eight ropes
of 58 mm diameter each have been provided.
(iii) The bridge is designed for IRC class- B loading and accordingly
information board is required to be fixed on both sides specifying the
load limit of plying vehicle.
B) 5ub Structure and Foundationr
(i) The abutments on both sides have been founded on non.scour
consideration having safe bearing capacity of 40.00 Tonne/m'z. No sign ofcracking has been noticed. However, spalling has occurred on Udaipur
side abutment where skin reinforcement is exposed and needsjmmediate repair.
{ii) On Salpat side protection provided to the abutment in the shape of
Babion is damaged which is required to be re done.

- C) Super Structure:-
{i) The truss of the bridge is riveted whereas suspenders are bolted but it
has been found that painting has not been done since the completion of
the bridge. As a result, rusting has been noticed at a few places The
greasing of rollers and ropes are also required'
(ii) The bridge is required to be inspected by Mechanical wing atleast once In
a year and submit their inspection report with regard to bridge member
connection, anchorage systems and any other aspect requiring attention
of field functionarie ,
(iii) lt has been noticed that there is no proper drainage spout which could
facilitate the easy drain out of water from the deck This needs to be
Provided(iv) Water from approaches is draining into the bridge deck frorn both sides
On Udiapur side approach level can be lowered thereby preventing the
water drainage into the deck However, on Salpat side approach cannot
be lowered as road is in upgrade As such best way is to drain oLlt the
approach water through drain pipe of proper size so that water gushrng
into the deck is diverted.
(v) The expansion ioints between approach and deck are totally dama8ed
and need to be fixed.
(vi) Decking of M S. trough plate has been provided over which bituminous
premix has been provided which is damaged at few places and needs
immediate attention.
(vii) The spalling of concrete is taking place in towers at few locations which
need immediate attentlon
(viii) Greasing and painting of rollerl, anchoring system and ropes are required
' as same has not been done since the completion ofthe bridge
D) Anchorage Galleryi
(i) Anchorage gallery needs extra attention as it has been found that
openings in the gallery have been left open without providing grills and
thereby enhancing the chances of likely damage/sabotage of the
anchoring system of roPes.
Even opening provided for gate in inspection gallery on Udaipur side has
been left open which need to be closed immediately by providing suitable
gate.
Open holes has been left in the masonry of gallery on Udaipur side
probably for raising scaffolding during construction which has been left

(i) This is a stiffened suspension bridge on river Chenab (Chandra Bhaga) and is
the only connection between Killar and chamba from Chamba side
(iii The bridge is designed for IRC class B loading and accordingly information
board is required to be fixed on both sides specifying the load limjt of plying
vehicles over the bridge.
B) sub structure and Foundationi.
(i) Abutments of the bridge are intact and there is no sign of spalling or cracks
in the concreie. However, general maintenance like cutting of bushes and
clearing of weeds etc. is required to be done atleast twice ln a year before
the monsoon and after the monsoon which at present is not being done.
There is a pit in front of Killar side abutment where rain water accumulates.

This pit needs to be filled so that water is drained into the river and no
erosion/percolation of water takes place in front of the abutment.
{ii) The Abutments are founded on non-scour consideration with Safe Bearinp
Capacity at founding level of40 tonne/m'?.

Both towers are also intact and no sign of cracks on spalling noticed in theconcrete.
The truss ofthe brldge is riveted whereas suspenders are bolted. Steeltruss
and ropes has not been painted since the completion ofthe bridge and as
such the same is required to be painted with two coats of paint to prevent
rusting of steel and ensure the durability of the bridge.
Suspenders, ropes, wheel gaurd also needs to be painted to prevent thecorrosion of these members.
The soffit of the bridge also needs to be painted as erosion of memberhave started.
Some suspenders have started loosening and are not in vertical position.
This needs to be attended to immediately as it may endanger the safety ofthe bridge. Mechanical team need to visit site and carrv out necessary
repair immediately. Nuts, bolts and clamps also need to be checked
periodically by the mechanical wing. lt is recommended that Mechanical
wing should inspect such bridges atleast once in a year and issue thenecessary report accordingly.
Greasing of rollers and ropes, it seems have not been done since thecompletion of the bridge. As such it should be done immediately and should
form part ofthe routine inspectidn done dufingthe year.
The bridge decking is of M.S. trough plates over which wearing coat ofbituminous premix has been provided which is totally damaged. At some
places the trough plates also seem to have loosened their grip from thestiuctura members below the decking. The kerb is also damaged and need
to be repaired and re-fixed. One of the reasons for damaged wearing seems
to be the absence of proper drainage spout which is not allowing the freedrainage of water. These deficiencies need to be attended to immediatelv.
D)Anchorage Gallery:-
(i) The anchorage galleries are not at all maintained properlV as bushes andgrass have grown inside it and it seems that painting and greasing of ropes
and clamps have not been done. The galleries needs to be cleared of these

Anchorage gallery masonry work is damaged at few places which require
repair.
As such it is recommended to ensure that above points are

The piers & abutment are founded on scour consideration with safe bearing
capacity at founding level for A1 as 47.5 Tonnesfm2,lot A2 as 62 Tonnes/m'?,
for P1 as 52 Tonnes/m':and for P2 as 70 Tonnes/m'?. lt is advisable to ensure
the attaining of safe bearing capacity before laying of concrete for abutment
and pier which are yet to be started..
Cables used should conform to lS: i4268 and it is necessary to obtain a test
ceitificate to ensure that cables comply with the specification requirements.
The mix design being used as per structural design is M30 grade for sub
structure and M40 grade for super structure. The mix desiSn has been got
done from private agency which is required to be approved by
Em p loyer/ E ngineer.
As per the mix design shown at site the ratio for M30 grade is 1:1.81:3.28 by
weight for cement, fine aggregate & coarse aggregate with water cement
ratio of 0.41. The quantity of cement per m3 as per design is 376k8. Similarly,
for M40 grade the ratio of cement, fine aggregate and coarse aggregate is
1:!.4412.72 wlth waler cement ratio of 0.35 and cement to be used @
477.2kg/mr. The mixes of these proportion are required to be checked
independently in Circle Laboratory to ensure its authenticity.

6. It should also be ensured that mix design is revised in case there is change inthe source and quality of aggregates. Being rich mix special care is required tobe taken to ensure that proper water cement ratio which is a major factor forcontributing to durability of concrete is maintained and regular cube testingis done to ensure the quality. The quality assurance mechanism should bemade effective and for this the senior officers of the department should alsooversee that regular tests are being conducted and workmanship is ensured.The work has been delayed and only 51.53% of financial progress has beenachieved even after 1 year of stipulated date of completion. The reasonputforth by the field officers for delay is involvement of excessive dewaterinsin the foundation.
With the present place of work it seems that work can be completed in theextended tlme only if proper monitoring is done bV senior officers.It is suggested that a resource based working programme is obtained fromthe contractor and same is monitored effectivelV to ensuae the timelvcompletion of the work.

. OBSERVATIONS:-
This is very old PMGSY sanctioned road. The work was sanctioned in 2006-07 under
package number HP-09 116. The work was awarded to the Contractor but the
agreement was closed as forest clearance under FCA 1980 & gift deeds from private
land owners for transfer of land to HP.PWD was not obtained.
In the earlier tender, formation cutting was completed in first 4.3 km.
The tender for the balance work was awarded to the Contractor on 3O-10,2014 &
some work of formation cutting in portion 4/300 to 7/015 km has been done.
However, as per the present position the work is completely held up at site.
During the interaction with the representative of Contractor & Assistant Engineer
Kawar, it revealed that there is a dispute between the Contractor and the Department.
As per the version ofthe Contractor following are the reasons for his inability to
continue with the work:-
a) Non release of the payment by the Department for the work done.
b) Non availability of blasting material.
cJ Non availability of private land beyond 7/105 km.
As per the version of the Department the payment could not be released to the
Contractor as he has not followed the alignment given by the Department as a result
of which excessive steep gradient has resulted immediately beyond 4/300 km.
Regarding non availability of blasting material the version of the Department was that
the same is being provided to the Contractor at the store as per agreement.

5. As such there is a dispute which needs to be settled immediately as such old
sanctioned project cannot be continued to be halted for such a lonS time. Not only
this, the extension of this road beyond 8/00 km has been sanctioned in Package No.
HP 09 117 under the name, 'Jiskoon to lakha road' which also is awarded to the same
Contractor on 22-08-2017. This is also a tender awarded for balance work as earlier
agreement was closed due to non-availability of land. This Package No. HP-09 117
cannot be started till the Package No. HP-09-116 is completed atleast for formation
cutting. As such, resolving the dispute of Package No. HP-09-116 becomes all the more
important. As per site condition, the grade is required to be improved beyond 4/300

damaged which is also required to be restored. There is a village above this proposed
road, below which sliding is taking place where Breast Walls along the road of proper

issues so that work can be carried out at site and finished in some fixed time frame.

1.) lt was intirnated by Executive Engineer that Sarahan side abutment has been
founded on non-scour consideration with safe bearing capacity of 45 tonnes/nn'z
The leori side abutment ls also being founded on non-scour consideration with
safe bearing capacity of 20 tonnes/m'z. Since, the leori sid e abutment had just
started it is necessary that bearing capacity is ensured at founding level. The pier
which ls complete has been founded on scour consideration.
2.) The pier is prone to damage due to failing/flowing boulders of nalla whlch has
steep slope. As such, it is advised to get the pier protected by getting it jacketed.
3.) On Jeori side there is likelihood of hill slide just adjoining to the abutment and as
such protection of hill as advised at site is required to safeguard the protection
wall being provided ln front ofthe abutment.
4.) The desi8n for sub structure was available which provide concrete mix of M30
grade. The mix design has been got done fron'r private laboratory which ls
required to be approved by the Employer/Engineer.
5.) The structLrre design of super structure is required to be finalised in time.
6.) No Quality Control laboratory was available at site and as such no test could be
conducted at site. The tests of material are being done from Circle laboratory. Lt
is advised that proper Quality Assurance mechanism is pul in place which wlll
include setting up of field laboratory and carrying out regular tests at site. The
tests like workability of concrete, Cement Concrete cube, etc., should be
conducted at site and properly entered in a Quality Control register. The
frequency ofthe tests is also required to be ensured. Only those tests whlch
require high quality equipnrents whlch are difficult to setup at site should be got
done from NABL accredited laboratory or Government laboratories. lt should
also be ensured that rnix design is revised in case there is change in source and
quality of material based on which mix desiSn was evolved. As such, it ls

1) lt is a six span PsC girder bridge having total span of 174.30 meters.
2) The work has been delayed as the stipulated date ofcompletion of work was 02-02-
2017. There has been some delay due to some land dispute at site which is said tobe resolved now.
3) Extension is said to be granted up to 31-01-2018, but with the present pace ofwork,it seems unlikely that work will be completed by this date.
4) The work on Pier P-2 has not at all been started so far.
5) The shuttering work for super structure between Abutment A-2 and Pier P-5 has
been started.
6) As per the version of Executive Engineer complete structure design is available.
7) The material testing lab is available at site and tests are being performed at site.
8) The mix design has been got done from StrLrcture Engineering Group Department ofCivil Engineering ltT Roorkee on 29-07-2015 for M30, M35, M40 grade of concrete.

OBSERVATIONS:-
It has been found that the Quality control test of material are being done through
private empanelled labs but adequate Quality management system is not in place at
site.
For such project, material testing lab js required to be established at site so that
routine tests like gradation of aggregate, physical properties of material, workability
of concrete and compressive strength of concrete and other material are conducted
regularly at site. Only those tests which require high qualjty of equipments can be
outsourced to the NABL accredited empanelled lab houses.
Not only the routine tests, as mentioned above, are required to be conducted but a
record of such tests in Quality control register has to be maintained and duly
authenticated routinely by the supervising staff during their inspection.
Though the concrete mix design for M25 mix have been got done through private
agency but the mix design is not approved by the employer/engineer as per the
codal provision.
For achieving the above objective, it is essential that suitable Quality assurance
mechanism is put in place which can be done by establishing a material testing lab
at site, performing requisite number of tests, having record of such tests in Quality
control register and making the supervisioh effective.
Since the building is coming up on steep site slope, proper arrangement for drainage
of surface runoff and rainwater from the roof is required to be put in place.
It is advisable to provide properly designed rain water harvesting system which can
caterto some ofthe needs ofthe hospital.
The concrete finishing particularly in ramp area is required to be improved.
Executive Engineer was asked to send the sample of material to the Central Material
Testlng Laboratory for testing which has been received vide letter No.PW-MCSD-I-
New OPD/2017-891 Dated 06.09.2017. fhe test results are attached annexed-'A'
except for compressive strength of concrete which will be sent after 28 days.

OBSERVATIONS:-

OBSERVATIONS:-
It is found that the drainage system around the complex needs to be properly put in
place so that sLrrface water is properly channelized and does not pose any danger to
the complex.
The protection wall between cart road and the proposed building is required to be
raised as there seefBto be tendency to sliding of hill above cart road
The level of approach road need to be examined so that the surfaces run off does
not flow into the complex.
Properly designed rain water harvesting system is required to be put in place as the
water stored is proposed to be used for flushing oftoilets.
It has been found that sanitary pipes are being laid in the soffit of the slab which are
further proposed to be covered with suitable ceiling. lt has to be ensured that door
bends are provided for facilitating the clearing of these pipes in case of blockage.
The drainage pipes for draining of water for balcony has been taken out into the roof
around the balcony which need to be properly fixed to avoid any leakage which may
endanger ceiling below.
The finishing quality of concrete is required id to be improved particularly in the stair
portion.
It was found that only nominal concrete mix is being used for such important proiect
whereas design mix should have been used. Proportioning of concrete as per design
mix is important to ensure the workability of fresh concrete, strength of concrete
and durability of hardened concrete. water cement ratio is one ofthe important
factors contributing to the durability of concrete. Though certain test like gradation
of aggregate, physical property of aggregate have been got conducted from the
private lab which are conforming to the requirement as stated by the Executive
Engineer, however in the absence of material testing lab at site test are not
performed at site. lt is advisable to put in place a proper quality assurance
mechanism which will include establishing a site lab and performing, tesa like
gradation of aggregate, physical property of aggregate, workability, compressive
strength of concrete cube and brick etc. Only the tests which require sophisticated

of tests conducted at site are also required to be maintained in Quality Control
register which should be duly authenticated by the supervising staff.
9. For achieving above requirement, it is essential that concrete mix is designed and
only designed mix concrete is used for such projects.
10.The Executive Engineer was asked to send the sample of the material to Central
Materjal Testing Laboratory Shimla-1 but same has not been senfio far. tt is advised
to send the sample of material so that same could be tested in Central Material
Testing Laboratory and results conveyed accordingly.
The field officers are advised to ensure the above provisions regarding
Quality Assurance in the project.
