of improving lives with compassionate health care and
support services regardless of ability to pay remains
central to all that we do.
We know that staying healthy encompasses so much more
than a doctor’s visit for many of our neighbors. From bus
passes to get to a new job until that first paycheck comes;
to mentoring a single mother learning to care for her child;
to coaching a homeless veteran to self-sufficiency; to
helping a retiree obtain prescriptions he can’t afford, we
seek to care for the whole person.
We hope you enjoy learning more about some of these stories
through our 2012-2013 Annual Report. Thank you for your

HealthNet Northeast Grand Opening
In August 2013, HealthNet opened the HealthNet Northeast Health
Center inside the new Avondale Meadows Health & Wellness
Center on Indianapolis’ northeast side.
The first of its kind in the country, the Avondale Meadows Health
& Wellness Center marks the latest milestone in the 100-acre
neighborhood revitalization within the Meadows community. The $18
million, 70,000 square-foot, LEED-certified facility combines cutting-
edge urban health care with state-of-the-art fitness and education
programs that will be transformative for this neighborhood and the
broader Indianapolis community. In addition to HealthNet Northeast,
the facility includes the 32,000-square-foot Avondale Meadows
YMCA, United Northeast Community Development Corporation and
the Meadows Community Foundation.
The 18,000 sq. ft. health center provides adult primary care,
pediatrics, OB/GYN, social work services, integrated behavioral
health services, lab services as well as access to discounted
prescriptions. The health center will serve 7,000 patients at
capacity. Construction was made possible through a $4.95 million
gift from IU Health, including $1 million from the Methodist Health
Foundation’s Charles Lynn Trust.

Sixth Dental Location Opens
In April 2013, HealthNet’s sixth dental location opened in the IU
Health Methodist Medical Tower. Staffed by experienced pediatric
dentists and hygienists, the HealthNet Pediatric & Adolescent
Dental Center provides dental care for children of all ages,
including check-ups, cleanings, x-rays, extractions and restoration.
Services Expand in Fountain Square
In August 2013, HealthNet celebrated the expansion of HealthNet
Southeast Health & Dental Center in Fountain Square, so that the
health center can serve more patients. Thanks to a grant made
possible by the Affordable Care Act, HealthNet converted space
previously used for medical records storage and office space into
three new exam rooms and space for behavioral health services,
enabling the center to serve an additional 1,150 patients annually. The
46-year-old center provides more than 35,000 patient visits each year.
Access to Affordable Insurance
In July 2013, HealthNet was one of 19 health centers in Indiana
awarded a grant from the U.S. Department of Health & Human Services
to enroll uninsured Americans in new health coverage options made
available by the Affordable Care Act. HealthNet now has 27 trained
and certified staff to help area residents enroll in all available health
insurance programs. Clients can make an appointment at any HealthNet
health center or call the referral line (317-957-2676) to schedule an
appointment and apply for health insurance.
HealthNet West Expansion
Thanks to a New Access Point grant awarded by the U.S. Department
of Health & Human Services, HealthNet opened the 4,500-square-foot
HealthNet West Health Center in December 2012 in an area that has
seen a decrease in providers. The health center quickly exceeded
capacity within its first few months of operation. HealthNet launched
a $150,000 capital campaign in August 2013 to expand the center. The
outpouring of community support enabled renovations to begin on
the remaining 1,800 square feet. The expansion, opening in the spring
of 2014, will become a Women’s Health area and include 5 additional
exam rooms and a procedure room.

The demand for quality, affordable primary health care for the city’s uninsured and underinsured residents reached all time highs and led HealthNet to complete the 2012 fiscal year with a $1.5 million financial deficit. The new HealthNet West Health Center opened in December 2012 and HealthNet Northeast Health Center opened in August 2013, expanding access to care in both communities.
