About This Project This project consists of many kinds of birth records, such as registers, birth certificates, physicians' records, reports of birth, affidavits, and so on, and provides rich vital record information. Some types of images you will encounter: Birth Certificate Birth Register Affidavit Report of Birth Card File Physician’s Record Each document that contains the name of a principal is to be indexed as follows: • If two documents on the same image refer to the same principal, the information
should be combined in a single record in the data entry area. The purpose of indexing is to direct a researcher to a certain image. So there is no reason to create two records for the same person on the same image.
• If two or more documents on different images refer to the same principal, each document needs to be indexed as a separate record. Researchers will be able to find every document pertaining to their ancestors.
The following slides show how images from this project would be indexed. First, a slide shows the entire image. Then, subsequent slides show more detail of individual records.

This image contains an affidavit on the left side which does not include the name of a principal and so would not be indexed and a document on the right side that would be indexed. There is no need to add an additional record and mark it blank for the document on the left.
