Current research projects:
Title: Does total intravenous anesthesia with propofol for greater than 4 hours produce occult biochemical evidence of the propofol infusion syndrome?
Role: Principal InvestigatorM01-RR00069, General Clinical Research Centers Program, NCRR, NIH
David M. Polaner, M.D. 21
Seed Grant for Translational Research, Department of Anesthesiology, UCDHSC
The major goals of this project are:1. To determine if subclinical alterations in cellular respiration develop during propofol-based total intravenous anesthesia in children, as evidenced by a rise in fatty acid intermediaries (C3, C44 and C5 acylcarnitine) in blood and analysis of respiratory chain enzymes and intermediaries in skeletal muscle.2. To determine if lactate levels and the lactate-pyruvate ratio are useful measurements for early detection of such changes.3. To determine the biochemical and bioenergetic nature of propofol infusion syndrome.4. To determine if specific genetic polymorphisms play a role in the risk of propofol infusion syndrome.
Title: Multi-center international study of the use and complications of pediatric regional anesthesia
Role: Principal Investigator; steering committee, chairman, Pediatric Regional Anesthesia Network
The major goals of this project are:1. To determine, using a web-based multicenter prospective investigation, the practice patterns of regional anesthesia in infants and children.2. To determine the nature and incidence of complications in pediatric regional anesthesia.3. To develop benchmark data with which institutions can compare local practices and outcomes.
Title: A multi-site RCT comparing regional and general anaesthesia for effects on neurodevelopmental outcome and apnoea in infants
Role: Co- Investigator (site Principal Investigator; with Mary Ellen McCann, Harvard/ Boston Children’s and Andrew Davidson, Melbourne, AU)NIH R01 HD061336-01A1; Thrasher Research Fund Grant; NIH 1RO1HD081303-01
The major goals of this project are:1. To determine whether different types of anaesthesia [regional versus general] given to infants undergoing inguinal hernia repair result in equivalent neurodevelopmental outcomes.2. To describe the frequency and characteristics of apnea in the post-operative period after both regional and general anaesthesia for inguinal hernia repair in infants, and determine the factors associated with apnoea.
Title: Cerebrospinal fluid pressure and acute mountain sicknessRole: Co- Investigator (with Robert Roach, PhD)
The major goals of this project are:1. To determine the role of elevated intracranial pressure in
the development of acure mountain sickness2. To more fully understand the fundamental pathophysiology
of acute mountain sickness

3. To understand where treatment interventions may be effective
4. To evaluate optic nerve sheath diameter, a non-invasive technique for evaluating intracranial pressure, in the setting of acute hypoxia
Title: Altitude-omics 2012Role: co- investigator; expedition physicianThe major goals of this project are:1. To determine the nature of specific gene expression that
correlates with performance and adaptation to very high altitude
2. To study the integrative physiology of human adaptation to hypobaric hypoxia at extremely high altitude
3. To serve as an expedition physician and monitor the health of expedition participants and provide care as needed.
4. Field studies were performed at an altitude of 5300m (17,300ft) on Mt. Chacaltaya, Bolivia during July-August 2012 on 24 volunteers from sea level.
Title: PediR: a multicenter registry of difficult intubation of infants and children
Role: Co-investigator (site PI)The major goals of this project are:1. To determine the incidence and nature of difficult intubation
in infants and children2. To determine the incidence and nature of complications of
difficult intubation3. To investigate the correspondence between difficult
intubation, successful techniques and specific syndromes4. To serve as a vehicle for quality improvent in pediatric
airway managementTitle: The effect of morning versus afternoon general anesthesia on
hospital discharge times following brain MRI Role: Co-investigator (PI: Judit Szolnoki)
The major goals of this project are:1. To determine if afternoon versus morning timing of
anesthesia disrupts the circadian cycle in a different phase2. To determine if time to discharge is different in children
receiving anesthesia in the morning versus afternoonTitle: Is caudal block associated with an increased incidence of
urethrocutaneous fistula formation following hypospadias repair in infants?Role: Principal InvestigatorThe major goal of this study is to determine if the administration of a caudal block is associated with an increased incidence of urethrocutaneous fistula formation in infants following single stage hypospadias repair.
Clinical interests:1. general pediatric anesthesia2. the pediatric airway, assisted ventilation in infants and
children3. anesthesia for pediatric transplantation4. anesthesia for pediatric spine surgery5. pediatric regional anesthesia6. acute pain management in pediatrics
