HOW TO APPLY
Grant requests must be submitted at least 60 days prior to the activity start date. The
following information is required for application submission:
Grant request letter (signed and printed on organization letterhead)
Activity agenda
Needs assessment with references
Learning objectives
Educational outcomes plan
Activity description
Honoraria policy (if accredited activity)
Grantor attendee policy
Firewall policy
W-9 form
Activity budget
Accreditation certificate
WHO MAY APPLY
The following types of organizations are eligible to submit requests for Allergan Medical
Education grants funding:
Accredited Providers
Medical or Professional Associations
Community Health Centers
Patient Associations or Advocacy Groups
Medical Education Companies
Universities or Colleges
Grant requests for accredited activities must be submitted by an accredited provider, even
if jointly sponsored by a Medical Education Company.
Allergan is unable to accept grant requests from individual physicians or groups of
physicians in private practice.
GRANT TYPES
Allergan considers medical education grant funding for the following:
• Medical Education Activities (Live, Print, Enduring)
• Fellowships (Submitted by an Academic Institution)
• In-Kind Product (Botox® Cosmetic, Botox® Therapeutic and Juvéderm®)
• Anatomical Models (Neurosciences and Urology Only)

HOW TO REGISTER AS A NEW USER
In order to be able to submit a grant request to the Allergan Grants Management website,
you must register as a user
1. Open the Allergan Grant Management Homepage and locate: Need a User ID
2. Complete all fields on the Registration pop-up window
3. Click “OK” after completing the form
After completing the registration information you will be sent your username and
password.
HOW TO ACCESS YOUR LEGACY GRANT:
Enter your User ID from the former grant management system and enter the following (password is case sensitive): Password1

UNABLE TO REGISTER
You may encounter difficulty with user registration.
1. Information entered as part of your registration was invalid. Please see the error message and re-enter correct information.
2. An email address or Username was entered during registration that has already been registered with the Allergan Grant Management System. Please choose a different Username or email address.

UNABLE TO LOGIN
You may be unable to log in to the Allergan Grant Management System. Your account may
have been disabled due to inactivity. Please re-register. If problems persist, contact the
Allergan Medical Education Department at [email protected] and request that
your account be reactivated.
FORGOTTEN USERNAME OR PASSWORD

You may request a new password by clicking on “Forgot my Password” on the top left side
of the Allergan Grant Management System login screen.
If you have forgotten your User ID or cannot login, please click on the Customer Support
link located at the top left of the login screen for assistance.
USERNAME/PASSWORD UPDATE
You may change your User Name and Password at any time by locating the “Change
Password” link on the left side of the Allergan Grant Management System homepage.
Please keep in mind the following parameters:
• Minimum Characters: 8
• Password Configurations: Minimum Characters: 8
• Current password cannot be reused as the new password
• Last 10 passwords cannot be reused as the new password
At least one character from three (3) of the following categories must be used in a
password:
• Upper-case letter
• Lower-case letter
• Number
• Upper and lower case
• Alphanumeric character
• Special character (e.g.!@#$%^&*?/)
PROFILE UPDATE
Click on “Update Profile” located on the top right-hand side of the Allergan Grant
Management System homepage. After you have logged in, your profile will appear in a pop-
up window, which will allow for changes and or updates.
Click "Save" to update the information entered, or "Cancel" to revert back to the
homepage.
TIME ALLOTTED FOR APPLICATION ENTRY
Your application session will time out after 60 minutes, and you will be prompted to re-
enter your Username and Password information.

User accounts lock out after five (5) invalid login attempts. If you are locked out of the
system after five (5) failed login attempts, you will need to request a new password.
